PHENOTYPIC TYPING METHODS
Early methods of typing Clostridium difficile were, of necessity, based on phenotypic properties such as antibiograms. In one of the first documented outbreak investigations, Burdon et al. [1] found a common resistance pattern to three antibiotics in isolates from cases on a surgical ward that were distinct from isolates in the rest of the hospital. However, this approach is at best only rudimentary, and a more detailed approach was tried by Wu¨st et al. [2] who combined plasmid analysis, soluble protein polyacrylamide gel electrophoresis (PAGE), immunoelectrophoresis of extracellular antigens and antibiograms to 16 isolates from related cases of C. difficile infection. Using these methods they showed that 12 of the 16 strains were indistinguishable, providing strong evidence that cross-infection had taken place.

U jednoj od prvih, dokumentovanih ispitivanja širih razmera, Burden i saradnici (1) došli su do opšteg obrasca rezistentnosti na tri antibiotika kod izolata uzetih od pacijenata sa hiruškog odeljenja koji su bili odvojeni od ostalih pacijenata u celoj bolnici. U suštini, ovaj način je najbolji samo u svojoj osnovi, a mnogo detaljniji pristup izveli su Wüst i saradnici (2) koji su kombinovali analize plazmida, polyacrylamide gel elektroforezu (PAGE) na rastvorljive proteine, imunoelektroforezu vanćelijskih antigena i antibiograme 16 izolovanih pacijenata sa sličnom dijagnozom infekcije Clostridium difficile. Korišćenjem ovih metoda oni su pokazali da je 12 od 16 sojeva identično, što je bio jak dokaz da se radilo o unakrsnoj (međusobnoj) infekciji.

Importance to the field: Acamprosate, marketed under the brand name Campral, (Forest Pharmaceuticals, Inc., Saint Louis, MO, USA; Merck Sante s.a.s., Lyon, France) is an orally administered drug approved in the US and throughout much of the world for treating alcohol dependence. Its safety and efficacy have been demonstrated in a number of clinical trials worldwide and as with all pharmacotherapies for alcoholism, it is used in conjunction with psychosocial interventions.
Areas covered in this review: This article reviews the mechanism of action, clinical efficacy and safety of acamprosate in Phase I, II and III randomized controlled trials involving healthy and alcohol-dependent populations using published reports from 1984 to 2009.
What the reader will gain: This review provides an update of the mechanism of action and the safety and efficacy profile of acamprosate. Take home message: Acamprosate appears to act centrally to restore the normal activity of glutamatergic neurotransmission altered by chronic alcohol exposure. Acamprosate’s excellent safety profile along with several pharmacokinetic and pharmacodynamic characteristics make it well suited for treating a broad population of alcohol-dependent patients.

Značaj u praksi: Akamprost, prisutan na tržištu pod nazivom Campral®, (Forest Pharmaceuticals, Inc., Saint Louis, MO, USA; Merck Sante s.a.s., Lyon, France) je lek za oralnu upotebu odobren u SAD i većem delu sveta za lečenje zavisnosti od alkohola. Njegova bezbednost i efikasnost dokazane su brojnim kliničkim studijama širom sveta i, kao što je to slučaj kod ostalih farmakoterapija za lečenje alkoholizma, upotebljava se u kombinaciji sa psihosocijalnim tretmanima.
Oblasti pokrivene ovom studijom: Ovaj rad opisuje mehanizme delovanja, kliničku efikasnost i bezbednost akamprost u fazama I, II i III randomizovanih kontrolisanih studija sprovedenih na zdravim osobama i osobama zavisnim od alkohola na osnovu objavljenih radova od 1984 do 2009.
Šta će čitalac saznati: Ova studija nudi dodatne informacije o mehanizmu delovanja, bezbednosti i efikasnosti akamprosta. Važne napomene: Izgleda da Akamprost deluje centralno kako bi normalizovao aktivnosti glutamatske neurotransmisije poremećene hroničnim dejstvom alkohola. Pored izvrsnog bezbedonosnog profila, Akamprost poseduje i nekoliko farmakokinetičkih i farmakodinamičkih karakteristika koje ga čine jako pogodnim za lečenje velike populacije pacijenata zavisnih od alkohola.

As adiponectin is produced exclusively by the adipocyte, it is not surprising that adiponectin concentrations in the serum are affected by changes in adipose tissue mass. However, unlike most adipose-derived hormones and secreted proteins, adiponectin mRNA and serum levels are decreased in obesity [10, 11]. In addition to obesity, the inverse relationship between adiponectin and body weight is upheld in extremely lean subjects such as those with anorexia nervosa [12, 13]. In support of these findings, cross-sectional studies show an inverse relationship between adiponectin mRNA and/or serum levels and body mass index (BMI) [11, 14]. However, there is an even stronger inverse relationship between adiponectin and fat mass (assessed by bioelectrical impedance or computerized tomography) than there is between adiponectin and BMI [15].
