In 2004m the first results of the British Million Women Study, a prospective cohort study that included more than a million women, showed that women using tibolone had nearly 1.5 times the risk of breast cancer as women who never used hormone replacement therapy. The difference was statistically significant.
In 2005, after a mean follow-up of 3.4 years, there were 31 cases of endometrial cancer per 10 000 women who used tibolone. This was almost double the level of risk experienced by women who never used hormone replacement therapy.
Tibolone has no proven advantage over estrogen-progestin hormone combinations for treatment of symptoms of menopause, either in terms of efficacy or cancer risks. It is associated with an increase risk of stroke. There is no justification for using tibolone as a treatment for menopausal symptoms.
TIBOLONE AND STROKE
In October 2005 an interim analysis of a randomized placebo-controlled trial of tibolone in osteoporosis prevention indicated an excess of strokes in the tibolone group (1). After a mean follow-up of 2.4 years, there were 4.26 strokes per 1000 women-years in the tibolone group, compared to 1.64 in the placebo group (p=0.01).
Tibolone has no proven advantage over hormone replacement therapy for treatment menopausal symptoms, in terms of efficacy or cancer and stroke related adverse effects (2, 3). There is currently no reason to expose women to the dangers associated with tibolone.

Las conclusiones preliminares del año 2004 del estudio británico “Un millón de mujeres”, trabajo prospectivo y de cohorte, que incluyó a más de un millón de mujeres, dieron como resultado que el riesgo de padecer cáncer mamario en mujeres bajo tratamiento con tibolona, era 1,5 veces mayor que en mujeres que nunca habían recibido terapia de reemplazo hormonal. La diferencia es estadísticamente significativa.
Luego de un seguimiento promedio de 3,4 años, en el año 2005, se detectaron 31 casos de cáncer de endometrio cada 10.000 mujeres que recibían tibolona, lo que constituye casi el doble del riesgo, comparado con aquellas mujeres que nunca habían recibido terapia de reemplazo hormonal.
En otro estudio de cohorte británico, utilizando una base de datos de práctica médica general, el riesgo de cáncer de endometrio entre 4995 mujeres bajo tratamiento con tibolona fue casi el doble, comparado con mujeres que recibieron terapia de reemplazo hormonal con estrógeno-progestágeno en forma secuencial.
La tibolona no ha mostrado ventajas sobre la combinación estrógeno-progestágeno para el tratamiento de los síntomas de la menopausia, tanto en términos de eficacia como de disminución del riesgo de cáncer. Además, se asocia con un aumento del riesgo de infarto. No existe justificativo para el uso de tibolona en el tratamiento de los síntomas menopáusicos.
TIBOLONA E INFARTO
En octubre de 2005, un análisis interino de un estudio aleatorio con placebo para control, sobre tibolona como prevención de la osteoporosis indicó un exceso de infarto en el grupo de tibolona (1).
