Source text - English WHAT IS THE PURPOSE OF THE STUDY?
The purpose of this study is to complete our clinical knowledge on a new biologic disease-modifying anti-rheumatic drug: XXX. Intravenous XXX has been approved in EU in patients with active moderate to severe Rheumatoid arthritis and inadequate response to treatment to at least one anti-TNF agent (infliximab, etanercept, adalimumab). The use of XXX remains experimental for other diseases. The main objective of this study is to evaluate if XXX in combination with background methotrexate therapy has an early effect on the joint inflammation of your finger joints of your hands using an innovative imaging technique called Power Doppler Ultrasonography. Power Doppler Ultrasonography is a quick, easy, safe technique which is also more sensitive than X-rays. It uses sound waves to study and treat hard to reach body areas. It provides the ability to study quickly the site of inflammation of your joints (synovitis), the presence or absence of synovial fluid in your joints related to inflammation. Moreover, Power Doppler signal will study the vessels flow in the synovium of your joints.
WHY HAVE I BEEN INVITED?
You have been invited to participate in this study because you have active rheumatoid arthritis, and you have had a poor response to methotrexate.
You could benefit by receiving an investigational drug that blocks inflammation and slows down the joint damage. Although you may meet all the criteria for participation, you may not be enrolled to participate in the study. The study doctor or staff will discuss these with you. About 100 subjects in total will take part in the study.
WHAT WILL HAPPEN TO ME IF I TAKE PART?
You will be in the study for 6.5 months during which time you will be required to visit your study doctor at certain period of time during the course of the study. Below is a description of the visits that you will be asked to make.
Screening visit:
These screening tests include:
• A medical and medication history.
• Physical examination, to include measurement of your height, weight and vital signs (heart rate, blood pressure, temperature and breathing rate).
• A breast screening/examination which may include a mammogram. Your study doctor will explain which test is appropriate for your age and riskfactor( s).
• Collection of blood and urine samples for routine laboratory testing including testing for Hepatitis B and C.
• A tuberculosis screening skin test, unless you have one performed within the past 6 months before screening.
• A chest x-ray, unless you have had one in the past 6 months and documentation is available.
• If you are a woman of childbearing potential, a pregnancy test will also be done and a sample of urine or blood will be collected for this purpose before the first dose study medication. The results of the pregnancy test should be negative for you to participate in the study.
WHAT ARE THE ALTERNATIVES TO TAKING PART IN THIS STUDY?
You have the option not to participate in this study. If you decide not to participate in this study, there are other symptomatic treatments for arthritis such as non-steroidal anti-inflammatory drugs (NSAIDs), glucocorticoids. In addition there are also non biological disease modifying arthritis drugs (DMARDs) such as gold, sulfasalazine, chloroquine, hydroxychloroquine, D-penicillamine, azathioprine, leflunomide and cyclosporine. Your doctor can prescribe one that is appropriate for you. Your study doctor can discuss any options for treatment that may be relevant for you.
Translation - Italian QUAL È LO SCOPO DELLO STUDIO?
Lo scopo di questo studio è perfezionare le nostre conoscenze cliniche relative a un nuovo farmaco biologico antireumatico modificante la malattia: XXX. XXX per infusione endovenosa è stato approvato nell’UE per l’uso nei pazienti con artrite reumatoide da moderata a grave e insufficiente risposta clinica al trattamento con almeno un agente anti-TNF (infliximab, etanercept, adalimumab). L'uso di XXX rimane sperimentale per altre malattie. L'obiettivo principale di questo studio è valutare, mediante una tecnica innovativa di imaging chiamata Eco Power Doppler, se XXX, somministrato in combinazione con terapia di fondo con metotressato, possa avere un effetto precoce sull’infiammazione delle articolazioni delle dita delle Sue mani. L’Eco Power Doppler è una tecnica rapida, semplice e sicura, ancora più sensibile dei raggi X. Essa utilizza le onde sonore per lo studio e il trattamento di aree del corpo meno accessibili. Consente inoltre di studiare velocemente il sito d’infiammazione delle Sue articolazioni (sinovite) e la presenza o l'assenza di fluido sinoviale nelle Sue articolazioni nelle quali è presente infiammazione. Inoltre, il segnale Power Doppler esaminerà i flussi vascolari a livello sinoviale nelle Sue articolazioni.
PERCHÉ SONO STATO/A INVITATO/A?
Lei è stato/a invitato/a a partecipare a questo studio perché soffre di artrite reumatoide attiva e ha mostrato una risposta insoddisfacente al metotressato.
Potrebbe trarre beneficio da un farmaco sperimentale che blocca l'infiammazione e rallenta il danno articolare. Qualora dovesse soddisfare tutti i criteri per la partecipazione, è possibile, tuttavia, che non venga arruolato/a per la partecipazione allo studio. Il medico o il personale dello studio ne discuterà con Lei. Prenderanno parte allo studio circa 100 soggetti in totale.
CHE COSA COMPORTERÀ LA MIA PARTECIPAZIONE?
Parteciperà allo studio per 6 mesi e mezzo, durante i quali, in un determinato momento, Le verrà chiesto di recarsi presso il medico dello studio di riferimento. Di seguito è fornita una descrizione delle visite alle quali Le verrà chiesto di sottoporsi.
Visita di screening
I test di screening sono i seguenti:
• storia medica e farmacologica;
• esame fisico, che include la misura della Sua altezza, del Suo peso e dei Suoi segni vitali (frequenza cardiaca, pressione arteriosa, temperatura e frequenza respiratoria);
• screening/esame mammografico, che può includere una mammografia. Il medico dello studio di riferimento spiegherà quale esame è appropriata per la Sua età e il Suo o i Suoi fattori di rischio;
• prelievo di campioni di sangue e di urine per esami di laboratorio di routine, compresi i test per l'epatite B e C;
• test cutaneo di screening della tubercolosi, a meno che non ne sia disponibile uno eseguito nel corso degli ultimi 6 mesi prima dello screening;
• radiografia al torace, a meno che non vi sia già stato/a sottoposto/a negli ultimi 6 mesi e ne sia disponibile la documentazione;
• se Lei è una donna in età fertile, verrà sottoposta anche a un test di gravidanza e a questo scopo Le sarà prelevato un campione di urine o di sangue prima della somministrazione della prima dose del farmaco in studio. Per poter partecipare allo studio i risultati di questo test di gravidanza devono avere esito negativo.
QUALI SONO LE ALTERNATIVE ALLA PARTECIPAZIONE A QUESTO STUDIO?
Lei non è obbligato/a a partecipare a questo studio. Qualora decida di non partecipare al presente studio, sono disponibili altri trattamenti sintomatici per l'artrite, come antinfiammatori non steroidei (FANS) e glucocorticoidi. Inoltre sono disponibili anche farmaci antireumatici modificanti la malattia (DMARD) non biologici, quali sali d'oro, sulfasalazina, clorochina, idrossiclorochina, D-penicillamina, azatioprina, leflunomide e ciclosporina. This chapter only provides guidelines to follow.
Specific information is provided in the Installation Data Package and should be followed before installation of ****** medical linear accelerators.
****** is not responsible for any damage or injury that occurs because these guidelines were not followed.
Planning Operations
To prepare for the safety of patients and staff in the treatment room and surrounding areas, the owner is responsible for the installation planning and installation of certain emergency and safety equipment.
EMERGENCY OFF Buttons
Provide sufficient EMERGENCY OFF buttons in the treatment room and console area.
These buttons should complement the EMERGENCY OFF buttons supplied by ****** on the dedicated keyboard, on both sides of the treatment couch, and on both sides of the drive stand.
For additional information about the location of EMERGENCY OFF buttons, refer to National Council on Radiation Protection and Measurements (NCRP) Report Number 51.
Main Circuit Breaker
Install the main facility circuit breaker within 3 meters (10 feet) of the control console.
The main circuit breaker for the accelerator should have an undervoltage trip that causes the main breaker to trip if an EMERGENCY OFF button is pressed.
Ventilation and Temperature Regulation
Maintain adequate room ventilation.
Heat and air conditioning should be available to maintain the accelerator at room temperature (18°–21° C / 65°–70° F).
Fire Extinguishers
Provide suitable fire extinguishers in the treatment room and near the control console.
In the United States, the type of extinguisher must be approved for electrical fires by federal, state, and local codes and regulations.
Radio Frequency and Electromagnetic Interference
Ensure that nearby rooms with RF-emitting equipment have proper room shielding to prevent leakage and door interlock switches to prevent operation with doors open.
Adopt operating procedures that minimize leakage potential.
Electromagnetic interference (EMI) can disrupt the operation of electronic equipment and medical devices, and can interact with the accelerator.
Take precautions to avoid EMI.

Stabilire procedure operative che riducano al minimo la possibilità di dispersioni.
Le interferenze elettromagnetiche possono disturbare il funzionamento di apparecchiature elettroniche e dispositivi medici e, di conseguenza, influire sull’acceleratore.
