Source text - French Selon Peter PIOT ( ancien directeur de l'ONUSIDA) "Nous ne devons jamais oublier que le SIDA est un problème exceptionnel qui continuera d'exiger une riposte exceptionnelle de nous tous, aujourd'hui et au cours des décennies à venir".
De façon générale, on doit retenir trois (3) points essentiel pour comprendre la situation dans la zone Ashoka Sahel.
1): Le taux de couverture encore faible des services de la zone en matière de prévention, traitement, soin et soutien face au VIH met en danger les perpectives d'atteindre l'accès universel et les objectifs du millénaire pour le développement ( OMD 4, OMD 5, OMD 6) .
2): La perspective des taux de sero-prevalence faible de la région Sahel de Ashoka cache l'urgence de répondre aux défis considérables pour l'accès universel et détourne l'attention et l'appui des donateurs classiques de la région.
3): La société civile a prêché depuis le début de l'épidémie du SIDA par l'exemple et la mobilisation. Elle doit se mobiliser pour la prise en compte des populations clefs(homosexuels, professionnels de sexe et usagers de drogue). Et surtout dans l'accompagnement à autonomisation des personnes vivant avec le VIH/SIDA et à leurs communautés à partir des activités génératrices de revenus (AGR).
De façon générale, la prévalence chez l'adulte tourne autour de 2% avec des extrêmes de 4% en côte d'ivoire et de 0,4% au Senegal. Des tendances aggravées par des phénomènes comme la pauvreté chronique, l'analphabétisme, la violence sexuelle et le genre, la stigmatisation et la discrimination constituent des déterminants majeurs de la propagation de l'épidémie au VIH.
Il faut aussi noter que les crises politiques et les conflits en côte d'ivoire et au Mali ont plongé la région dans une crise humanitaire sans précédent avec les déplacements internes et externes des personnes.
Selon l'ONUSIDA, dans la région Afrique de l'Ouest et centrale (AOC), moins de 50% des jeunes âgés de 15 à 24 ans utilisent un préservatif lors des rapports sexuels. La couverture de la prévention de la transmission mère - enfant est d'environ 1,3%. L'accès aux traitements ARV est de 16% ( 47% au Mali).

Broadly speaking, we must consider three (3) essential points in order to understand the situation in the Ashoka Sahel area.
1): The prospect of attaining universal access for prevention, treatment, care and support for HIV sufferers is being put in danger by the level of, still insufficient, service in the area (MDG 4, MDG 5, MDG 6).
2): The view of weak, zero-prevalence levels in Ashoka’s Sahel region is concealing the urgency that is needed to respond to the considerable challenge of gaining universal access, and to redirect the attention and support of the benefactors.
3): Since the beginning of the AIDS epidemic, civil society has led by example and mobilisation. It must prepare for the most at risk populations (homosexuals, sex workers and drug users), and in particular, consider how to support and encourage the independence of those already living with HIV/AIDS and their communities, through revenue generating activities.
Broadly speaking, occurrence of the disease in adults is around 2%, with extremes of 4% on the Ivory Coast and 0.4% in Senegal. These trends are exaggerated by the incidence of chronic poverty, illiteracy, sexual violence and other sexually related crimes, stigmatism and discrimination; all key factors in the spread of HIV.
We must also remember the political crises and conflicts on the Ivory Coast and in Mali have caused the region to plunge into an unprecedented humanitarian crisis with people being uprooted both within and around the region.
According to UNAIDS, less than 50% of young people aged between 15 and 24 use a condom during sexual intercourse in the regions of West and Central Africa (WCA). Prevention of mother-child transmission is around 1.3%. Access to ARV treatments is 16% (47% in Mali).
