Antigen used is a total extract from P. falciparum cultivation enriched with recombinant antigens from P. vivax, allowing detection of antibodies to P. falciparum and P. vivax. Due to antigenic similarity between Plasmodium species, P. ovale and P. malariae antibodies can be detected as well. Unbound material is removed during a washing step and peroxydase (HRP)-linked antibodies directed against human IgG and IgM are used in a second incubation step. Excess HRP-conjugate is washed off and final incubation with an HRP substrate solution based on tetramethylbenzidine (TMB) and hydrogen peroxide (H2O2) leads to a blue coloration directly correlated to the antibody concentration in the patient’s specimen. 2. SUMMARY
Oral PD-0332991 is a highly selective reversible inhibitor of cyclin-dependent kinases (CDK) 4 and 6 that is being studied for use in the treatment of cancer.
PD-0332991 inhibits purified CDK4-catalyzed phosphorylation of retinoblastoma protein with an IC50 of less than 20 nM, and also tumor growth of several types of human xenograft tumors (SF-295, MDA-MB-435, Colo-205, and others), grown in SCID mice. Estimated steady-state plasma concentrations of 1000 ng/mL resulted in 80% to 90% inhibition of phospho-Rb formation and 50% reduction of tumor growth. Reduction in phospho-Rb was rapidly reversible as plasma PD-0332991 concentrations declined.
In nonclinical species (rat, dog, and monkey), PD-0332991 exhibits low to moderate plasma clearance, large volume of distribution, and moderate oral bioavailability ranging from 23% to 56%. Plasma protein binding of PD-0332991 is moderate in mouse, rat, dog, and human plasma. PD-0332991 shows a preferential distribution to red blood cells over plasma in humans, but similar distribution between red blood cells and plasma in nonclinical species. Radioequivalents were widely distributed to most rat tissues and fluids following an oral dose of [14C]PD-0332991, with radioactivity levels consistently greater than those observed in blood. PD-0332991 is metabolized to multiple metabolites in a qualitatively similar manner in rat, dog, and human liver microsomes. In vitro, PD-0332991 is primarily metabolized by CYP3A enzymes. Following oral administration of [14C]PD-0332991 to intact and bile duct-cannulated rats, the metabolic pathways involve oxidation, glucuronidation, sulfation, and combinations of these reactions. In rats, [14C]PD-0332991 was mainly eliminated via the feces; the high fecal elimination occurred via biliary excretion. PD-0332991 and its oxidative metabolite, PF-05089326, demonstrated little or no inhibition of CYP1A2, CYP2B6, CYP2C8, CYP2C9, CYP2C19, or CYP2D6 enzyme activities and thus, showed low potential for CYP-mediated pharmacokinetic drug interactions. However, PD-0332991 and PF-05089326 caused time-dependent inhibition of CYP3A midazolam 1’- hydroxylase and testosterone 6?-hydroxylase activities and may have the potential for pharmacokinetic drug interactions with compounds for which CYP3A-mediated metabolism constitutes the primary mechanism of clearance. PD-0332991 did not cause induction of CYP1A2, CYP2B6, CYP2C8, or CYP3A4 mRNA expression or enzyme activity in vitro in human hepatocytes; thus, the potential for PD-0332991 to induce these enzymes is considered to be low at clinically relevant concentrations. In vitro, PD-0332991 inhibits P-glycoprotein (P-gp) and breast cancer resistance protein (BCRP) and may have the potential to affect the absorption and/or pharmacokinetics of drugs that are substrates of these efflux transporters.
PD-0332991 had no effect on heart rate, rhythm, or blood pressure in dogs given single oral doses of 10 and 30 mg/kg (200 and 600 mg/m2, respectively); however, evidence of QT prolongation was noted at both doses. Corresponding Cmax and AUC(0-24) values associated with these doses were ?511 ng/mL and ?8800 ng•h/mL, respectively. PD-0332991 inhibited hERG current in HEK293 cells and increased action potential duration at 90% repolarization in Purkinje fibers consistent with QT prolongation observed in vivo.
Significant and transient pulmonary effects occurred in anesthetized dogs given a single IV dose of PD-0332991 at 5 mg/kg (100 mg/m2). Associated observed peak plasma drug concentrations were ?2040 ng/mL. These effects were consistent with centrally mediated respiratory depression. No changes in pulmonary function were noted in dogs with observed peak plasma concentrations ?414 ng/mL at 1 mg/kg (20 mg/m2).
In pivotal repeated-dose toxicology studies, PD-0332991 was lethal in rats; associated steady-state AUC(0-24) values were 46,600 and 11,100 ng•h/mL in males and females, respectively. The reason for increased susceptibility of females for lethality on an exposure basis is unknown. Reversible bone marrow pancytopenia occurred in male and female rats at ?50 mg/kg (300 mg/m2) and ?100 mg/kg (600 mg/m2), respectively. Changes in males resulted in decreased peripheral blood cell counts. Testicular degeneration occurred in rats at ?50 mg/kg and was progressive in severity throughout the reversal period (1-month treatment-free observation period). Rales and tracheal mucosal atrophy occurred in male rats at ?50 mg/kg and in female rats at ?100 mg/kg. Target organ toxicity in rats generally occurred to a greater extent in males than in females at equivalent doses and may have been related to systemic drug exposures that were significantly higher in males (17,300 to 46,600 ng•h/mL) than females (2530 to 11,100 ng•h/mL). Dose- and time-dependent decreases in hematology parameter values occurred in dogs given PD-0332991 at ?0.6 mg/kg (12 mg/m2); associated steady-state AUC(0-24) values were ?548 ng•h/mL. Neutrophils and monocytes were most affected; the nadir occurred at the end of dosing, and all changes were reversible. Reversible bone marrow pancytopenia and lymphoid depletion occurred in dogs at ?0.6 mg/kg. Testicular degeneration occurred in dogs at ?0.6 mg/kg and was progressive in severity throughout the reversal period.
PD-0332991 was not mutagenic in bacteria or clastogenic in human lymphocytes in vitro with or without metabolic activation. PD-0332991 was positive for micronucleus formation in vitro, and in male rats following oral administration. Additional work demonstrated that PD-0332991 may have aneugenic potential.
There was no difference in overall plasma exposure of PD-0332991 when comparing the isethionate salt (previously used) or the free base (will be used moving forward) in the rat or the dog.
Based on results from in vitro testing in the 3T3 neutral red uptake assay, PD-0332991 is considered non-phototoxic.
7. SUMMARY OF DATA AND GUIDANCE FOR THE INVESTIGATOR
7.1. Mode of Action and Intended Indications
PD-0332991, an orally active pyridopyrimidine, is a potent and highly selective reversible inhibitor of cyclin-dependent kinase (CDK) 4 and CDK6. The compound prevents cellular DNA synthesis by prohibiting progression of the cell cycle from G1 into the S phase, as demonstrated in laboratory models and early clinical trials. PD-0332991 preclinical data indicate that it may be expected to have direct effect on growth arrest as well as potential secondary cytoreductive activity. Treatment of cultured tumor cells with PD-0332991 causes growth arrest that is accompanied by the inhibition of specific retinoblastoma (Rb) phosphorylation by CDK4 or CDK6 on residues serine -780 and -795 of Rb. Consequently, the phosphorylation status of these sites serves as specific biomarkers of CDK4/6 inhibition by PD-0332991.
Clinical data from a recently completed Phase 1 study in patients with mantle cell lymphoma also support proof of mechanism for PD-0332991 (A5481002 CSR). Using immunohistochemistry, significant reductions (?50%) were observed in the percent of phosphor-Rb positive cells in tumor biopsies at Cycle 1 Day 21 relative to baseline values in all 10 evaluable patients. Significant reductions were also observed in tumor cell proliferation and tumor metabolism, as analyzed by the repeated measures hierarchical model of percent change in positron emission tomography (PET) standard uptake value (SUV) max from baseline to Cycle 1 Day 21.
The only known natural substrate for CDK4/6 kinase activity is the Rb protein. Thus, tumors that do not express Rb are not expected to respond to PD-0332991. In most common tumors, however, other genetic and epigenetic changes lead to increases in CDK4/6 activity contributing to tumor cell growth. Certain tumors, including subtypes of non-Hodgkin's lymphoma (NHL), express high levels of Cyclin D1. Cyclin D1 complexes with and activates CDK4, facilitating movement through the cell cycle via Rb phosphorylation.
Tumors with high Cyclin D1 expression, therefore, present a unique opportunity for assessing PD-0332991 activity.
Indications currently under investigation in Phase 1/2 trials include advanced breast cancer (Study A5481003) and refractory multiple myeloma (Study A5481004).
7.1.1. Treatment of ER-Positive, HER2-Negative Advanced Breast Cancer (Study A5481003)
PD-0332991 was tested in vitro on molecularly characterized human breast cancer cell lines. Results from these experiments indicate that those cell lines more sensitive to PD-0332991 (maximum inhibitory concentration [IC]50 30 ?M) and by inducing CYP3A4 and CYP1A2 activity. In vitro, PD-0332991 demonstrated time-dependent inhibition of CYP3A in human liver microsomes. In vivo, inactivation of this enzyme by PD-0332991 may result in drug interactions with compounds that are predominantly metabolized by CYP3A. Therefore, caution must be exercised in patients receiving PD-0332991 in combination with drugs that are predominantly metabolized by CYP3A. In particular, co-administration of PD-0332991 with CYP3A4 substrates with narrow therapeutic indices (e.g., astemizole, terfenadine, cisapride, pimozide, quinidine, tacrolimus, cyclosporine, sirolimus, alfentanil and fentanyl, excluding transdermal patch) or ergot alkaloids (ergotamine, dihydroergotamine) must be avoided. In vitro evaluation of the potential for PD-0332991 to inhibit the drug efflux transporter P-glycoprotein (P-gp) was conducted in human multidrug-resistance (MDR1)-transfected Madin-Darby canine kidney (MDCK) cells. PD-0332991 (6.09 ?M/2730 ng/mL) inhibited the efflux of the P-gp substrates, digoxin (42%) and talinolol (71%).
These in vitro data suggest that PD-0332991 concentrations in the range of those observed at the recommended Phase 2 dose (RP2D) may inhibit P-gp activity. However, the clinical implications are unknown.
7.6. Fertility, Pregnancy, and Lactation
Fertility and teratology studies with PD-0332991 have not been conducted. Therefore, safety for pregnant women of childbearing capacity and for the fetus cannot be implied from the existing data. If the drug is used during pregnancy or if the patient becomes pregnant while receiving this drug, the patient should be apprised of the potential hazard to the fetus.
Women of childbearing potential must have a negative pregnancy test prior to treatment with PD-0332991. Female patients must be surgically sterile or postmenopausal, or they must agree to use effective contraceptive during the on-treatment period of the study and for at least 90 days after completion of treatment. Male patients must be surgically sterile or must agree to use effective contraception during the on-treatment period of the study and for at least 90 days after completion of treatment. The decision of effective contraception will be based on the judgment of the principal investigator or a designated associate.
PD-0332991 caused testicular degeneration in nonclinical studies. Testicular degeneration was not reversed after cessation of treatment and increased in severity in both rat and dog. Testicular toxicity is a well recognized sequelae to cytotoxic anticancer therapy. Testicular degeneration produced by PD-0332991 is consistent with CDK inhibition and alterations in cell cycle kinetics. Patients should consider sperm preservation prior to beginning therapy with PD-0332991.
7.7. Effects on Ability to Drive and Use Machines
No studies on the effects on the ability to drive or operate machinery have been performed. PD-0332991 has been reported to cause dizziness, visual impairment, and hypotension in clinical studies.
7.8. Undesirable Effects
The list of the adverse drug reactions (ADRs) or undesirable effects for PD-0332991 contributes to the information necessary for investigators to assess the safety profile of PD-0332991. A treatment-emergent adverse event (TEAE) is included as an ADR if, after careful examination by sponsor it is determined to be reasonably associated with PD-0332991. The threshold for inclusion of these TEAEs depends in part on the seriousness and frequency of the ADR, the quality of the information for identified reports, the accumulated body of information regarding the event, and the strength of the evidence. In the situation where strong suspicion of a causal relationship cannot be determined, the sponsor may wait for additional clinical information from its clinical studies before the ADR is included in this section. Adverse drug reactions indentified on the basis of safety information from subjects who received PD-0332991 in studies conducted by the sponsor are listed below.
Overall, the safety profile for PD-0332991 as a single agent and in combination with other anticancer therapies is characterized by manageable and reversible neutropenia, thrombocytopenia, anemia, fatigue, constipation, nausea, and diarrhea. Table 7-1 presents the adverse drug reactions (ADRs) for PD-0332991, the drug-related events that are considered to be part of the product profile.
7.9. Overdose
Three (3) patients (2 patients in Study A5481003 and 1 patient in Study A5481004) have reported a PD-0332991 overdose (defined as increase in daily dose) as of the 01 September 2012 cutoff.
One patient from A5481003 received 200 mg/day for 13 days instead of 125 mg. This patient reported nausea, vomiting, and dizziness, but the events resolved and no clinically significant findings were reported. The other patient received 200 mg/day for 21 days instead of 125 mg. This patient reported no relevant AEs and no SAEs during this period. Both cases were due to drug dispensing errors that occurred at the sites. The patient from A5481004 received 150 mg/day for 12 days instead of 75 mg. The patient reported no relevant AEs and no SAEs during this period. This case was also due to drug dispensing error that occurred at the site. Overall, none of the 3 patients demonstrated any clinical or laboratory complications attributed to the overdose.
No specific antidotes exist for the treatment of patients with PD-0332991 overdose. Since renal excretion of PD-0332991 is minimal, the benefit of hemodialysis in the treatment of a PD-0332991 overdose is probably negligible. The treatment of overdose of PD-0332991 should consist of general supportive measures. If indicated, elimination of unabsorbed drug should be achieved by emesis or gastric lavage.
7.10. Drug Abuse and Dependence
There are no data available on either drug abuse or dependence.
7.11. Evolving Safety Information
7.11.1. Clinical Safety
7.11.1.1. Patient Exposure
As of the cutoff date of 01 September 2012, data were available for 74 patients in Study A5481001, 17 patients in Study A5481002, 95 patients in Study A5481003, and 51 patients in Study A5481004.
7.11.1.2. Phase 1 Experience as a Single Agent in Adult Patients with Advanced Cancers (Studies A5481001 and A5481002)
Since studies A5481001 and A5481002 both tested PD-0332991 as a single agent in advanced cancers, the relevant safety data have been combined. The most frequently reported TEAEs (?20% of patients) of any grade, regardless of causality were fatigue, neutropenia, nausea, diarrhea and anemia, constipation, decreased appetite, and vomiting and thrombocytopenia.
The most frequently reported TEAEs (?20% of patients) of any grade that were considered to be related to the study treatment were neutropenia, fatigue, diarrhea and anemia, and nausea.
The most frequently reported Grade 4 TEAEs were neutropenia and thrombocytopenia. Grade 4 leukopenia, anemia, pulmonary embolism, increased blood uric acid, decreased hemoglobin level and hyperglycemia were also observed. Grade 5 events regardless of causality were reported for 8 patients and included disease progression (5 patients), cardiac arrest (2 patients) and failure to thrive (1 patient). No Grade 5 event was considered to be related to PD-0332991.
7.11.1.3. Phase 1/2 Experience with PD-0332991 in Combination with Letrozole in Women with Breast Cancer: Study A5481003
The most frequently reported TEAEs (?20% of patients) of any grade, regardless of causality were neutropenia, leucopenia, fatigue, nausea, anemia, diarrhea, arthralgia and hot flush. All reports of neutropenia and leukopenia were considered to be related to the study treatment.
The most frequently reported Grade 4 TEAEs were neutropenia, fatigue and pulmonary embolism. As of the 01 September 2012 cutoff, one Grade 5 event of unrelated disease progression was reported. The most frequently reported TEAEs (>20% of patients) of any grade, considered to be treatment related were neutropenia, leukopenia, anemia, and fatigue.
No significant changes in blood pressure, pulse rate and body weight have been observed as of June 2012. No patients experienced QTcF intervals >500 msec as of June 2012.
7.11.1.4. Phase 1/2 Experience with PD-0332991 in Combination with Bortezomib and Dexamethasone in Patients with Multiple Myeloma: Study A5481004
The most frequently reported TEAEs (?20% of patients) of any grade, regardless of causality were thrombocytopenia, fatigue, neutropenia, anemia, nausea, back pain, constipation, diarrhea, peripheral neuropathy, cough, pyrexia, peripheral edema, dyspnea and increased blood creatinine. Febrile neutropenia was reported for 4 patients (7.8%). The most frequently reported TEAEs (>20% of patients) of any grade, considered to be treatment related were thrombocytopenia, neutropenia, anemia, fatigue, nausea, diarrhea, and peripheral neuropathy.
Grade 4 febrile neutropenia was reported for 1 patient and was considered to be related to the study treatment. Grade 5 events were reported for 2 patients; both events (disease progression and sepsis) were considered to be not related to study treatment.
No patient experienced a QTcF interval >500 msec on study as of June 2012.
7.11.1.5. Serious Adverse Events
As of the 01 September 2012 cutoff, 84 cases in 64 patients corresponding to 134 events have been recorded. The only events occurring ?3 times are febrile neutropenia, abdominal pain, constipation, diarrhea, nausea, vomiting, bronchopneumonia and pneumonia, back pain, and pulmonary embolism. The majority of these events were considered unrelated to PD-0332991. The only events occurring >2 times and considered related to PD-0332991 were febrile neutropenia (3) and pneumonia (3). These events are described in more detail in Section 6.2.3.
7.11.1.6. Specific Adverse Events of Note
7.11.1.6.1. Effects on Bone Marrow
Based on the nonclinical studies performed to date, bone marrow is the main target organ for toxicity. Consistent with nonclinical observations, patients enrolled to the clinical studies of PD-0332991 have experienced treatment-related changes in their blood counts such as leukopenia, neutropenia, thrombocytopenia, and anemia. In general, the myelosuppression was reversible, not cumulative, and easily managed.
During clinical studies with PD-0332991, patients’ complete blood counts should be closely monitored. In case of clinically significant toxicities, treatment with PD-0332991 should be interrupted and the dose of PD-0332991 modified as defined in the protocol. Myelosuppression and associated events should be managed based on the judgment of the investigator and according to local clinical practices. Erythropoietin may be used for the supportive treatment of anemia. Granulocyte-colony stimulating factors should not be used prophylactically but they may be used in treatment-emergent neutropenia as indicated by the current American Society of Clinical Oncology (ASCO) guidelines.
7.11.2. Effects on the Cardiovascular System
Data from in vitro and in vivo nonclinical studies indicated that PD-0332991 has the potential to delay cardiac repolarization as measured by prolongation of the QT interval on the electrocardiogram (ECG).
No significant changes in blood pressure, pulse rate and body weight have been observed in the 2 completed Phase 1 clinical studies in advanced cancers (A5481001 and A5481002; see Section 6.2.2.1). In Study A5481001, an analysis of the corrected QT interval using Fridericia’s formula (QTcF) and corrected QT interval using Bazett’s formula (QTcB) data from 73 patients in Study A5481001 demonstrated that QTcF was the more appropriate correction method based on plots of the QTc versus RR interval. O PD-0332991 inibe a fosforilação catalizada por CDK4 purificada da proteína de retinoblastoma com uma IC50 inferior a 20 nm, bem como o crescimento tumoral de vários tipos de tumores de xenoenxerto humano (SF-295, MDA-MB-435, Colo-205 e outros), desenvolvido em camundongos SCID. Os valores de Cmax e AUC(0-24) correspondentes associados com essas doses foram de ?511 ng/ml e ?8800 ng•h/ml, respectivamente. O PD-0332991 inibiu a corrente hERG em células HEK293 e aumentou a duração do potencial de ação a 90% de repolarização em fibras de Purkinje, consistente com a prolongação do QT observada in vivo.
Ocorreram efeitos pulmonares significativos e transitórios em cães que receberam um única dose IV de PD-0332991 de 5 mg/kg (100 mg/m2). As concentrações plasmáticas máximas associadas do fármaco observadas foram ?2040 ng/ml. Estes efeitos foram consistentes com a depressão respiratória mediada centralmente. Não foram notadas alterações da função pulmonar em cães com concentrações plasmáticas máximas observadas ?414 ng/ml com 1 mg/kg (20 mg/m2).
Em estudos fundamentais de toxicologia de dose repetida, o PD-0332991 foi letal em ratos; os valores associados de AUC(0-24) em estado estável foram de 46.600 e 11.100 ng•h/ml, respectivamente em machos e fêmeas. A razão do aumento de susceptibilidade das fêmeas a letalidade com base na exposição é desconhecida. Pancitopenia reversível da medula óssea ocorreu em ratos machos e fêmeas a ?50 mg/kg (300 mg/m2) e ?100 mg/kg (600 mg/m2), respectivamente. As alterações nos machos resultaram na diminuição das contagens de células sanguíneas periféricas. Ocorreu degeneração testicular em ratos a ?50 mg/kg e foi progressiva em severidade durante todo o período de reversão (período de observação de 1 mês sem tratamento). Ralas e atrofia da mucosa traqueal ocorreram em ratos macho a ?50 mg/kg e em ratos fêmea a ?100 mg/kg. Nos ratos a toxicidade do órgão alvo geralmente ocorreu em maior medida nos machos do que nas fêmeas em doses equivalentes e pode estar relacionada a exposições sistêmicas ao fármaco que foram significativamente mais elevadas nos machos (17.300 a 46.600 ng•h/ml) do que nas fêmeas (2530 a 11.100 ng•h/ml). Diminuições dos valores dos parâmetros hematológicos dependentes da dose e do tempo ocorreram em cães que receberam PD-0332991 a ?0,6 mg/kg (12 mg/m2); os valores associados da AUC(0-24) em estado estável foram ?548 ng•h/ml. Neutrófilos e monócitos foram os mais afetados; o nadir ocorreu no final da administração e todas as alterações foram reversíveis. Ocorreu pancitopenia reversível da medula óssea e depleção linfóide em cães a ?0,6 mg/kg. Ocorreu degeneração testicular em cães a ?0,6 mg/kg e foi progressiva em severidade durante todo o período de reversão.
O PD-0332991 não foi mutagênico em bactérias nem clastogênico em linfócitos humanos in vitro com ou sem ativação metabólica. O PD-0332991 foi positivo para formação de micronúcleo in vitro e em ratos macho após administração oral. Trabalhos adicionais demonstraram que o PD-0332991 pode ter potencial aneugênico.
Não foi observada diferença da exposição plasmática geral do PD-0332991 quando comparado com sal isetionato (usado anteriormente) ou com a base livre (será usada no futuro) no rato ou no cão.
Com base nos resultados dos testes in vitro do ensaio de absorção do vermelho neutro 3T3, o PD-0332991 é considerado não fototóxico.
7. RESUMO DOS DADOS E ORIENTAÇÃO PARA O INVESTIGADOR
7.1. Modo de ação e indicações previstas
O PD-0332991, uma piridopirimidina ativa oralmente, é um inibidor reversível poderoso e altamente seletivo da cinase dependente de ciclina (CDK) 4 e CDK6. O composto previne a síntese do DNA celular impedindo a progressão do ciclo celular da fase G1 para a fase S, como demonstrado em modelos laboratoriais e ensaios clínicos anteriores. Os dados pré-clínicos do PD-0332991 indicam poder se esperar ter efeito direto sobre a paragem do crescimento bem como atividade citoredutora secundária potencial. O tratamento de células tumorais de cultura com PD-0332991 causa paragem do crescimento acompanhada pela inibição da fosforilação de retinoblastoma (Rb) específico por CDK4 ou CDK6 em resíduos serina-780 e -795 do Rb. Conseqüentemente, o estado de fosforilação desses locais serve como biomarcador específico da inibição de CDK4/6 pelo PD-0332991.
Os dados clínicos de um estudo de Fase 1 concluído recentemente em pacientes com linfoma de células de manto também suportam a prova do mecanismo do PD-0332991 (A5481002 CSR). Usando imunohistoquímica, foram observadas reduções significativas (?50%) na percentagem de células fósforo Rb-positivas em biopsias tumorais no Ciclo 1 Dia 21 em comparação com os valores de linha basal em todos os 10 pacientes avaliáveis. Também foram observadas reduções significativas na proliferação de células tumorais e no metabolismo tumoral, tal como analisados pelo modelo hierárquico de medições repetidas da alteração percentual desde a linha basal na tomografia por emissão de positrões (PET) valor padrão de absorção (SUV) máx até ao Ciclo 1 Dia 21.

Os tumores com elevada expressão de ciclina D1, por conseguinte, apresentam uma oportunidade única para avaliar a atividade do PD-0332991.
As indicações atualmente em investigação em ensaios de Fase 1/2 incluem câncer mamário avançado (Estudo A5481003) e mieloma múltiplo refractário (Estudo A5481004).
7.1.1. Tratamento de câncer mamário avançado ER-positivo, HER2-negativo (Estudo A5481003)
O PD-0332991 foi testado in vitro em linhagens celulares de câncer mamário humano molecularmente caracterizadas. Os resultados destes experimentos indicam que as linhagens celulares mais sensíveis ao PD-0332991 (concentração inibitória máxima [IC]50 30 ?M) e pela indução da atividade de CYP3A4 e CYP1A2. In vitro, o PD-0332991 demonstrou inibição dependente do tempo de CYP3A em microssomos hepáticos humanos. In vivo, a inativação desta enzima pelo PD-0332991 pode resultar em interações medicamentosas com compostos que são predominantemente metabolizados pela CYP3A. Por conseguinte, é necessária cautela em pacientes que recebem PD-0332991 em combinação com fármacos que sejam predominantemente metabolizados pela CYP3A. Particularmente, a co-administração de PD-0332991 com substratos de CYP3A4 com índices terapêuticos estreitos (por exemplo astemizol, terfenadina, cisapride, pimozide, quinidina, tacrolimo, ciclosporina, sirolimo, alfentanil e fentanil, exceto penso transdérmico) ou alcalóides da cravagem (ergotamina, diidroergotamina) deve ser evitada. A avaliação in vitro do potencial do PD-0332991 inibir o transportador de efluxo do fármaco P-glicoproteína (P-gp) foi conduzida em células humanas multi-fármaco resistentes (MDR1) transfectadas com rim canino Madin-Darby (MDCK). O PD-0332991 (6,09 ?m/2730 ng/ml) inibiu o efluxo dos substratos P-gp digoxina (42%) e talinolol (71%).
Estes dados in vitro sugerem que as concentrações de PD-0332991 nos limites observados na dose recomendada de Fase 2 (RP2D) pode inibir a atividade de P-gp. No entanto as implicações clínicas são desconhecidas.
7.6. Fertilidade, gravidez e aleitamento
Não foram realizados estudos de fertilidade e teratologia com o PD-0332991. Por conseguinte, não é possível deduzir a segurança de mulheres grávidas ou em idade fértil e do feto a partir dos dados existentes. Se o fármaco for usado durante a gravidez ou se a paciente ficar grávida enquanto estiver recebendo esse fármaco, a paciente deve ser informada do risco potencial para o feto.
As mulheres em idade fértil devem ter um teste negativo de gravidez antes do tratamento com PD-0332991. As pacientes do sexo feminino têm de ser cirurgicamente esterilizadas ou pós-menopáusicas, ou têm de concordar usar contraceptivos eficazes durante o período do tratamento do estudo e por pelo menos 90 dias após a conclusão do tratamento. Os pacientes do sexo masculino têm de ser cirurgicamente esterilizados ou têm de concordar usar contraceptivos eficazes durante o período do tratamento do estudo e por pelo menos 90 dias após a conclusão do tratamento. O PD-0332991 causou degeneração testicular em estudos não clínicos. A degeneração testicular não reverteu após a cessação do tratamento e aumentou em severidade no rato e no cão. A toxicidade testicular é uma seqüela bem conhecida da terapia anti-câncer citotóxica. A degeneração testicular produzida pelo PD-0332991 é consistente com a inibição de Cdk e com as alterações cinéticas do ciclo celular. Os pacientes devem considerar a preservação de esperma antes de iniciarem a terapia com PD-0332991.
7.7. Efeitos sobre a capacidade de dirigir e utilizar máquinas
Não foram realizados estudos sobre os efeitos sobre a capacidade de dirigir ou utilizar maquinário. Foi reportado o PD-0332991 causar tontura, visão comprometida e hipotensão em estudos clínicos.
7.8. Efeitos indesejáveis
A lista de reações adversas ao fármaco (ADRs) ou de efeitos indesejáveis do PD-0332991 contribui para a informação necessária para os investigadores avaliarem o perfil de segurança do PD-0332991. Uma reação adversa decorrente do tratamento (TEAE) é incluída como ADR se, após exame cuidadoso pelo patrocinador, for determinada como estando razoavelmente associada ao PD-0332991. O limiar para inclusão dessas TEAEs depende em parte da gravidade e freqüência da ADR, da qualidade da informação para relatórios identificados, do corpo de informação acumulada referente à reação e da força da evidência. No caso de não ser possível determinar uma forte suspeita de relação causal, o patrocinador pode aguardar informação clínica adicional dos estudos clínicos antes da ADR ser incluída nesta seção. As reações adversas ao fármaco identificadas com base na informação de segurança de voluntários que receberam PD-0332991 em estudos conduzidos pelo patrocinador são enumeradas abaixo.
No geral o perfil de segurança do PD-0332991 como agente único e em combinação com outras terapias anti-câncer é caracterizado por neutropenia tratável e reversível, trombocitopenia, anemia, fadiga, obstipação, náusea e diarréia. A Tabela 7-1 apresenta as reações adversas ao fármaco (ADRs) para o PD-0332991, as reações relacionadas ao fármaco consideradas como parte do perfil do medicamento.
7.9. Sobredosagem
Três (3) pacientes (2 pacientes no Estudo A5481003 e 1 paciente no Estudo A5481004) reportaram sobredosagem de PD-0332991 (definida como aumento da dose diária) na data imite de 1 de setembro de 2012.
Um paciente do A5481003 recebeu 200 mg/dia por 13 dias em vez de 125 mg. Esse paciente reportou náusea, vômitos e tontura, mas as reações resolveram e não foram reportadas conclusões clinicamente significativas. O outro paciente recebeu 200 mg/dia por 21 dias em vez de 125 mg. esse paciente não reportou nenhuma AE relevante nem SAEs durante esse período. Ambos os casos se deveram a erros de distribuição que ocorreram nos centros. O paciente do A5481004 recebeu 150 mg/dia por 12 dias em vez de 75 mg. O paciente não reportou AEs relevantes nem SAEs durante esse período. Esse caso também se deveu a erro de distribuição que ocorreu no centro. No geral nenhum dos 3 pacientes demonstrou qualquer complicação clínica ou laboratorial atribuídas a sobredosagem.
Não existem antídotos específicos para tratamento de pacientes com sobredosagem por PD-0332991. Dado que a excreção renal do PD-0332991 é minima, o benefício da hemodiálise no tratamento da sobredosagem por PD-0332991 é provavelmente insignificante. O tratamento da sobredosagem por PD-0332991 deve consistir de medidas gerais de suporte. Caso indicado, a eliminação do fármaco não absorvido deve ser obtida por emese ou lavagem gástrica.
7.10. Abuso e dependência do fármaco

7.11. Informação de segurança em evolução
7.11.1. Segurança clínica
7.11.1.1. Exposição do paciente
A partir da data limite de 1 de setembro de 2012, estão disponíveis dados de 74 pacientes no Estudo A5481001, 17 pacientes no Estudo A5481002, 95 pacientes no Estudo A5481003 e 51 pacientes no Estudo A5481004.
7.11.1.2. Experiência de Fase 1 como agente único em pacientes adultos com cânceres avançados (Estudos A5481001 e A5481002)
Dado que os estudos A5481001 e A5481002 testaram o PD-0332991 como agente único em cânceres avançados, os dados de segurança relevantes foram combinados. As TEAEs mais frequentemente reportadas (?20% dos pacientes) de qualquer grau e independentemente da causalidade foram fadiga, neutropenia, náusea, diarréia e anemia, obstipação, diminuição do apetite e vômitos e trombocitopenia.
As TEAEs mais frequentemente reportadas (?20% dos pacientes) de qualquer grau que foram consideradas relacionadas ao tratamento do estudo foram neutropenia, fadiga, diarréia, anemia e náusea.
As TEAEs de Grau 4 mais frequentemente reportadas foram neutropenia e trombocitopenia. Também foram observadas leucopenia de Grau 4, anemia, embolia pulmonar, aumento do ácido úrico sanguíneo, diminuição do nível de hemoglobina e hiperglicemia. Foram reportada reações de Grau 5 independentemente da causalidade em 8 pacientes e incluíram progressão da doença (5 pacientes), paragem cardíaca (2 pacientes) e deficiência de crescimento (1 paciente). Nenhuma reação de Grau 5 foi considerada relacionada ao PD-0332991.
7.11.1.3. Experiência de Fase1/2 com PD-0332991 em combinação com letrozol em mulheres com câncer mamário: Estudo A5481003
As TEAEs mais frequentemente reportadas (?20% dos pacientes) de qualquer grau e independentemente da causalidade foram neutropenia, leucopenia, fadiga, náusea, anemia, diarréia, artralgia e afrontamento. Todos os reportes de neutropenia e leucopenia foram considerados relacionados ao tratamento do estudo.
As TEAEs de Grau 4 mais frequentemente reportadas foram neutropenia, fadiga e embolia pulmonar. Na data limite de 1 de setembro de 2012, foi reportada uma reação de Grau 5 de progressão da doença não relacionada. As TEAEs mais frequentemente reportadas (?20% dos pacientes) de qualquer grau que foram consideradas relacionadas ao tratamento foram neutropenia, leucopenia, anemia e fadiga.
Não foram observadas alterações significativas da pressão arterial, freqüência de pulso e peso corporal até junho de 2012. Nenhum paciente experimentou intervalos QTcF >500 mseg até junho de 2012.
7.11.1.4. Experiência de Fase 1/2 com PD-0332991 em combinação com bortezomibe e dexametasona em pacientes com mieloma múltiplo: Estudo A5481004
As TEAEs mais frequentemente reportadas (?20% dos pacientes) de qualquer grau e independentemente da causalidade foram trombocitopenia, fadiga, neutropenia, anemia, náusea, dor nas costas, obstipação, diarréia, neuropatia periférica, tosse, pirexia, edema periférico, dispnéia e aumento da creatinina sanguínea. Neutropenia febril foi reportada em 4 pacientes (7,8%). As TEAEs mais frequentemente reportadas (>20% dos pacientes) de qualquer grau consideradas relacionadas ao tratamento foram trombocitopenia, neutropenia, anemia, fadiga, náusea, diarréia e neuropatia periférica.
Foi reportada neutropenia febril de Grau 4 em 1 paciente e foi considerada relacionada ao tratamento do estudo. Foram reportadas reações de Grau 5 em 2 pacientes; ambas as reações (progressão da doença e sepse) foram consideradas não relacionadas ao tratamento do estudo.
Nenhum paciente experimentou um intervalo QTcF >500 mseg no estudo até junho de 2012.
7.11.1.5. Reações adversas graves
Na data limite de 1 de setembro de 2012, foram registrados 84 casos em 64 pacientes correspondendo a 134 reações. As únicas reações ocorrendo ?3 foram neutropenia febril, dor abdominal, obstipação, diarréia, náusea, vômitos, broncopneumonia e pneumonia, dor nas costas e embolia pulmonar. A maioria dessas reações foram considerada não relacionadas ao PD-0332991. As únicas reações ocorrendo >2 e considerada relacionadas ao PD-0332991 foram neutropenia febril (3) e pneumonia (3). Essas reações são descritas com mais detalhe na Seção 6.2.3.
7.11.1.6. Reações adversas específicas de notar
7.11.1.6.1. Efeitos sobre a medula óssea
Com base nos estudos não clínicos realizados até à data, a medula óssea é o principal órgão alvo para toxicidade. Consistentemente com as observações não clínicas, os pacientes incluídos nos estudos clínicos do PD-0332991 experimentaram alterações relacionadas ao tratamento de seus hemogramas, como leucopenia, neutropenia, trombocitopenia e anemia. No geral a mielosupressão foi reversível, não cumulativa e facilmente tratada.
Durante os estudos clínicos com PD-0332991, os hemogramas completos dos pacientes devem ser monitorados de perto. No caso de toxicidades clinicamente significativas, o tratamento com PD-0332991 deve ser interrompido e a dose de PD-0332991 modificada tal como definido no protocolo. A mielosupressão e reações associada devem ser tratadas com base na opinião do investigador em conformidade com as práticas clínicas locais. Pode ser usada eritropoietina para tratamento de suporte da anemia. Fatores estimulantes de colônia de granulócitos não devem ser usados como profilácticos, mas podem ser usados na neutropenia decorrente do tratamento, tal como indicado pelas atuais diretrizes da Sociedade Americana de Oncologia Clínica (ASCO).
7.11.2. Efeitos sobre o sistema cardiovascular

Princípios do teste de lípidos
Após a aplicação da amostra de sangue no disco e o encerramento da tampa da dobradiça, é perfurado um recipiente de diluição existente no instrumento, disseminando soro tampão fosfato numa câmara de mistura.
Os eritrócitos das amostras de sangue capilar ou venoso são separados do plasma por centrifugação.
No passo seguinte o tampão é misturado com a amostra de plasma e transferido através de canais fluídicos para as câmaras de reação.
O sistema determina o colesterol total e o colesterol HDL por um método enzimático.
O teste de triglicéridos consiste de um método enzimático de ponto terminal que utiliza diferentes enzimas.
Quando a concentração de triglicéridos for ≤ 400 mg/dL, as lipoproteínas de baixa densidade (LDL) são calculadas usando a fórmula de Friedewald: (medidas em mg/dL).
Quando a concentração de triglicéridos for 400 mg/dL, o colesterol LDL não é calculado.
Classificação de segurança

Existem várias superfícies ópticas no disco. Por conseguinte, sempre que manipular um disco certifique-se de que o segura apenas pela tampa, não toque nas superfícies transparentes.
Quando escrever no disco, por exemplo o nome do doente, certifique-se de apenas usar a área reservada.
Não cole rótulos no disco.
O lado impresso tem de estar virado para cima quando introduzir o disco.

SOAKING CAP OE-C9
This cap must be securely attached before immersion. Align the black arrow on the soaking cap with the green dot at the base of the silver collar surrounding the electrical contacts on the PVE connector. Press the cap down onto the metal collar and turn clockwise to secure.
Prior to immersion this cap must be attached to the scanning unit connector.
The scanning unit connector for this Ultrasound Bronchoscope is NOT immersible without its soaking “cap” attached. Prior to immersion, the two-component soaking cap/casing must be securely attached to the connector to maintain the instrument’s watertight integrity. Failure to properly secure the soaking cap can result in fluid invasion and instrument damage.
SCANNING UNIT CONNECTOR LOCK
Securely locks the scanning connector of the endoscope into the scanning unit to ensure proper contact with and functioning of the electronic circuitry.

TAMPA DE IMERSÃO OE-C9
Esta tampa deve estar firmemente ligada antes da imersão. Alinhe a flecha preta da tampa de imersão com a pinta verde da base do colar prateado que envolve os contactos elétricos do conector PVE. Prima a tampa sobre o colar metálico e rode no sentido horário para fixar.
Antes da imersão, esta tampa tem de estar ligada ao conector da unidade de imageologia.
O conector da unidade de imageologia deste Broncoscópio Ultrassónico NÃO é submergível sem a “tampa” de imersão ligada. Antes da imersão, os dois componentes da tampa/invólucro de imersão têm de estar firmemente ligados ao conector para manter a integridade da impermeabilidade do instrumento. Se a tampa de imersão não estiver bem fixa, pode resultar em inundação por fluidos e danos no instrumento.
BLOQUEIO DO CONECTOR DA UNIDADE DE IMAGEOLOGIA
Bloqueia firmemente o conector de imageologia do endoscópio à unidade de imageologia para garantir o bom contacto e funcionamento dos circuitos eletrónicos.

One CO2 endoscopic insufflator tubing set with 183 cm (6 ft) of vinyl tubing. The system includes a Luer connector, 0.2 (max) ?m hydrophobic filter, and connector to CO2 endoscopic insufflator. This product is latex-free and supplied non-sterile.
INDICATIONS:
For CO2 insufflation of the gastrointestinal tract with the Bracco CO2 endoscopic insufflators. Please refer to the CO2 endoscopic insufflator Operator's Manual for additional instructions for use.
CONTRAINDICATIONS:
• Do not use the CO2 endoscopic insufflator or the CO2 endoscopic insufflator tubing set for hystero-scopic insufflation, i.e., it must not be used for intrauterine distention.
• Do not use the CO2 endoscopic insufflator or the CO2 endoscopic insufflator tubing set for CT Colonography (Virtual Colonoscopy).
WARNINGS:
• Tubing set is to be changed following standards and guidelines set forth by SGNA and APIC for reprocessing of water bottles used during Gastrointestinal Endoscopy.
• This product contains phthalates which have been perceived as having possible carcinogenic, mutagenic and reproductive risks. However, based on all existing scientific data, the long history of safe use of medical device products containing phthalates, as well as the short duration of contact with this device, there are no known cancer or reproductive risks to humans. Physician discretion is required to ensure that benefits outweigh risks when this device is used in children, elderly and pregnant women.
• Tubing set is to be changed daily.
Instructions For Use:
• Please refer to CO2 endoscopic insufflator Operator's Manual for additional Instructions for Use.
• Make sure the CO2 endoscopic insufflator is powered up with no tubing set in place.
• Make sure the CO2 endoscopic insufflator is in STOP mode.
• Set the desired operational modes for the system.
• Remove the tubing set from its packaging.
• Connect tubing set fitting to the endoscopic system.
• Connect the tubing set fitting to CO2 endoscopic insufflator.
NOTE: Position the CO2 endoscopic insufflator tubing set so that the tubing is unobstructed.
• Start the endoscopic system by turning on the necessary light and video equipment.
• Verify that the air supply from the endoscopic system is turned off.
• Start CO2 endoscopic insufflator by pressing the STOP/RUN button.
• Monitor the Patient during procedure.
• Stop the unit at conclusion of procedure.
• Discard the entire tubing set daily in accordance with standards and guidelines set forth by SGNA and APIC for reprocessing of water bottles used during Gastrointestinal Endoscopy.

INDICAÇÕES:
Para insuflação do tracto gastrintestinal com CO2 com os insufladores endoscópicos Bracco de CO2. Para instruções de utilização adicionais, queira consultar o Manual do Operador do insuflador endoscópico de CO2.
CONTRA-INDICAÇÕES:
• Não utilize o insuflador endoscópico de CO2 nem o conjunto de tubagens do insuflador endoscópico de CO2 para insuflação histeroscópica, ou seja, não deve ser utilizado para distensão intra-uterina.
• Não utilize o insuflador endoscópico de CO2 nem o conjunto de tubagens do insuflador endoscópico de CO2 para colonografia CT (colonoscopia virtual).
ADVERTÊNCIAS:
• O conjunto de tubagens deve ser trocado seguindo as normas e directrizes estipuladas pela SGNA e pela APIC para reprocessamento dos frascos de água utilizados durante endoscopia gastrintestinal.
• Este produto contém ftalatos, considerados como tendo possíveis riscos carcinogénicos, mutagénicos e reprodutivos. No entanto, e com base em todos os dados científicos existentes, no longo historial de utilização segura de produtos de dispositivos médicos contendo ftalatos, bem como na curta duração de contacto com este dispositivo, não existem riscos carcinogénicos nem reprodutivos conhecidos para o ser humano. É necessária prudência por parte do médico para garantir que os benefícios se sobrepõem aos riscos quando este dispositivo for utilizado em crianças, idosos e mulheres grávidas.
• O conjunto de tubagens deve ser trocado diariamente.
Instruções de utilização:
• Para instruções de utilização adicionais, queira consultar o Manual do Operador do insuflador endoscópico de CO2.
• Certifique-se de que o insuflador endoscópico de CO2 é ligado à corrente sem o conjunto de tubagens colocado.
• Certifique-se de que o insuflador endoscópico de CO2 se encontra em modo STOP.
• Configure os modos operacionais pretendidos para o sistema.
• Remova o conjunto de tubagens da embalagem.
• Ligue o adaptador do conjunto de tubagens ao sistema endoscópico.
• Ligue o adaptador do conjunto de tubagens ao insuflador endoscópico de CO2.
NOTA: Posicione o conjunto de tubagens do insuflador endoscópico de CO2 de modo que a tubagem não fique obstruída.
• Inicie o sistema endoscópico ligando o equipamento de luz e vídeo necessário.
• Verifique se o fornecimento de ar do sistema endoscópico se encontra desligado.
• Inicie o insuflador endoscópico de CO2 premindo o botão STOP/RUN.
• Monitorize o doente durante a intervenção.
• Desligue a unidade após a conclusão da intervenção.
• Descarte todo o conjunto de tubagens diariamente em conformidade com as normas e directrizes estipuladas pela SGNA e pela APIC para reprocessamento dos frascos de água utilizados durante endoscopia gastrintestinal.
