Clopidogrel is indicated in adults for the prevention of atherothrombotic events in:
Patients suffering from myocardial infarction (from a few days until less than 35 days), ischaemic stroke (from 7 days until less than 6 months) or established peripheral arterial disease.
Patients suffering from acute coronary syndrome:
- Non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), including patients undergoing a stent placement following percutaneous coronary intervention, in combination with acetylsalicylic acid (ASA).
- ST segment elevation acute myocardial infarction, in combination with ASA in medically treated patients eligible for thrombolytic therapy.

I, as the name mentioned above is the parents/guardian/husband/wife/relatives* to the patient as the name mention above, has been informed that the patient requires a blood or blood component transfusions. The medical practitioners who do the treatment has given explanation to me about the risks and the benefits of blood transfusions and I am satisfied will all the answers given to my questions. Mild complications and the rarely severe ones can occur during the blood transfusion or for a few days or after that. I understand and aware that even though the blood or the blood components had gone through pre-screening test for HIV, Hepatitis B, Hepatitis C and syphilis, but the risks of disease infections via blood transfusions can still occur. Moreover, other reactions can occur which include allergic reactions which might cause itchiness and fever. I also understand and aware that other complications of blood transfusions that cannot be prevented might occur as well.
