As to the services provided at the adult Day Care Center, the Appellant’s care plan, progress notes and planning tool (docs 1, 2 and 3) do not indicate the need for any skilled nursing services. In fact the progress notes refer to group exercise activities, reinforcement of proper body mechanics, card games and current event groups.
