Source text - English Adjuvant chemotherapy with cisplatin and gemcitabine versus chemotherapy at relapse in patients with muscle-invasive bladder cancer submitted to radical cystectomy: an Italian, multicenter, randomized phase III trial
Background: The purpose of the study was to evaluate the benefit of adjuvant chemotherapy (AC) versus surgery alone in patients with muscle-invasive bladder cancer (MIBC).
Patients and methods: One hundred and ninety-four patients with pT2G3, pT3–4, N0–2 transitional cell bladder carcinoma were randomly allocated to control (92 patients) or to four courses of AC (102 patients). These latter patients were further randomly assigned to receive gemcitabine 1000 mg/m2 days 1, 8 and 15 and cisplatin 70 mg/m2 day 2 or gemcitabine as above plus cisplatin 70 mg/m2 day 15, every 28 days.
Results: At a median follow-up of 35 months, the 5-year overall survival (OS) was 48.5%, with no difference between the two arms [P = 0.24, hazard ratio (HR) 1.29, 95% confidence interval (CI) 0.84–1.99]. Mortality hazard was significantly correlated with Nodes (N) and Tumor (T) stage. The control and AC arms had comparable disease-free survival (42.3% and 37.2%, respectively; P = 0.70, HR 1.08, 95% CI 0.73–1.59). Only 62% of patients received the planned cycles. A significant higher incidence of thrombocytopenia was observed in patients receiving cisplatin on day 2 (P = 0.006). A similar global quality of life was observed in the two arms.
Conclusion: The study was underpowered to demonstrate that AC with cisplatin and gemcitabine improves OS and disease-free survival in patients with MIBC.

Translation - Italian Chemioterapia adiuvante con cisplatino e gemcitabina rispetto a chemioterapia alla recidiva in pazienti con carcinoma vescicale muscolo-invasivo sottoposti a cistectomia radicale: studio italiano di fase III multicentrico randomizzato
Premessa: scopo del presente studio è stato quello di valutare il beneficio della chemioterapia adiuvante (adjuvant chemotherapy, AC) rispetto alla sola chirurgia nei pazienti con carcinoma vescicale muscolo-invasivo ( muscle-invasive bladder cancer, MIBC).
Pazienti e metodi: centonovantaquattro pazienti con carcinoma a cellule transizionali pT2G3, pT3-4, N0-2 sono stati assegnati casualmente all'osservazione (92 pazienti) o a quattro cicli di AC (102 pazienti). Questi ultimi sono stati ulteriormente randomizzati a ricevere gemcitabina 1000 mg/m 2 ai giorni 1, 8 e 15 e cisplatino 70 mg/m 2 al giorno 2 o gemcitabina come sopra più cisplatino 70 mg/m 2 al giorno 15 ogni 28 giorni.
Risultati: a un follow-up mediano di 35 mesi, la sopravvivenza globale (overall survival, OS) a 5 anni è risultata pari al 48,5%, senza differenze tra i due bracci [p = 0,24, hazard ratio (HR) 1,29, intervallo di confidenza (IC) al 95% 0,84-1,99]. Il rischio di mortalità è risultato significativamente correlato allo stato linfonodale (N) e alla dimensione del tumore (T). La sopravvivenza libera da malattia nei bracci di controllo e AC è stata comparabile (rispettivamente 42,3% e 37,2%; p = 0,70, HR 1,08, IC al 95% 0,73-1,59). Solamente il 62% dei pazienti ha ricevuto il numero di cicli pianificati. È stata osservata un'incidenza significativamente superiore di trombocitopenia nei pazienti che hanno ricevuto cisplatino al giorno 2 (p = 0,006). La qualità di vita globale osservata nei due bracci è stata simile.
Conclusione: lo studio non aveva una potenza sufficiente per dimostrare che la AC con cisplatino e gemcitabina migliora la OS e la sopravvivenza libera da malattia nei pazienti con MIBC.

PHARMACOLOGICAL PROPERTIES
Pharmacodynamic properties
XXXXXXX is an parasympatholytic alkaloid extracted from different genera of plants belonging to the solanaceae family, of which the best-known species is Atropa belladonna .
XXXXXXX acts by blocking the muscarinic receptors for acetylcholine, being a competitive antagonist of the neurotransmitter, both at a central and a peripheral level.
It is used in veterinary medicine in dogs and cats as the principal antidote in cases of overdose or poisoning by substances with an anticholinesterase effect (phosphoric esters, carbamates, organochlorides).
XXXXXXX is also administered as a coadjuvant in general anaesthesia, to contrast the bradycardia produced by any anaesthetic and to decrease bronchial secretions in cases of gaseous anaesthesia. It is an alkaloid which also has an antispastic effect on the muscles of the uterus, the gastrointestinal tract, the urinary bladder, the ureters, the choledochus and the bronchioles.
Pharmacokinetic properties
After administration by the parenteral route, the systemic effects of atropine are detectable after a few minutes and last for 3 to 6 hours. The highest concentrations of the active substance are found in the kidney, the liver, the gall-bladder and in the intestinal tract.
The cat, the rabbit and the rat can inactivate a reasonable quantity in the liver.
In man, the dog, the guinea-pig, the rat and the mouse, approximately 10% to 33% of the dose injected is excreted via the urine in unmodified form.
TARGET SPECIES: dogs and cats.
ADMINISTRATION ROUTE: subcutaneous, intramuscular or intravenous.
THERAPEUTIC INDICATIONS
XXXXXXX is indicated:
as an antidote in intoxications due to substances with an anticholinesterase effect (phosphoric esters, carbamates, organochlorides);
as a coadjuvant in general anaesthesia, to contrast bradycardia and bronchial secretions;
as a spasmolytic of gastrointestinal, urinary, uterine and bronchial muscles and the muscle of the choledochus;
as a cardiac stimulant in cases of sinus bradycardia or atrioventricular blockage;
as a respiratory stimulant and in reducing bronchial secretions.
CONTRAINDICATIONS
XXXXXXX is contraindicated in animals affected by glaucoma, as the mydriatic effect does not permit deflux of the aqueous humour.
It is also contraindicated in elderly patients with arhythmia or cardiac decompensation and it can slow the remission of symptoms in cases of intestinal atony.
POSOLOGY AND METHOD OF USE
XXXXXXX may be administered at a dose of
0.02-2 ml/kg b.w. (equivalent to 0.02-2 mg of XXXXXXX /kg b.w.)
The posology may be varied at the discretion of the Veterinary Surgeon: the lowest dose is to be used in preanaesthesia, while the highest is indicated in severe poisoning by substances with an anticholinesterase effect.
In cases of sinus bradycardia and atrioventricular blockage, it is recommended that administration be regulated to produce the desired effect, or every 6-8 hours until the effect is obtained.
In the case of poisoning by substances with an anticholinesterase effect, it is advisable to administer 1/4 of the dose by the intravenous route and the rest by the subcutaneous or intramuscular route every 6-8 hours, until the desired effect is obtained.
UNDESIRABLE EFFECTS
The blockage of the muscarinic receptors in various organs, which produces the presence of a dominant sympathetic or parasympathetic tone, is strictly linked to the quantity of XXXXXXX administered. In fact, the salivary glands are sensitive to low doses of active substance, while the vagolytic effect on cardiac muscle is obtained only with higher posologies.
Therefore, in certain more sensitive subjects, with the same posology different reactions may be observed, such as:
cardiovascular effects: tachycardia, ectopic complexes;
gastrointestinal effects: vomiting, paralytic ileum, dry mouth, abdominal distension;
urinogenital effects: urinary retention;
effects on the respiratory system: dryness of the respiratory tract.
USE IN PREGNANCY AND LACTATION
The placental barrier is permeable to the medicinal product, consequently its use is not recommended during pregnancy.
Moreover, as it crosses the mammary barrier, it can produce anticholinergic effects in the pup; consequently, it should not be used during suckling.
INTERACTIONS WITH OTHER MEDICAMENTS AND OTHER FORMS OF INTERACTION
The use of XXXXXXX is compatible with the use of anaesthetic substances normally used in the veterinary field, such as: gaseous anaesthetics, barbiturates, xylazine, ketamine, acepromazine. It potentiates the vagolytic effect of the latter.
XXXXXXX antagonises the action of metoclopramide.
OVERDOSE (SYMPTOMS, EMERGENCY PROCEDURES AND ANTIDOTES)
An excessive dose of atropine causes dry mouth, dysphagia, mydriasis, thirst, tachycardia, constipation, hyperpnea, muscle tremors, ataxia and excitation phenomena.
The antidotes to be used are anticholinesterase drugs such as physostigmine salicylate, used by the intravenous route at a posology of 0.1-0.6 mg/kg b.w..

PROPRIETÀ FARMACOLOGICHE
Proprietà farmacodinamiche
XXXXXXX è un alcaloide parasimpatolitico estratto da diversi generi di piante appartenenti alla famiglia delle Solanaceae, di cui la specie più nota è la Atropa belladonna .
XXXXXXX agisce bloccando i recettori muscarinici dell'acetilcolina, essendo un antagonista competitivo del neurotrasmettitore, sia a livello centrale che periferico.
È impiegato in medicina veterinaria nei cani e nei gatti come antidoto principale nei casi di sovradosaggio o di avvelenamento da sostanze con effetto anti-colinesterasico (esteri fosforici, carbammati, organoclorurati).
XXXXXXX è somministrato anche come coadiuvante nell'anestesia generale, per contrastare la bradicardia indotta da qualsiasi anestetico e per ridurre le secrezioni bronchiali in caso di anestesia gassosa. È un alcaloide che ha anche effetto antispastico su muscolatura uterina, tratto gastrointestinale, vescica urinaria, ureteri, coledoco e bronchioli.
Proprietà farmacocinetiche
In seguito alla somministrazione per via parenterale, gli effetti sistemici dell'atropina sono rilevabili dopo alcuni minuti e perdurano per 3-6 ore. Le concentrazioni più elevate di principio attivo si riscontrano nei reni, nel fegato, nella colecisti e nel tratto intestinale.
Gatti, conigli e ratti possono inattivarne una discreta quantità nel fegato.
Nell'uomo, nel cane, nella cavia, nel ratto e nel topo, una percentuale compresa tra il 10% e il 33% della dose iniettata è escreta attraverso l'urina in forma immodificata.
SPECIE DI DESTINAZIONE: cani e gatti.
VIA DI SOMMINISTRAZIONE: sottocutanea, intramuscolare ed endovenosa.
Indicazioni terapeutiche
XXXXXXX è indicato:
come antidoto per le intossicazioni da sostanze con effetto anti-colinesterasico (esteri fosforici, carbammati, organoclorurati);
come coadiuvante nell'anestesia generale, per contrastare la bradicardia e le secrezioni bronchiali;
come spasmolitico della muscolatura gastrointestinale, urinaria, uterina e bronchiale e del muscolo del coledoco;
come stimolante cardiaco in casi di bradicardia sinusale o blocco atrioventricolare;
come stimolante respiratorio e per la riduzione delle secrezioni bronchiali.
Controindicazioni
XXXXXXX è controindicato negli animali affetti da glaucoma, poiché l'effetto midriatico non permette il deflusso dell'umore acqueo.
È controindicato anche in pazienti in età più avanzata con aritmia o scompenso cardiaco e può rallentare la remissione dei sintomi in casi di atonia intestinale.
Posologia e modo di somministrazione
XXXXXXX può essere somministrato a una dose di
0,02-2 ml/kg p.v. (pari a 0,02-2 mg di XXXXXXX /kg p.v.)
La posologia può essere variata a descrizione del medico veterinario: la dose minima deve essere usata nella preanestesia, mentre la dose più alta deve essere somministrata in avvelenamenti gravi da sostanze con effetto anticolinesterasico.
Nei casi di bradicardia sinusale e blocco atrioventricolare, regolare la somministrazione per produrre l'effetto desiderato (somministrare ad effetto), ogni 6-8 ore fino al raggiungimento dell'effetto desiderato.
In caso di avvelenamento da anticolinesterasici, si consiglia di somministrare 1/4 della dose per via endovenosa e il resto per via sottocutanea o intramuscolare ogni 6-8 ore, fino all'ottenimento dell'effetto.
Effetti indesiderati
Il blocco dei recettori muscarinici in vari organi, che determina la presenza di un tono simpatico o parasimpatico dominante, è strettamente legato alla quantità di XXXXXXX somministrata. Infatti, le ghiandole salivari sono sensibili ai bassi dosaggi di principio attivo, mentre l'effetto vagolitico sul muscolo cardiaco si verifica solo con posologie più elevate.
Pertanto, in soggetti più sensibili, è possibile osservare reazioni diverse anche a parità di posologia, quali ad esempio:
effetti cardiovascolari: tachicardia, complessi ectopici;
effetti gastrointestinali: vomito, ileo paralitico, secchezza delle fauci, distensione addominale;
effetti genitourinari: ritenzione urinarie;
effetti sull'apparato respiratorio: secchezza delle vie respiratorie.
IMPIEGO IN GRAVIDANZA E LATTAZIONE
La barriera placentare è permeabile al prodotto medicinale, perciò il suo uso non è consigliato durante la gravidanza.
Inoltre, dato che attraversa la barriera mammaria, può produrre effetti anticolinergici nei cuccioli; di conseguenza non deve essere utilizzato durante l'allattamento.
INTERAZIONI CON ALTRI MEDICINALI ED ALTRE FORME D’INTERAZIONE
L'uso di XXXXXXX è compatibile con l'uso di anestetici normalmente utilizzati in campo veterinario, quali ad esempio: anestetici gassosi, barbiturici, xilazina, ketamina, acepromazina. Potenzia l'effetto vagolitico di quest'ultimo.
XXXXXXX antagonizza l'effetto della metocloprammide.
SOVRADOSAGGIO (SINTOMI, PROCEDURE DI EMERGENZA E ANTIDOTI)
Una dose eccessiva di atropina causa secchezza delle fauci, disfagia, midriasi, sete, tachicardia, costipazione, iperpnea, atassia e fenomeni di eccitazione.
Gli antidoti da utilizzare sono farmaci anticolinesterascici come fisostigmina salicilato, usati per via endovenosa con posologia di 0,1-0,6 mg/kg p.v.
PRECAUZIONI PARTICOLARI PER LA CONSERVAZIONE
Conservare al riparo dalla luce.

Healthcare professionals and other users testing multiple patients with this system should handle everything that comes into contact with human blood carefully to prevent transmitting infectious diseases, including sanitized objects
For patients with impaired peripheral circulation, collection of capillary blood from the approved sample sites is not advised as the results might not be a true reflection of the physiological blood glucose level. It may apply under the following circumstances: severe dehydration as a result of diabetic ketoacidosis or due to stress hyperglycemia, hyperosmolar non-ketotic coma, shock, decompensated heart failure NYHA Class IV or peripheral arterial occlusive disease.
Intended Use
ZZ test strips, when used together with XX and YY Blood Glucose Monitoring Systems, allow your blood glucose levels to be measured by yourself at home or by healthcare professionals. It uses fresh capillary whole blood samples from the finger, palm, and from venous, arterial and neonatal whole blood.
The GlicTest test strips allow AST to be performed on sites other than the fingertip. We strongly recommend that you perform AST ONLY at the following times:
During a pre-meal or fasting state (more than 2 hours since the last meal).
Two hours or more after taking insulin.
Two hours or more after exercise.

Le strisce reattive ZZ possono essere utilizzate per eseguire misurazioni in siti diversi dal polpastrello, tuttavia si raccomanda di ESEGUIRE PRELIEVI DA SITI ALTERNATIVI SOLO nei seguenti casi:
