Source text - English C5-C6 INTRADURAL LESION
Clinical evolution: two years (the patient underwent thyroidectomy at the age of 59). Before hospitalization, she experienced initial cervical discomfort, which progressively worsened, leading to persistent bilateral arm numbness and tingling, especially during neck flexion. Symptoms were associated with intermittent headaches and mild ataxia.
Neurological examination: partial tetraparesis: upper limb strength reduced, particularly in the right arm. Mild spasticity noted in both legs. Positive Hoffman’s sign bilaterally. Sensory deficit in C6 dermatome. From C7 downward, reduced pinprick and vibration sense.
Neuroradiological examination: MR revealed an intradural, intramedullary lesion at C5-C6, causing moderate spinal cord compression. The lesion was oval, well-defined, and hyperintense on T2- weighted images. No evidence of hemorrhage or edema in adjacent structures.
Operation: complete resection of the intramedullary mass through posterior cervical laminectomy. The lesion appeared encapsulated and was successfully dissected from the surrounding cord tissue.
Histological examination: consistent with ependymoma, WHO Grade II.
Follow-up: post-operative period marked by gradual improvement in motor strength and coordination. At one-month follow-up, the patient regained full function of the right upper limb and showed improved gait stability. Six months later, neurologic function was nearly normal, with only mild residual paresthesia in the fingertips.
Translation - Italian LESIONE INTRADURALE C5-C6
Evoluzione clinica: due anni (il paziente si è sottoposto a una tiroidectomia all'età di 59 anni). Prima del ricovero, soffriva di un dolore cervicale iniziale, che è progressivamente peggiorato, causando un persistente intorpidimento bilaterale e formicolio bilaterale alle braccia, soprattutto durante la flessione del collo. I sintomi erano associati a cefalea intermittente e atassia lieve.
Esame neurologico: tetraparesi parziale: riduzione della forza degli arti superiori, in particolare del braccio destro. Lieve spasticità riscontrata in entrambe le gambe. Segno di Hoffman positivo bilateralmente. Deficit sensoriale nel dermatoma C6. Da C7 in giù, ridotta sensibilità al dolore puntorio e alle vibrazione.
Esame neuroradiologico: alla risonanza magnetica, lesione intradurale intramidollare a livello di C5-C6, che causava una moderata compressione del midollo spinale. Lesione ovale, ben definita e iperintensa nelle immagini pesate in T2. Nessuna evidenza di emorragia o edema nelle strutture adiacenti.
Intervento: resezione completa della massa intramidollare mediante laminectomia cervicale posteriore. La lesione appariva incapsulata ed è stata sezionata con successo dal tessuto midollare circostante.
Esame istologico: compatibile con ependimoma, grado II secondo la classificazione OMS.
Catamnesi: periodo post-operatorio caratterizzato da un graduale miglioramento della forza motoria e della coordinazione. Al follow-up di un mese, il paziente ha recuperato la piena funzionalità dell'arto superiore destro e mostra una migliore stabilità nell'andatura. Sei mesi dopo, la funzione neurologica è quasi normale, con solo una lieve parestesia residua sulla punta delle dita.
