Translation - English Immigration Medical Exam Report
The patient presented to the clinic for the mandatory United States permanent residency application medical examination. During the initial consultation, the patient indicated he has no history of communicable diseases, recent hospitalizations, nor major surgeries within the last five years. He further denied respiratory symptoms, persistent fever, and unintentional weight loss. The family medical history did not reveal hereditary conditions relevant to the migration process.
A general physical examination was conducted that included a cardiovascular, respiratory, and neurological evaluation. The vital signs were within normal limits for the age and physical condition of the patient. The vaccination review showed that immunizations were not current; therefore, the appropriate influenza and tetanus-diphtheria vaccines were administered in accordance with current CDC guidelines.
