WHO’s policy package
Increasing numbers of bacteria are becoming resistant to antimicrobials and there is a need to take urgent action. On World Health Day, WHO introduced a policy package to combat antimicrobial resistance (Box 1). This package reframes the critical actions to be taken by governments to stimulate change by all stakeholders.
The first overarching component is for governments to commit to a comprehensive national plan against antimicrobial resistance that brings together all the required recommended measures. A national inter-sectoral steering committee should be established to guide actions by several stakeholders under the overall stewardship of the government. Adequate resources should be earmarked and an accountability framework should be set up, with measurable indicators to be reported annually. A well informed public is a catalyst to health actions; thus, building strong public awareness is vital. Civil society representatives should be involved in the development of antimicrobial resistance policies, their implementation and the monitoring activities.
A second component is strengthening surveillance and laboratory capacity. The surveillance of antimicrobial resistant organisms and the tracking of the use of antimicrobials are both essential. There is also an urgent need to build laboratory capacity to ensure reliable and rapid test results on which to base prescribing decisions and measures for the prevention and control of infections. Standard protocols are needed to assess antimicrobial resistance trends consistently over time and across geographical areas. Surveillance data should be regularly reported and shared at the regional and global levels. Antimicrobial resistance surveillance systems must be expanded to veterinary services and supported by food safety authorities to help assess the impact on human health of the use of antimicrobials in animals for human consumption.

La primera medida global consiste en que los gobiernos se comprometan a promover un plan nacional integral contra la resistencia a los antimicrobianos que reúna todas las medidas recomendadas que sean necesarias. Se debería establecer un comité directivo nacional intersectorial para guiar las acciones de los diversos participantes bajo la dirección global del gobierno, así como asignar los recursos necesarios y preparar un marco de rendición de cuentas con indicadores medibles para realizar informes anuales. Que los ciudadanos estén bien informados es un punto de comienzo necesario para las acciones públicas, por lo que sería de vital importancia sensibilizar a la opinión pública. Habría también que implicar a los representantes de la sociedad civil en el desarrollo, puesta en práctica y seguimiento de políticas contra la resistencia a los antimicrobianos.
La segunda medida implica reforzar el seguimiento y la capacidad del laboratorios. Tanto la vigilancia de los organismos resistentes a los antimicrobianos como el seguimiento del uso de estos últimos es esencial; así como es también urgente aumentar la capacidad del laboratorio para asegurar que los resultados de las pruebas sean rápidos y fiables, para basar en ellos las decisiones de prescripción y las medidas para la prevención y el control de infecciones. Se necesitan protocolos normalizados para evaluar las pautas de resistencia a los antimicrobianos a lo largo del tiempo y entre las zonas geográficas, y también es importante informar de manera regular sobre los datos de vigilancia y compartirlos tanto a nivel regional como global. Los sistemas de vigilancia de la resistencia a los antimicrobianos deben ampliarse a los servicios veterinarios y las autoridades de seguridad alimentaria deben apoyarlos para ayudar a evaluar el impacto que tiene en la salud humana el uso de antimicrobianos en animales destinados al consumo humano.

RED VINE LEAVES
