There is no biologic reason to suggest that persons should increase their body weight as they age; there is more evidence to the contrary (Langseth, 1991). Energy restriction in genetically obese animals greatly increases longevity and slows signs of aging, even when animals remain obese.Typically, a 40% reduction in food intake below that of animals eating freely increases the length of life in rodents by 50% (Masoro, 1994). Studies on rhesus monkeys suggest that the antiaging effects of calorie restriction in rodents can also occur in longer-lived species (Lane et al, 1997). These studies strengthen the possibility that calorie restriction will also prove beneficial in humans.

ما من سببٍ بَيولوجِيٍّ يوحي بحتميّة ترافق زيادة الوزن البدنيّ للأشخاصِ مع تقدّمهم في العمر، بل ثمّة دليلٌ أوثق صلةً بنقيضِ ذلك، فبرأي (لانغسيث، 1991)، يزِيدُ التقييد الطاقِيّ للحيواناتِ البدينةِ جينيّاً (وراثيّاً) من طولِ عمرها كثيراً، ويُؤخّر من ظهورِ علامات شَيْخُوخَتها، ولو احتفظت ببدانتها. وفي الحالة النّموذجيّةً، يُطيلُ تخفيض 40% من مقدار مَدْخولُ الغذاءِ عند القوارض التي تتناول الطّعام بحريتها، بحدود 50% (ماسورو، 1994). كما تَقترحُ دِراساتٌ أجريت على نِسْناس الرِّيْسيوس، احتمال ظهور تأثيرات َتقييدِ السُعرات الحراريةِ المضادّةِ للهرم، في الفصائل الأطول عمراً، أيضاً، (لين وزملاؤه، 1997). وتعزّز هذه الدِراسات فرص إثبات فائدة تقييدَ السُعرات الحراريةِ لدى البشرِ أيضاً.

They are unlikely to confront me directly, but if they do I shall tell them the truth. I shall state my business. I’m planning to stop at Heathrow Airport until I see someone I know. (...)
Astonishingly, I wait for thirty-nine minutes and don’t see one person I know. Not one, and no-one knows me. I’m as anonymous as the drivers with their universal name-cards (some surnames I know), except the drivers are better dressed. Since the kids, whatever I wear looks like pyjamas. Coats, shirts, T-shirts, jeans, suits; like slept-in pyjamas. (...)
I hear myself thinking about all the people I know who have let me down by not leaving early on a Tuesday morning for glamorous European destinations. My former colleagues from the insurance office must still be stuck at their desks, like I always said they would be, when I was stuck there too, wasting my time and unable to settle while Ally moved steadily onward, getting her PhD and her first research fellowship at Reading University, her first promotion.
Our more recent grown-up friends, who have serious jobs and who therefore I half expect to be seeing any moment now, tell me that home-making is a perfectly decent occupation for a man, courageous even, yes, manly to stay at home with the kids. Bacteria play a major role in the development and
progression of pulp and periapical diseases, as shown
by many authors.1-4 Infections within the root canal
system of a tooth cause periapical inflammatory
responses which usually manifest as periapical
radiolucencies and occasionally as radiopacities on
radiographs.2,4 Bacteria also play a major role in the development of apical periodontitis associated with
root-filled teeth, although studies have shown that the
microflora differs in these teeth from that present when
there has been pulp necrosis with infection.5,6
Bacteria can exist within the root canal itself, or
within other related regions such as the dentinal
tubules, accessory canals, canal ramifications, apical
deltas, fins, and transverse anastomoses.7 Apart from
the canal itself, all of these other areas are inaccessible
to mechanical instrumentation procedures and to the
irrigating solutions used during endodontic treatment.
In order to predictably eliminate as many bacteria as
possible from the entire root canal system, a
combination of mechanical instrumentation and
irrigating solutions is used to remove or dissolve organic
and inorganic debris, to destroy bacteria, to remove the
smear layer and to maintain dentine permeability.7
However, several studies have shown that mechanical
instrumentation with antibacterial irrigation will only
render 50–70 per cent of infected canals free of microorganisms,
depending on which irrigants are used.2,8,9
Since there is no entirely predictable way, in one
treatment session, to ensure complete elimination of
root canal bacteria, an effective antimicrobial agent in the root canal is required for a predetermined time period to predictably eradicate or destroy any remaining bacteria.10,11 Therefore, antimicrobial agents used as inter-appointment medicaments must be able to penetrate through the dental tissues in the presence of microbes to reach a sufficiently high concentration in order to eliminate the disease-causing bacteria in a predictable manner.12-14

حسبما يرى معظم المؤلفون، تضطلع الجرائيم بدورٍ كبيرٍ، في تطوّرِ الآفات الذرويّة، والأمراض اللبيّة، وسيرورتها وإنذارها.1-4 فالإنتانات التي تصيب النظام القنوي الجذري للسنّ، تؤدي إلى رد فعل التهابي ذروي، وتتظاهر عادةً، بشفافيّة شعاعيّة، وأحياناً على هيئة ظلاليّة شعاعيّة، عند التصوير الشعاعي.2-4 كما تلعب الجراثيم أيضاً، دوراً لايقلُّ أهميّة عن سابقه، في تطوّر التهاب السن حول الذروي.
(أي الإنتان الذروي)، في الأسنان محشوّة الجذور، بالر غم من أنّ الأبحاث أظهرت، اختلاف الفلورا الجرثوميّة، في هذه الأسنان، عن مثيلاتها المصابة بالتموّت اللبي والإنتان،
ويمكن للجراثيم أن توجد في القناة الجذريّة تحديداً، أو في مناطق أخرى متّصلة بها، كالقنيّات العاجيّة، والأقنية الإضافيّة، وتشعبات القناة الأصلية، والدلتا الذروية، والنهايات والتفاغرات المستعرضة. 7 وباستثناء القناة بمفردها، يتعذّر الوصول للمناطق الأخرى، في سياق التهيئة الميكانيكيّة، أو سوائل الإرواء المستخدمة أثناء المداواة اللبيّة.
وللتخلّص من أكبر قدرٍ ممكنٍ من الجراثيم، على نحو يمكن توقّعه، من مجمل النظام القنيوي الجذري، يُلجأ إلى مشاركة من الاستخدام المكيانيكي للأدوات اللبيّة، وسوائل الإرواء، لإزالة الفضلات العضوية وغير العضوية، أو تفكيكها، بغية القضاء على الجراثيم، ولإزلة طبقة اللطاخة، والمحافظة على النفوذيّة العاجيّة. 7 وعلى كل حال، فقد أظهرت عدّة دراسات، أن التّهيئة الميكانيكية للقناة، المترافقة مع إرواءٍ مضادٍ للجراثيم، قد تعيد 50-70 بالمائة فقط من الأقنية المصابة بالإنتان لوضعٍ عقيم، ويختلف ذلك باختلاف نوع الإرواء المُستخدم.2,8،9 ونظراً لعدم إمكانيّة التكهّن بنتائج أية طريقة من الطرق على نحوٍ تام، في جلسةٍ علاجيّةٍ واحدةٍ، فلابدّ، لضمان التخلص التّام من جراثيم القناة الجذريّة، من وضع وسيطٍ فعّالٍ مضادٍ للجراثيم، وتركه في القناة الجذريّة، لمدّة تُحدد سلفاً، للقضاء على الجراثيم المتبقيّة، وإزالتها، على نحو يمكن الاطمئنان له.10،11 لذا، يجب أن تتّصف الوسائط المضادّة للجراثيم، التي تُستخدم كضمادات بين الجلسات، بالقدرة على النفوذ عبر الأنسجة السنيّة، بوجود الجراثيم، وأن يمكنها بلوغ تراكيز عالية بما يكفي، كي تزيل الجراثيم الممرضة بشكل يوافق التوقعّات المرجوّة منها. 12-14 الغزو الجرثومي للعاج
يتعلّق الغزو الجرثومي للنظام القنوي الجذري، بعامل الزّمن، ويعتمد على الأنواع والسلالات الجرثوميّة. لذلك، لاشكّ في تقليّل المداواة اللبيّة المبكرة للسن، من عدد الكائنات الحيّة الدقيقة، المستوطنة في القنيّات العاجيّة،15 التي قد تشكّل مستودعاً جرثوميّاً، يُسهم في إنتان القناة الجذرية، أوالأنسجة المحيطة بالسن، أو في نكس إنتاناتٍ سابقة. فهذه الجراثيم المقيمة في القنيّات العاجيّة، تبقىٍ بمنأى عن تأثير الخلايا الدفاعيّة للمضيف، والمضادات الحيوية الجهازيّة، والتهيئة الكيموميكانيكيّة للقناة الجّذريّة. ولذا، يجب أن يكون لدى الضمادات اللبيّة الدوائّية، القدرة على النفوذ إلى هذه القنيّات العاجية، والقضاء على الجراثيم الموجودة فيها.
عندما تغزو الجرائيم القنيّات العاجية، فهي لاتغزو جميع القنيّات بالدرجة نفسها، 16 وتظهر الدراسات السريرية، والمختبرية، بأنّ الاختراق الجرثومي للقنيّات العاجية، يحدث بطريقة عشوائية، وتُشاهد المستعمرات الجرثوميّة كتراكماتٍ كثيفةٍ، ومتقطعةٍ من الخلايا (عوضاً عن طبقة متواصلة)، متمدّدة خارج القناة الرئيسية نحو المحيط. 17 ويتراوح تواتر توثيق وجود الغزو الجرثومي للقنيّات العاجيّة، في الأسنان المصابة بالإنتان أو التموّت، بين 50 و90 بالمئة. 17
وعندما يتطوّر إنتان في القناة الجذرية، فسرعان ما ينتقل إلى طَليعَةُ العَاج، في حين لا يصاب العاج المُتَكَلِّس بذات السهولة. ويغلُب على الأنواع الجرثومية التي تخترق العاج العصيّات إيجابيّة الغرام (67 بالمئة) والمكوّرات (27 بالمئة). والأجناس السائدة هي العصيّات اللبنية (30 بالمئة)، والعقديات(1بالمئة)، والبرُوبْيُونِيَّات 9) بالمئة.18 وقد تأكّد وجود الجراثيم سلبية الغرام في عاج قناة الجذر، بشكل غير مباشر، باكتشاف التراكيز العالية لعَديد السَّكاريدِ الشَّحْمِيّ (LPS) في الطبقات الداخلية، بعمقٍ يصل إلى 300 µm.18 وأظهرت دراسة مفردة، 15 أن العِقْدِيَّةُ البِرازِيَّة، تلبث أسبوعين في فترة حضانةٍ، قبل أن تغزو القنيّات العاجيّة، في حين سلِم الملاط منها. وبحلول ثلاثة أسابيع، اخترقت الكائنات الحية أكثر من نصف سماكة العاج في الثلث العنقي للجذر، وحوالي منتصف سماكته في الثلث المتوسّط، وثلثها فقط في الثلث الذروي. وأظهرت العِقْدِيَّات البِرازِيَّة قدرتها على اختراق ما يتراوح بين 50 - 300µm من العاج البشري.19،20 كما وُثّق حدوث اختراق للعاج من أنواع جرثومية أخرى.16، 21- 25
