Source text - English The rapid spread of Zika virus through the Americas and its devastating consequences for pregnant women and infants have precipitated an international, multisectoral response. Current prevention strategies focus on mosquito control, protection of the blood supply, barrier protection during sex, and other forms of contraception. When this explosive epidemic abates, Zika virus could remain endemic in many countries, where the risk to pregnant women, the general public, and travelers will persist. Therefore, a safe and effective vaccine is essential.
Development of a safe, effective Zika vaccine should be feasible. Vaccines against related flaviviruses, such as yellow fever and Japanese encephalitis, have been developed and deployed, and Zika infection appears to generate protective immunity in nonhuman primates.1 Scientific feasibility, however, does not ensure successful development. An efficient development pathway must be delineated, including the optimal ways to evaluate the safety, immunogenicity, and effectiveness of vaccine candidates for intended target populations.
The primary goal of Zika vaccination is to prevent infection and protect against serious sequelae of the virus, particularly fetal congenital anomalies following in utero infection; however, given the relatively low frequency of these events in relation to the total number of infections, generating evidence of efficacy against these sequelae in pre licensure trials may not be feasible within a reasonable time frame. Demonstrating that vaccination averts congenital anomalies will most likely require postlicensure studies.
Prevention of congenital anomalies through vaccination of women during pregnancy faces several challenges. First, to protect the developing fetus, one must achieve protective immunity before the time of peak vulnerability, which is probably during the first and early second trimesters (although complications resulting from infections later in pregnancy have been reported).2 Furthermore, many women are unaware of their pregnancies until well into the first trimester. Although replicating live-virus vaccines may be protective after a single dose, these are generally not good candidates for vaccines administered during pregnancy.

لابد ان تطوير مصل امن و فعال لفيرس زيكا ان يكون عملي . انتشرت و تتطورت الامصال ضد الفيروسات المصفرة مثل حمي الصفر و التهاب الدماغ الياباني .ولقد ظهر ان لفصيلة المقدمات " القرود "لديهم مناعة ضد هذا المرض. فعلميا لا يمكن ذمان نجاح تطوير هذا المصل علي الرغم من ذلك .لذلك لابد ان يكون هذا التطوير للمصل دقيق و يشمل الطرق المثلي للحضول علي مصل يكون امن و فعال ومناسب مناعيا بالنسة للاشخاص المستهدفين
ان الهدف الاساسي لهذا المصل هو منع الاصابات و الحماية ضد عقبول "تطوير "هذا الفيرس وخصوصا الاجنة المشوهة خلقيا نتيجة اصابة الرحم . فبالنظر الي التردد المنخفض نسبيا لهذه الاحداث مقارنة بعدد الاصابات الكلي , يعطينا دليل واضح ان وجود هذه الدراسات الغير مرخصة لن تفيد بدون تحديد فترة منية معقوله لانهائها . لذلك فان تجنب تكون الاجنة المشوهة خلقيا يستلزم دراسات مرخصة
ان حقن المراه الحامل بهذا المصل لمنع الاجنة المشوهة خلقيا اثناء الحمل يضعنا امام عدة تحديات . اولها ،لكي احمي هذا الجنين لابد ان يتم حقن المراه خلال الربع الاول او بداية الربع الثاني من الحمل ( علي الرغم من انه تم رصد اصابات في اوقات متاخرة من الحمل ) . بالاصافة الي ان هناك العديد من النساء تكون غير مدركة بالحمل الا في نهاية الربع الاول من فترة الحمل .فعلي الرغم من ان تكرار المصل يكون وقائي فقط بعد اول جرعة ، الا ان هناك قليل من المتطوعين ياتون لاخذ جرعة من المصل اثناء الحمل
