Source text - English INTRODUCTION
You are being asked to participate in a clinical research study of an investigational use of a drug called XX, which is described in this consent form. The purpose of this consent form is to give you the information you will need to help you decide whether or not to be in this study. Please read this form carefully. You may ask questions about what you will be asked to do, the risks, the benefits, your rights as a volunteer, or anything else about the research or this form that is not clear to you. When all of your questions have been answered to your satisfaction, you can decide if you want to be in this study or not. This process is called "informed consent." You will be given a copy of this form for your records.
Your decision to take part in this study is voluntary. You are free to either take part or not take part in this study.
PURPOSE OF THE STUDY
The purpose of the study is to see how safe, effective, and tolerable the use of XX is for type 2 diabetes when added to your current anti-diabetic therapy when taken for 52 weeks.
XX has been approved by the U.S. Food and Drug Administration (FDA) to treat high cholesterol but it has not been approved to treat diabetes. The word “investigational” means this use of XX is still being studied and is not FDA approved for sale in the United States for diabetes. The FDA allows XX to be used for diabetes only in research.
The study will be conducted at approximately 80 study centers in the United States. Approximately 780, male and female adult participants with type 2 diabetes will be in the study. The investigator or study center is being paid by the sponsor to conduct this study.
Because you have successfully completed your participation in a XX Pharma Development protocol, you are being asked to consider continuing on in this extension study.
If you are enrolled in this study:
• In addition to the study medication, you will continue to take your current anti-diabetic medication at the same time you have been taking it during the WEL study you just completed.
• You are required to attend all designated clinic appointments.
If, for any reason, the study medication is stopped before your final visit, you will be asked to return to the study site immediately.

Se le solicita que participe en una investigación clínica del uso investigacional de la droga denominada XX, la cual se describe en este formulario de consentimiento. El propósito de este formulario de consentimiento es brindarle la información necesaria para ayudarlo a decidir si va a participar o no en este estudio. Por favor, lea este formulario cuidadosamente.
