大口先 is a major/prime client, so I think it should be 大口の薬剤部,
strictly speaking.

I guess 薬剤部 would be "pharmaceutical department", "pharmacy" etc.,
so 大口先の薬剤部 can be "pharmacy which is a major client" or
"pharmaceutical department / pharmacy with large transaction (volume)".

Maybe 大口先の薬剤部 means 大口先である薬剤部...difficult to tell without
context but can still be described like the above, I think.
