Background
Level of insight and its relationship to clinical variables and neurocognitive functions was assessed in bipolar I patients.
Methods
Verbal memory, executive functioning, sustained attention, general intelligence and other neurocognitive functions were compared between 37 chronic in- and outpatients and 31 matched normal controls. Detailed psychiatric interviews were completed to define the level of symptomatology and psychosocial functioning. Insight was assessed by the Scale to Assess Unawareness of Mental Disorder (SUMD).
Results
Seventy percent of the patients were classified as having impaired insight. Prevalence of impaired insight was 47% and 94% in remitted and symptomatic patients, respectively. Symptomatic patients scored significantly below remitted patients on insight and neurocognition. Illness and symptom unawareness were related to overall level of symptoms, measures of memory, conceptual ability and right hand psychomotor speed and accuracy. Misattribution of symptoms and signs was correlated to visuomotor speed and visuospatial performance in addition to affective symptoms and thought disturbance.
Conclusions
Our findings support the hypothesis that impaired insight and other neurocognitive dysfunctions were present in a large percentage of cases among symptomatic as well as remitted bipolar patients. This may be of clinical relevance and raises important questions about the course and outcome of the illness.

Nivel de percepción, funcionamiento ejecutivo, atención sustancial, inteligencia general y otras funciones neurocognitivas fueron comparadas entre 37 pacientes internos crónicos y pacientes ambulantes y 31 encajaron controles normales. Detalladas entrevistas psicológicas fueron completadas para definir el nivel de sintomatología y funcionamiento psicosocial. Esto debe ser de relevancia clínica y eleva preguntas importantes sobre el curso y resultado de la enfermedad.

Vietnam se empieza a perfilar como un país con un probable futuro como productor de minerales de interés gemológico. Recientes estudios geológicos indican un alto potencial de riqueza mineral en numerosos yacimientos en cualquier parte del país: berilo (aguamarina) y topacio en Thuong Xuan, jadeíta y actinolita (nefrita) en Co Phuong, berilo y cuarzos en Thach Khoan, corindón (rubí) en Quy Chau y corindón (zafiro) en las provincias de Dak Lak y Binh Thuan.
El yacimiento que nos ocupa está situado en la provincia de Yen Bai, distrito de Luc Yen, con capital en la ciudad del mismo nombre, a unos 180 km al noroeste de Hanoi. La pequeña ciudad de Luc Yen está situada todavía en la llanura, pero junto a la cabecera del valle, rodeada de cerros puntiagudos recubiertos de una espesa vegetación de intenso color verde. Desde este último lugar se divisan ya algunos puntos de color blanco que rasgan el verdor de las faldas de las montañas. Subiendo a lo largo del río Rojo, y ya penetrando en la montaña valle arriba, a unos 8 o 10 km, se llega a la aldea de An Phu. La que se conoce como mina de An Phu es en realidad unos arañazos aquí y allá en diferentes puntos, siguiendo la inclinación de las montañas. La extracción de los minerales se efectúa de forma poco organizada, y con técnicas rudimentarias, por gente del pueblo que trabajan ocasionalmente. El depósito mineral de la región de Luc Yen está formado dentro de un mármol recristalizado a moderada alta temperatura de la época del Proterozoico superior-Bajo Cambriano, en la orilla este del río Rojo, en la zona tectónica de Lo Garn.
Como principales minerales de formación primaria en este depósito, aparecen, en cristales diseminados dentro del mármol, flogopita, dravita, pirita, rutilo, espinela, edenita, margarita y pargasita. Los depósitos secundarios son concentraciones de gravas dentro de hoyos kársticos llenados por aluviones de los valles que rodean Luc Yen, y al parecer de estos aluviones provienen los mejores rubíes (corindón rojo), zafiros (corindón azul) y espinelas con calidad gema. Dado que es material rodado, carecen de interés coleccionístico.
Si bien los habitantes del país conocían de antiguo la existencia de estos minerales, por hallazgos que habían hecho en el transcurso de los trabajos agrícolas habituales, no fue hasta el año 1987 que se empezó la recolección de los placeres y las primeras labores mineras.
Los principales minerales citados en el conjunto de canteras abiertas en An Phu son: condrodita, corindón (rubí y zafiro), elbaíta, espinela, fluorcannilloíta, humita, liddicoatita, y pargasita. Hace falta mencionar que la fluorcannilloíta, inosilicato del grupo de los anfíboles próximo a la pargasita, de fórmula CaCa2Mg4Al[(F,OH)2/Al3Si5O22], a veces se encuentra etiquetada como proveniente de Yuang Yang (o Yueng Yang), Wenshan, provincia de Yunnan (China); pero muy probablemente procede en realidad de An Phu, debido a que algunos comerciantes chinos ofrecen minerales originarios del Vietnam.

The idea of Vietnam as a country with a potential future as a producer of minerals with a gemological interest begins to take shape. Recent geological studies have suggested a high potential of mineral wealth in a good number of beds in many spots in the country: beryl (aquamarine) and topaz in Thuong Xuan, jadeite and actinolite (nephrite) in Co Phuong, beryl and quartz in Thach Khoan, corundum (ruby) in Quy Chau and corundum (sapphire) at the provinces of Dak Lak and Binh Thuan.
The bed in question here is located in the province of Yen Bai, whose capital bears the same name, Luc Yen district, at about 180 km. from Hanoi (north-west). The small town known as Luc Yen sits on a plain near the valley’s head, surrounded by pointed hills covered by a dense, intensely green vegetation. From this last spot, some white points can be observed, which rip up the greenness of the mountains’ slopes. If you travel along the Red river, and head up the mountain, at a distance of 8-10 km., you reach the village of An Phu. What is known as An Phu mine is in fact a couple of scratches here and there in different spots, following the slope of the mountain. The mineral extraction is hardly organized and the used techniques are rather primitive, the task being performed by people in the town that work only occasionally. The mineral deposit in the Luc Yen area is formed within a re-crystalized marble at a moderate high temperature from the times of the Paleoproterozoic-Early Cambrian, on the east shore of the Red river, at the tectonic area of Lo Garn.
The main primary formation minerals appearing in this deposit, in crystals disseminated inside the marble, are flogopite, dravite, pirite, rutile, spinell, edenite, margarita and pargasite. The secondary deposits are gravel concentrations inside karst holes filled by alluvions from the valleys surrounding Luc Yen, and apparently these alluvions originate the best rubies (red corundum), sapphire (blue corundum), and spinells of gem quality. Since it is rolling material they are of no interest for collectors.
Although the country’s inhabitants have known since antiquity of the existence of valuable minerals in the area, through discoveries turned up by farmers, the first mining labor and collections only began by 1987.
The main minerals mentioned in the group of open quarries at An Phu are: condrodite, corundum (ruby and sapphire), elbaite, spinell, fluorcannilloite, humite, liddicoatite and pargasite. It is necessary to mention that the fluorcannilloite (inosilicate belonging to the group of amphiboles close to pargasite, with formula CaCa2Mg4Al[(F,OH)2/Al3Si5O22]) is sometimes labeled at Yuang Yang (or Yueng Yang), Wenshan, province of Yunnan (China) as place of origin; but it is very likely that it comes from An Phu, since some Chinese businessmen offer minerals brought from Vietnam.
