XXX’s P-CRM platform will extend to XXX SIM cards to create secure, privacy protected, real time personalization for mobile devices
Cannes, Feb. 21, 2001 - XXX, the world’s leading provider of smart card-based solutions, security and wireless applications, and XXX, a global leader in predictive customer relationship management (P-CRM) solutions, announced today at the 3GSM World Congress their joint development of SIM card-based CRM applications, marking a milestone in XXX’ strategy to leverage the inherent capabilities of the SIM card as the computing engine within the mobile device.
“The inherently secure and pervasive SIM card with its standard communication infrastructure is the ideal host for customer centric applications for mobile telephony and ultimately mobile computing,” said XXXX, vice president and general manager for XXX’ Next Generation Networks Division. “These applications will deliver fundamental business value for service providers, while offering utility and privacy for end users.”
The technology extends XXX’s P-CRM platform to the SIM card whereby subscriber profiles are captured in real time on the mobile device. As thresholds are reached, the P-CRM application sends alerts over the air to the operator, triggering the selection of an optimized offer to be sent back to the SIM card and displayed on the mobile device. The SIM card ensures security and maintains privacy for the subscriber. Inherent within this framework is the ability for the user to deactivate or “opt out” of this service. This technology is already in pilot at a tier one GSM operator.
XXX’s P-CRM solution uses real-time subscriber data to proactively identify end-users likely to churn, or opportunities to cross-sell and up-sell, then promotes targeted offers that have a high propensity to be accepted. With its Version 4.0 release, XXX has added Upsell/CPSTM and Cross-Sell/CPSTM capabilities that complement its Churn/CPSTM solution. “The enhanced platform addresses both cost mitigation and revenue generation, maximizing the value of each subscriber,” explained xxXX, CEO of XXX.

The Right to Voice a Complaint
Patients are encouraged to communicate their concerns or questions to any member of the staff, who will assist personally or refer the issue to someone who can. Patients may register a formal complaint verbally or in writing. All complaints are responded to in writing by the social worker within 14 days .
The Right to Access the Bio-Ethics Committee
Conflicts occasionally arise regarding medical or nursing care among patient, family, and caregivers. If an issue cannot be readily resolved a Bio-Ethics Committee consultation can be scheduled by contacting the nursing unit manager.
The Right to Access Protective Services
Each patient has the right to access protective services, e.g., child or adult services. In addition, a patient who is concerned about abuse, neglect or misappropriation of property has the right to file a complaint. The Social Services Department assists patients and their families with this process.
The Right to Spiritual Care
Pastoral care and spiritual support is encouraged and available. The Social Services Department will help to contact a member of the clergy on the behalf of a patient. A Chapel/Quiet Room is located adjacent to the Main Lobby.
Guide to Hospital Services
Waiting Areas & Public Restrooms
Several lobbies are available, as is the Main Dining Room. Public restrooms are located near the front and Emergency entrances. Employees can provide directions to a wheelchair accessible restroom. Please help to enforce fire and safety codes, which require hallways to be kept clear at all times. Please do not leave children unattended in public areas for their own safety and security.
Personal Possessions (ext. 8180)
Patients should only bring items that are essential to their comfort, such as personal toiletries. All valuables must be sent home or kept in the locked safe located in the Admitting Department. _________ is not responsible for lost personal articles, including dentures, eye glasses and jewelry. Personal items should not be left on the patient’s meal tray or in the bed.
Smoking Provisions
Smoking is not permitted in the _________ or adjacent to entrances where passive smoke can affect building occupants. The specific guidelines that exist for patients who wish to smoke can be discussed with a nurse.
Telephones & Cellular Phones (ext. 8112)
Each patient room has a telephone. There is no charge for local calls, by dialing 9 and then the number. Dial 80 to place a toll call through MCIC (long distance, local long distance, third party, calling card or collect calls). Cellular phones are not permitted in patient care areas but can be used in public areas. Pay phones are available throughout the ___________.
Gift Shop…Flowers, Balloons, etc. (ext. 8247)
Flowers are welcome, except in ICU. Mylar (foil-like) balloons are acceptable, but latex balloons are not permitted because of the increasing number of latex allergies. The main lobby Gift Shop is stocked with stationery, toiletries, snacks, magazines, and other gifts.
Laboratory Tests (ext. 8397)
Blood draws or other laboratory test often are taken at unusual hours so that results can be ready for physicians to review early in the morning. This helps to ensure the most efficient and effective medical treatment.
Advance Directives (ext. 8344)
(Durable Power of Attorney for Health Care) Upon admission, patients will receive a brochure on Advance Directives, which is a formal document written before an illness or injury occurs. It permits individuals to record their wishes about medical treatment if they are later unable to speak for themselves. Please take the time to read this important document, which we are required by federal law to distribute. We want patients to understand their right to make decisions about medical treatment. Questions should be directed to the Social Worker or the Case Management Department.
Food & Nutrition (ext. 8422)
Because nutrition is important to a patient’s recovery, appetizing meals are prepared in accordance with dietary needs. Nutrition assistants attend to patients’ dining needs, and a clinical dietitian coordinates all diets. Three meals are served every day, at approximately 7:45 a.m., 11:45 a.m., and 5:45 p.m.
Pain Management
Successful pain relief requires cooperation between patients and caregivers. To accomplish this goal, doctors, nurses and therapists need the patients to be involved:
Tell us when the pain starts. Don’t wait until the pain becomes unbearable. Early involvement often reduces the reliance on medications. A change in position often helps to minimize pain.
Keep us informed on how well the “pain relieving” efforts are working. A patient’s honest evaluation of
the treatment is needed. The faster pain is relieved the sooner a patient can move about and regain his or
her strength.
Restraints

Patients typically are discharged at 11:00 a.m. at which time personal discharge arrangements should be made. Patients must remember to take their personal belongings, and to reclaim valuables stored in the safe upon discharge.
Patient Satisfaction: Concerns, Complaints, Compliments
Our customers are important! Patient rounds are conducted routinely in order to determine whether the quality of care is appropriate. Patients can dial “O” to reach the Nursing House Supervisor if they have a problem. Questionnaire surveys focus on the opinions of both in-patients and out-patients. Formal complaints—or compliments, if you wish—can be registered verbally or in writing and forwarded to the Patient Advocate (___) or Administration. The Medical Center has implemented a proactive approach to patient safety. Patients, families, and visitors are encouraged to share their opinions, needs, and perceptions of the risk that a patient might face, and to recommend improvements. Employees are encouraged to report issues that impact patient safety, without the fear of reprisal, in a non-punitive work environment. Visiting Hours
General Visiting (ext. 0)
Visiting Hours occur daily between 11 am and 8 pm. Two visitors are permitted at-a-time. Children are welcome to visit for 15 minutes if accompanied by an adult.
Pediatric Visiting (ext. 8342)
Parents may visit at any time. One parent may stay overnight when arrangements are made with the nursing supervisor. Parents must authorize others to visit, and they must check in at the nursing station before visiting the patient.

One or two immediate family members may visit for 10 minutes at-a-time, between 8 am and 10 pm.
Birthing Center (ext. 8336)
Father or coach may visit 24-hours-a-day; grandparents, between 8 am and 8 pm; and siblings, when accompanied by an adult, during regular Birthing Center visiting hours. Other visitors (over 16 years of age) are welcome 2 pm – 4 pm, and between 6 pm – 8 pm. Please remember that new moms need their rest!
Ambulatory Surgery Department (ext. 8309)
Two visitors are permitted at-a-time. Children under the age of 16 are not permitted. Patients must arrange for appropriate discharge transportation.
Patient Responsibilities
The quality of care provided depends partially on the patient. In addition to their rights, patients also have certain responsibilities, including the following:
Provision of Information – The patient is responsible to provide accurate and complete information concerning his or her present complaint, past medical history, and other matters relating to the health history. It is important to let it be known whether the course of treatment and what is expected of him or her is clearly understood.
Compliance With Instructions – The patient is responsible to follow the treatment plan established by his or her physician, including the instructions of nurses and other professionals who carry out the physician’s orders. The patient is responsible to keep appointments and to notify the hospital or physician if they are unable to do so.
Refusal of Treatment – The patient is responsible for his or her actions if the prescribed treatment is refused or the physician’s orders are not followed.
Hospital Charges – The patient is responsible to assure that the financial obligations related to his or her care are fulfilled as promptly as possible.
Hospital Rules and Policies – The patient is responsible to follow all hospital policies.
Respect and Consideration – The patient is responsible to be considerate and respectful of the rights and property of other patients and hospital employees.

The State Department of Health Services has described certain patients’ rights that must be protected in general acute care hospitals in Title 22, _________ _________________. This regulation requires hospitals and medical staffs to adopt a written policy on patients’ rights. A list of these patients’ rights must be posted in English and Spanish in appropriate places within the hospital so that such rights may be read by patients. This list must include, but need not be limited to, the patient’s rights to:
1. Exercise these rights without regard to sex, economic status, educational background, race, color, religion, ancestry, national origin, sexual orientation or marital status, or the source of payment for care.
2. Considerate and respectful care.
3. Knowledge of the name of the physician who has primary responsibility for coordinating the care and the names and professional relationships of other physicians and nonphysicians who will see the patient.
4. Receive information about the illness, the course of the treatment and prospects for recovery in terms that the patient can understand.
5. Receive as much information about any proposed treatment or procedure as the patient may need in order to give informed consent or to refuse this course of treatment. Except in emergencies, this information must include a description of the procedure or treatment, the medically significant risks involved in this treatment, alternate courses of treatment or non-treatment and the risks involved in each and to know the name of the person who will carry out the procedure or treatment.
6. Participate actively in decisions regarding medical care. To the extent permitted by law, this includes the right to refuse treatment.
7. Full consideration of privacy concerning the medical care program. Case discussion, consultation, examination and treatment are confidential and should be conducted discreetly. The patient has the right to be advised as to the reason for the presence of any individual.
8. Confidential treatment of all communications and records pertaining to the care and the stay in the hospital. Written permission must be obtained before the medical records can be made available to anyone not directly concerned with the care.
9. Reasonable responses to any reasonable requests made for service.
10. Leave the hospital even against the advice of physicians.
11. Reasonable continuity of care and to know in advance the time and location of appointments as well as the identity of persons providing the care.
12. Be advised if the hospital/personal physician proposes to engage in or perform human experimentation affecting care or treatment. The patient has the right to refuse to participate in such research projects.
13. Be informed of continuing health care requirements following discharge from the hospital.
14. Examine and receive an explanation of the bill regardless of the source of payment.
15. Know which hospital rules and policies apply to the patient’s conduct while a patient.
16. Have all patients’ rights apply to the person who may have legal responsibility to make decisions regarding medical care on behalf of the patient.
17. Designate visitors of his/her choosing, if the patient has decision-making capacity, whether or not the visitor is related by blood or marriage, unless:
a) No visitors are allowed.
b) The facility reasonably determines that the presence of a particular visitor would endanger the health or safety of a patient, a member of the health facility staff, or other visitor to the health facility, or would significantly disrupt the operations of the facility.
c) The patient has indicated to the health facility staff that the patient no longer wants this person to visit.
18. Have the patient’s wishes considered for purposes of determining who may visit if the patient lacks decision-making capacity and to have the method of that consideration disclosed in the hospital policy on visitation. At a minimum, the hospital shall include any persons living in the household.
19. This section may not be construed to prohibit a health facility from otherwise establishing reasonable restrictions upon visitation, including restrictions upon the hours of visitation and number of visitors.

Rogamos encarecidamente a nuestros pacientes que comuniquen cualquier tipo de preocupación o duda que pueda surgirles a cualquier miembro de nuestro personal, que intentará atenderles personalmente o poner el asunto en conocimiento de la persona indicada para resolverlo. Nuestros pacientes pueden hacer constar sus quejas de forma verbal o por escrito. Dichas quejas serán respondidas por el trabajador social encargado en los 14 días siguientes a su manifestación.
Derecho a acceder al Comité Bio-Ético:
En algunas ocasiones surgen conflictos relativos al trato que enfermeras, médicos y personal auxiliar dispensan a los pacientes o a sus familiares. En el caso de que dichos conflictos no pudieran resolverse de forma inmediata, el interesado podrá solicitar una reunión con nuestro Comité Bio-Ético contactando con el Jefe de enfermería.
Derecho a acceder a los servicios sociales:
Cada paciente tiene derecho a acceder a este tipo de servicios, por ejemplo a los servicios de protección a menores. Además cualquier paciente que tenga conocimiento de una situación de abuso, negligencia o hurto tiene derecho a cursar la correspondiente denuncia. El departamento de Servicios sociales prestará la asistencia necesaria a los pacientes que los requieran y a sus familiares.
Derecho a la atención espiritual:
La atención y el apoyo espiritual se recomienda y se ofrece. El departamento de servicios sociales ayudará a nuestros pacientes a contactar con un miembro del clero de la religión que profesen. Nuestro centro cuenta con una capilla/sala de recogimiento situada junto al vestíbulo principal.
Guía de servicios hospitalarios:
Salas de espera y aseos públicos.
Nuestro centro cuenta con numerosas salas, entre ellas nuestro comedor principal. Los aseos públicos se encuentran situados junto a las puertas principales y de emergencia. Pregunte a nuestros empleados por el emplazamiento de los aseos para minusválidos. Por favor, ayúdenos a cumplir la normativa de seguridad y contra incendios, mantenga despejados nuestros pasillos en todo momento. Por favor, por su propia seguridad, le rogamos que no deje a los niños solos sin la supervisión de un adulto en las áreas públicas.
Objetos Personales (ext. 8180)
En el momento de ingreso en el Centro, los pacientes sólo deben llevar consigo aquellos efectos personales que sean imprescindibles para garantizar la comodidad de su estancia en el hospital, como por ejemplo sus objetos de aseo personal. Todos los objetos de valor deben permanecer en sus hogares o mantenerse en las taquillas de seguridad situadas en el Departamento de Admisiones. El ______no se hace responsable de la pérdida de objetos personales como dentaduras postizas, gafas y joyas. Le rogamos que no deposite sus objetos personales ni en la bandeja de las comidas ni sobre su cama.
Regulación para los fumadores
Queda terminantemente prohibido fumar en todo el ___________, incluso en las zonas situadas junto a las entradas, ya que el humo puede afectar a los ocupantes del edificio y convertirlos en fumadores pasivos. Las líneas de actuación específicas que existen para los pacientes que desean fumar pueden consultarse con las enfermeras.
Teléfonos fijos y móviles (ext. 812)
Cada habitación está equipada con teléfono. Las llamadas locales son gratuitas y podrá efectuarlas marcando 9 antes del número telefónico. Marque 80 para poner una conferencia a través del MCIC (llamadas a larga distancia, a larga distancia locales, con cargo a terceros o mediante tarjeta telefónica). Queda prohibido el uso de teléfonos móviles en las áreas de atención al paciente, pero pueden utilizarse en las áreas públicas. El Centro cuenta con un amplio número de cabinas telefónicas repartidas por todo el Hospital.
Tienda de regalos, flores, globos, etc. (ext. 8247)
Las flores están permitidas en todas las habitaciones excepto en la UCI. Se permiten también todos los globos que no estén fabricados en látex. Éstos últimos no están permitidos en el Centro debido al creciente número de casos de alergia a dicho material. En la tienda de regalos del vestíbulo podrá encontrar revistas, aperitivos, elementos de aseo personal y todo tipo de regalos.
Pruebas de laboratorio (ext.8112)
Las muestras de sangre y otros tipos de pruebas de laboratorio se realizan a horas un tanto intempestivas, con el fin de que estén listas para que nuestros técnicos de laboratorio puedan analizarlas a primera hora de la mañana. Este proceso nos ayuda a ofrecerle un tratamiento médico más eficiente y efectivo.
Disposiciones previas (ext.8344)
(Disposiciones legales para la atención sanitaria). En el momento de su admisión, los pacientes reciben un formulario sobre disposiciones previas, que es un documento oficial que se redacta en previsión de una posible situación de emergencia. Este documento permite que los pacientes especifiquen el tipo de tratamiento médico que desean recibir en caso de no poder pronunciarse por sí mismo. Por favor, le rogamos que preste especial atención y cuidado a la lectura de este importante documento que la Ley Federal obliga a distribuir a todos aquellas personas que ingresan en un centro hospitalario. Queremos que nuestros pacientes entiendan que tienen derecho a decidir sobre el tipo de tratamiento médico que desean recibir. Cualquier pregunta o dudas que puedan surgirles deberán dirigirlas al trabajador social o al departamento de gestión de casos.
Alimentación y nutrición (ext.8422)
Debido al papel fundamental que juega la nutrición en la recuperación de un paciente, en nuestro Centro procuramos preparar las comidas más apetitosas conforme a la dieta de cada paciente. Nuestros asistentes nutricionales atienden las necesidades alimenticias de nuestros pacientes, y un dietista clínico coordina todos los regímenes alimenticios. Se sirven tres comidas al día a las 7:45 am, 11:45 am, y a las 5:45 pm.
Comportamiento ante el dolor:
Para que nuestro equipo tenga éxito a la hora de aliviar su dolor, el proceso requiere de la cooperación entre usted y nuestros auxiliares. Para lograr esta meta, nuestros doctores, enfermeras y terapeutas necesitan su colaboración: díganos cuando comienza el dolor, no espere a que se haga insoportable. En algunas ocasiones , el hecho de frenar el dolor con analgésicos demasiado pronto hacen que éstos pierdan su efectividad. En ciertos casos un mero cambio de postura puede ayudar a disminuir el dolor.
Manténganos informados de los avances en el proceso de control del dolor. En todas las ocasiones se necesita una evaluación honesta del paciente. Cuanto antes se elimina el dolor antes puede moverse el paciente y recuperar su fuerza.
Restricciones:
El centro médico no tiene restricciones de movimiento, excepto en los casos en los que sea necesario por razones clínicas.
Servicios de traducción e interpretación:
Los pacientes de habla no inglesa tienen acceso a varios tipos de servicios de traducción. Muchos de nuestros empleados hablan idiomas extranjeros. También tenemos a su disposición dispositivos electrónicos de comunicación, al igual que servicios de interpretación de lengua de signos y servicios de ayuda a las personas con problemas de audición. Nuestros auxiliares se esmeran por ayudar siempre que identificación situaciones que requieren necesidades interpretativas, y los familiares de nuestros pacientes pueden ayudar en el proceso de traducción o notificar al centro médico que requieren este tipo de servicios.
Gestión de casos (ext.8159) y Servicios sociales.
Los gestores de casos y los trabajadores sociales cooperan con nuestros expertos para asegurar que el centro responde a todas las necesidades sociales y psicológicas de los pacientes, y de que les aporta el nivel de atención continua necesaria durante todo su proceso de recuperación tras el alta hospitalaria. Aquellos pacientes que deseen información sobre cómo solicitar nuestro formulario de disposiciones previas, su ingreso en grupos de apoyo o cualquier otro recurso comunitario deben poner en contacto con el auxiliar que se les haya asignado. Generalmente las altas suelen darse a las 11 am, hora en la que deben efectuarse todos los preparativos para la salida del hospital. Los pacientes no deben olvidar reclamar sus objetos personales y aquellos objetos valiosos que hayan depositado en las taquillas de seguridad antes de abandonar el hospital.
Satisfacción de nuestros pacientes: dudas, quejas y agradecimientos:
¡Nuestros clientes son importantes¡ por esta razón realizamos rondas rutinarias visitando a nuestros pacientes para determinar su grado de satisfacción con el servicio y atención que reciben. En caso de existir algún problema, los pacientes pueden marcar “O” para contactar con el supervisor de enfermería. Nuestras encuestas de opinión se centran tanto en los pacientes internos como en los externos. Las quejas formales, o los agradecimientos en caso de que desee expresarlos, se pueden realizar de forma escrita o verbal y remitirlas al servicio de atención al cliente (______) o a nuestra administración. La línea permanente de atención al cliente se encuentra disponible a través del teléfono _______ para pacientes de habla inglesa o a través del _____ para nuestros pacientes de habla hispana.
Seguridad del paciente (ext.8557)
Nuestro centro ha implementado un servicio proactivo para garantizar la seguridad de nuestros pacientes. Animamos a nuestros pacientes, a sus familiares y a nuestros visitantes a compartir sus opiniones, preocupaciones y necesidades sobre los riesgos que el paciente tenga que afrontar, y les recordamos que estamos abiertos a cualquier mejora del servicio que podamos realizar. También animamos a nuestros empleados a informarnos sobre cualquier tema que pueda afectar a la seguridad del paciente, sin temor a sufrir ninguna represalia o castigo en su entorno laboral. La línea permanente sobre temas de seguridad del paciente se encuentra disponible a través del teléfono _____ y está abierta las 24 horas.
Horas de visita:
Visitas generales (ext. 0)
Las horas de visita se realizan diariamente entre las 11 am y 8 pm. Se permite el acceso a la habitación de dos visitantes a la vez. La visita de niños está permitida durante 15 minutos al día siempre y cuando estos estén acompañados de un adulto.
Visita pediátrica (ext. 8342)
Los padres podrán visitar a sus hijos en la unidad de pediatría a cualquier hora del día. Uno de los progenitores podrá quedarse con el niño durante la noche siempre y cuando se hayan realizado con anterioridad los arreglos necesarios con el supervisor de la unidad de enfermería. Los padres podrán autorizar visitas de terceros y deberán presentarse en la unidad de enfermería antes de visitar al paciente.
Unidad de cuidados intensivos (ext. 8346)
De 8 de la mañana a 10 de la noche los pacientes de esta unidad podrán recibir visitas de 10 minutos de duración de uno o dos familiares directos.
Centro de maternidad (ext. 8336)
El padre o la pareja podrá visitar a la paciente y al niño 24 horas al día; los abuelos, entre las 8 am y 8 pm; y los hermanos o hermanas del recién nacido podrán visitar la unidad de maternidad durante las horas de visita siempre que estén acompañados de un adulto. Otros visitantes (personas mayores de 16 años) podrán acudir al centro entre las 2 pm – 4 pm y entre las 6 pm – 8 pm. Por favor, recuerden que las nuevas madres necesitan descansar.
Departamento de cirugía ambulatoria (ext. 8309)
Se permite la visita de dos personas al mismo tiempo. No se permite la visita de niños menores de 16 años. Los pacientes deben gestionar su propio transporte una vez reciben su alta médica.
Responsabilidades del paciente.
La calidad de la atención que nuestro centro ofrece, depende en parte del paciente. Además de sus derechos, el paciente también tiene ciertas responsabilidades entre las que se incluyen:
Informar – El paciente es responsable de ofrecer toda la información necesaria relativa a sus quejas, historial médico y otros asuntos relacionados con su historial sanitario. Para nosotros es importante disponer de esta información durante su tratamiento y también es importante que sepamos que el paciente ha entendido claramente lo que se espera de él.
Cumplir las pautas médicas – El paciente es responsable de seguir el tratamiento establecido por su médico incluyendo las pautas indicadas por enfermeras y otros profesionales que siguen órdenes del especialista. El paciente es responsable de asistir a sus citas y de informar al hospital o al médico encargado cada vez que no pueda asistir a una.
Rechazar el tratamiento – El paciente será enteramente responsable de las consecuencias que sufra si rechaza seguir el tratamiento prescrito por el especialista.
Pagar los costes hospitalarios – El paciente es responsable de cumplir lo antes posible con las obligaciones financieras relativas a su atención sanitaria.
Cumplir con las normas y políticas hospitalarias – el paciente es responsable de seguir las normas del centro.
Respetar y tratar con consideración – El paciente es responsable de tratar con consideración y de respetar los derechos y la propiedad de los empleados del hospital y de los otros pacientes.
Derechos del paciente en el Estado de _________
El departamento estatal de servicios sanitarios ha establecido ciertos derechos del paciente que deben ser respetados y protegidos en los centros del atención hospitalaria según el artículo 22 del _____________________. Esta regulación requiere que los hospitales y centros médicos adopten una política escrita sobre estos derechos del paciente. La lista de dichos derechos debe publicarse en inglés y en español en los lugares apropiados del hospital, de forma que los pacientes puedan leerla siempre que lo deseen. Dicha lista debe incluir los siguientes derechos del paciente, aunque no por ello debe dejar de incluir otros que el hospital considere necesario mencionar:
1. Estos derechos son válidos para toda persona que haga uso de estas instalaciones hospitalarias, sin distinción de sexo, nivel económico, nivel cultural, raza, color, religión, nacionalidad, orientación sexual, estado civil o fuente de la que proceda la financiación de sus servicios médicos.
2. Derecho a un trato considerado y respetuoso.
3. Derecho a conocer el nombre del médico encargado de coordinar su proceso de atención médica y los nombres de cualquier otra persona implicada en dicho proceso.
4. Derecho a recibir información sobre su enfermedad, evolución del tratamiento y posibilidades de recuperación en un lenguaje que el paciente pueda entender.
5. Derecho a recibir tanta información como pueda dársele sobre el tratamiento propuesto o los procedimientos a los que el paciente deberá someterse con el fin de dar su consentimiento consciente o rechazar dichos procedimientos y tratamientos. Excepto en los casos de emergencia, esta información debe incluir una descripción detallada del proceso de tratamiento, de los riesgos médicos significativos que implica, de los periodos de descanso y de los riesgos que éstos implican. El paciente también deberá estar al corriente del nombre de la persona que llevará a cabo el procedimiento o tratamiento.
6. Derecho a participar activamente en toda decisión que afecte a su atención médica. Este derecho contemplado por la ley, también implica el derecho a rechazar el tratamiento.
7. Derecho a la privacidad de su historial médico y tratamiento. La discusión de su caso, consultas, exámenes y tratamiento son estrictamente confidenciales y deben conducirse con total discreción. En caso de que otra persona deba estar presente durante alguno de los procesos anteriormente mencionados, el paciente debe ser avisado con anterioridad de la presencia de dicha persona y debe conocer la razón por la que ésta está presente.
8. Derecho a un tratamiento confidencial de todos los registros escritos y conversaciones relativas a su atención médica y a su estancia en el centro hospitalario. Se requiere del permiso escrito del paciente para consultar el caso o hacer públicos los archivos médicos ante cualquier persona no implicada de forma directa con su caso.
9. Derecho a obtener respuestas razonables a cualquier petición de servicio que haya realizado el paciente de forma razonada.
10. Derecho a dejar el hospital incluso cuando esto vaya en contra de los consejos de los expertos.
11. Derecho a una continuidad razonable en la atención médica y derecho a conocer con antelación la hora y el lugar de sus citas así como la identidad de las personas que van a prestarle atención sanitaria.
12. Derecho a ser informado en caso de que el hospital o los expertos al cuidado de su caso propongan incluirle en un programa de experimentación en humanos que afecte a su tratamiento. El paciente tiene derecho a rechazar formar parte de este tipo de proyectos de investigación.
13. Derecho a recibir información sobre las pautas sanitarias a seguir una vez recibida el alta del hospital.
14. Derecho a examinar y recibir explicación sobre la factura hospitalaria, independientemente de su fuente de pago.
15. Derecho a conocer las reglas y políticas hospitalarias aplicables a la conducta del paciente durante su estancia en el centro hospitalario.
16. Derecho a que la persona con responsabilidad legal para la toma de decisiones sobre el bienestar y el cuidado médico del paciente respete todos sus derechos.
17. Derecho a elegir los visitantes que desea recibir, en caso de tener capacidad de decisión. Este derecho será válido para cualquier visitante con relación de consanguinidad o matrimonio con el paciente, excepto en los siguientes casos:
a) Visitantes no permitidos por el centro.
b) En caso de que los médicos hayan determinado que la presencia de un visitante en concreto puede poner en peligro la salud o la seguridad del paciente, la salud o la seguridad de un miembro del personal sanitario o las de cualquier otro visitante del centro hospitalario; o en caso de que el visitante pueda alterar los procedimientos que se llevan a cabo en el centro hospitalario.
c) En los casos en los que el paciente haya indicado al personal sanitario que no desea recibir más la visita de una persona en concreto.
18. Derecho a que los deseos del paciente sean tenidos en cuenta a la hora de determinar quien puede visitarle en caso de perder la capacidad decisoria y derecho a que dichos deseos se hagan constar en la política del hospital respecto a sus visitas. Como mínimo, el hospital debe incluir entre los visitantes del paciente a todas aquellas personas que vivan bajo su mismo techo.
19. Derecho a que ningún miembro del equipo hospitalario emplee su potestad para determinar las personas autorizadas a visitar al paciente, para establecer restricciones fuera de lo razonable, incluyendo cualquier tipo de restricción que se haga respecto a las horas de visita y al número de visitante.

Source text - English CHILE COLORADO (RED CHILE SAUCE)
7 Dried California Chiles
3 cups water
1/4 cup Shallots, finely chopped
1/2 tsp. dried oregano, crumbled
Cumin powder, garlic and salt to taste
Break off stems and crack the chile pods, shaking out most of the seeds; rinse pods and pat dry. In saucepan, soak cleaned pods in 3 cups of boiling water, covered. Allow at least 30 minutes to soften. When pods are light red and spongy, purée the pods with the 3 cups rehydrating water in blender for 1 1/2 minutes.
Over a saucepan, pour mixture into a sieve, stirring continuously with a wooden spoon until all that remains is a residue of seeds and fragments of chile skin. The resulting chile purée will have the color and texture of a smooth tomato sauce. Add all other ingredients and season to taste. To cook sauce, simmer for at least 10 minutes. The flavor will improve over time as the spices and chile blend. Chile Colorado keeps well in your refrigerator or can be successfully frozen and thawed. To use, spoon over fried eggs served on a grilled tortilla for "Huevos Rancheros" . Makes 3 cups sauce.
Translation - Spanish SALSA DE CHILE COLORADO
Ingredientes para tres tazas de salsa:
7 Chiles secos californianos
3 tazas de agua
¼ taza de chalotas muy picadas.
1/2 cucharadita de oregano mezclado con comino en polvo, ajo y sal al gusto. .
Quite los rabos de los chiles, parta las vainas y agítelos para sacar las semillas; lave las vainas y séquelas. Ponga a hervir las vainas limpias en una cazuela con tres tazas de agua. Tape la cazuela y deje hervir las vainas durante 30 minutos hasta que se ablanden. Cuando adquieran un color rojo brillante y una textura esponjosa, bátalas durante 1 minuto y medio añadiéndoles el agua de la coción.
Emplee un tamiz sobre una cazuela para colar la mezcla, y remueva con una cuchara de madera hasta que sólo queden residuos de piel de los chiles y semillas. El puré de chile resultante debe tener un color y textura similares a los de una salsa de tomate suave. Añada el resto de los ingredientes y sazone al gusto. Para cocer la salsa, caliente la mezcla a fuego lento al menos durante 10 minutos. La salsa adquirirá su sabor característico cuanto más tiempo se dejen ligar las especias con el chile. El Chile Colorado puede conservarse en la nevera o en el congelador y descongelarse cuando se desee. Puede emplear una cucharada de salsa para rociar unos huevos fritos dispuestos sobre una tortilla de maiz precalentada y obtener así unos “Huevos Rancheros”
