Abstract :
De nombreuses techniques chirurgicales depuis des dizaines d’années ont été mises au point afin d’élever la membrane sinusienne en cas de faible hauteur résiduelle d’os. Nous avons voulu comparer deux techniques d’élévation sinusienne par abord crestal : La technique Intralift en utilisant la piézochirurgie et la technique de Summers en utilisant l’ostéotome.
Par expérimentations sur pièces anatomiques humaines fraîches nous avons recherché
Si la membrane sinusienne de Schneider est mieux préservée dans l’une des techniques par rapport à l’autre ;
Si le temps de réalisation est plus rapide sur l’une des techniques par rapport à l’autre ;
Si la technique Intralift est d’emblée à la portée d’un opérateur novice comme d’un opérateur expérimenté ;
Apres l’analyse des résultats nous avons émis des hypothèses sur le caractère plus sécurisant de la technique Intralift sur la membrane sinusienne par rapport à la technique de Summers ; Le temps de réalisation des deux techniques serait similaire et que la technique ultrasonore semble être plus facile à réaliser pour un opérateur novice que la technique de Summers.
1- Introduction :
Le développement de l’implantologie au cours de ces dernières années, a conduit à envisager des techniques chirurgicales de plus en plus sophistiquées.
De nombreuses techniques chirurgicales depuis des dizaines d’années ont été mises au point afin d’élever la membrane sinusienne en cas de faible hauteur résiduelle d’os.
Depuis 2001 le Pr Vercellotti [13, 14] a décrit une technique d’élévation par abord crestal utilisant les Ultrasons, qui s’avèrerait être une approche atraumatique d’élévation du plancher sinusien[4].
Nous avons réalisé une étude par expérimentation sur pièces anatomiques humaines fraîches pour déterminer si cette approche permet de diminuer le risque d’effraction de la membrane sinusienne par rapport à la technique de référence de Summers.
2- Matériel et méthode :
Notre étude a consisté à simuler différents protocoles sur pièces anatomiques humaines au laboratoire d’anatomie de l’université Bordeaux 2.
L’étude a été réalisée sur 12 pièces anatomiques fraîches. L’âge moyen des sujets est de 70 ans.
Sur chaque pièce anatomique nous avons réalisé une technique Intralift sur un sinus, et sur l’autre sinus la technique de Summers a été réalisée afin de comparer les résultats.
Matériel
Nous avons utilisé divers matériels pour la réalisation de cette étude :
Appareil photo avec flash annulaire et objectif macro (PENTAX K7, Objectif SIGMA MACRO 105 mm F2.8 EX DG, Flash annulaire SIGMA EM-140 DG).
Pour la technique Intralift : des fraises boules pour contre angle, des forets guides de diamètre 2mm,deux appareils Implantcenter 2(Satelec Actéon), deux contre angle classiques et deux contre angle piézo (Satelec Actéon), deux jeux d’insert TK1 à TK5 (Satelec Actéon), un Scanner Sensation 16, SIEMENS (Herlangen Germany), un Logiciel Dental pour acquisition et reconstruction des coupes,
Pour la technique de Summers : deux kits d’ostéotomes, deux kits de forage, deux maillets (Henry Schein ASA dental), deux kits de chirurgie, deux seringues à os (diamètre 2,5 mm), des éponges collagènes,
Pour le comblement sous-sinusien : du Bio oss factice (Geistlich pharma France),
Du Fil de suture (MERSILK 1-0),
Deux pieds à coulisses électroniques.
Méthode
Chronologiquement nous avons suivi les étapes suivantes :
Détermination des critères d’inclusion et d’exclusion des pièces anatomiques :
Critères d’inclusion :
Edentement bilatéral du secteur prémolo-molaire maxillaire ;
Maxillaire présentant une résorption osseuse des crêtes alvéolaires ;
Maxillaire possédant une hauteur de crête résiduel inférieur à 9mm.
Critères d’exclusion :
Sinus pathologiques ;
Maxillaire possédant une hauteur de crête résiduelle supérieure à 9mm ;
Sinus présentant des septums dans la zone d’intervention.
Scanners pré-opératoires :
Les scanners des 12 pièces anatomiques ont été réalisés au service de radiologie du Centre Hospitalier Universitaire de l’Hôpital Haut-Levêque à Pessac.
L’analyse des scanners pré - opératoires était minutieuse afin de décrire les caractéristiques de chaque sinus. La hauteur et la largeur de la crête sous-sinusienne ont été mesurées et enregistrées pour chaque pièce anatomique. Les pièces sélectionnées présentaient des hauteurs d’os allant de 1 à 9 mm. Cette hauteur correspond à la distance entre le plancher sous-sinusien et le sommet de la crête osseuse édentée. Nous avons également évalué la densité osseuse de chaque secteur par mesure Hounsfield avec le logiciel Dental Registred Siemens. Nous avons ensuite corrélé ces valeurs avec la classification de densité osseuse de Misch afin d’obtenir le type d’os de chacune des pièces anatomiques [5, 6, 9]. Les secteurs d’interventions ont ainsi été définis.
Après l’analyse des scanners pré – opératoires, les interventions se sont déroulées en 2 temps :
Elevation d’un lambeau muco-périosté:
Le lambeau intégral permettait de découvrir l’ensemble de la crête maxillaire et une meilleure visibilité des sites d’interventions (Dissection palais 1 à 3.JPG).
Marquage des repères sur la crête :
N’ayant aucune dent résiduelle, nous avons fait des marquages sur toutes les crêtes osseuses pour servir de repères lors de l’intervention (Marquage crêtes 1 à 3.JPG)
Nous avons mesuré sur les scanners pré-opératoires la longueur partant de la partie distale du foramen incisif jusqu’à la zone interventionnelle prédéfinie. Ces longueurs ont été reportées sur un fil de suture rigide
(MERSILK 1-0) à l’aide d’un pied à coulisse électronique, et ensuite sur le sommet de la crête osseuse de la pièce anatomique.
Simulation de l’acte sur pièce anatomique :
Les simulations chirurgicales ont été réalisées par deux opérateurs indépendants. Les protocoles réalisés étaient conformes aux recommandations des deux techniques [1, 10-12].
Scanners post-opératoires :
Les scanners post opératoires ont été réalisés avec la même machine (Scanner Sensation 16, SIEMENS, Herlangen Germany) en pré-opératoire et en post-opératoire et par le même opérateur.
Les acquisitions et les reconstructions des différentes coupes ont été réalisées à l’aide du même logiciel Dental Registered Siemens (voir DENTAL-ACQUIS.JPG et DENTAL-RECON.JPG).
Enregistrement des données post-opératoires :
Les données ont été recueillies dans des tableaux regroupant tous les paramètres qui étaient à évaluer (Tableau n° 13 et 14).
3- Résultats :
Les paramètres essentiels qui étaient à comparer étaient :
Le temps d’intervention ;
L’intégrité (ou non) de la membrane sinusienne.
Ces paramètres étaient à comparer non seulement avec le même opérateur, mais aussi entre les deux opérateurs.
L’aspect de la membrane et du soulevé du plancher pouvaient être appréciés en comparant le scanner avant et après l’intervention (voir coupes coronales 9 post op D intralift, 9 préop D intralift et 12 postop D summers, 12 préop D summers).
Analyses statistiques :
Hypothèses :
La technique Intralift est plus aisée pour un opérateur profane que la technique d'ostéotomie crestale (Summers).
La durée d’intervention est plus courte dans la technique Intralift que dans celle de Summers.
La technique Intralift préserve plus la membrane sinusienne (plus sécurisante) que celle de Summers.
Méthode : L’ensemble des tests a été réalisé avec le logiciel Matlab (R2010b) à l’unité CNRS-UMR 5287 de l’Université Bordeaux 2.
Comparaison des temps d’intervention des deux opérateurs en fonction de la méthode :
Préalablement un test de Bartlett a été réalisé pour vérifier l’homoscédasticité des données.
Ces tests (Bartlett) nous ont permis de valider les différentes analyses (Tableaux 1 et 2).
Une analyse de la variance multifactorielle a été réalisée, avec comme premier facteur l’opérateur (BE vs. JL), et comme second facteur la technique utilisée (Intralift vs. Summers) (Tableau 3).
On constate un effet de l’opérateur avec un temps d’expérimentation significativement plus court pour l’expert que pour le profane (p=0.0349).
Une courbe d’apprentissage est donc nécessaire pour l’utilisation des deux techniques. Il existerait donc une corrélation entre la dextérité de l’opérateur et le temps de réalisation des interventions dans les deux techniques. Mais on ne peut pas dire qu’une technique est plus rapide qu’une autre, même en différenciant le niveau d’expertise de l’opérateur. (Tableau 4).
Corrélation entre la densité osseuse moyenne et le temps d’intervention (Tableaux 5 et 6) :
On note une corrélation statistiquement significative uniquement dans le cas de l'utilisation de la technique Intralift par le profane. Cela signifie que la seule situation où l'augmentation de la densité osseuse la durée d'intervention est plus importante, c'est lorsqu'un profane utilise l'Intralift.
Si une autre technique est utilisée, ou si l'intervention est réalisée par un expert, la densité osseuse n’influencerait pas la durée d'intervention.
Préservation de la membrane en fonction de l’opérateur et de la technique :
On note qu’une membrane a été perforée avec l’utilisation de la technique de Summers par l’opérateur expert, alors que toutes les membranes ont été préservées avec la technique d’ultra-sons. (Tableau 7)
Une membrane sinusienne a été perforée avec la technique de Summers, alors que toutes les membranes ont été préservées avec la technique Intralift (Tableau 8).
La technique Intralift serait donc plus sécurisante pour la préservation de la membrane sinusienne.
En effet, on ne retrouve aucune effraction de la membrane de Schneider alors que l’on dénote deux membranes perforées avec l’utilisation de la technique de Summers (Tableau 9).
Par ailleurs, pour considérer un cas comme positif en fonction de l’opérateur, nous avons défini trois paramètres précis :
Effraction de la corticale sinusienne (le plancher sinusien);
Membrane sinusienne préservée (non perforée);
Comblement visible (aspect homogène en dôme, sans dispersion des matériaux).
Le point important recherché étant l’absence de perforation de la membrane sinusienne, les autres cas d’échecs sont plutôt liés à des erreurs opératoires beaucoup plus maitrisables avec l’apprentissage, ou par modification du protocole.
On remarque néanmoins que le profane semble d’emblée plus à l’aise avec le protocole Intralift qu’avec le protocole de Summers (Tableau 10).
4-Discussion
Technique Intralift :
On voit sur le graphique 9 (Tableau 9) qu’il n’y a aucune effraction de la membrane sinusienne sur un total de douze interventions. Cependant une prise en main du praticien serait nécessaire, car les vibrations provoquées par les ultrasons pourraient diminuer la sensation tactile de l’opérateur.
Technique de Summers :
D’après les résultats de cette expérimentation (Tableaux 10-11-12), cette technique est beaucoup plus opérateur dépendant.
Les résultats de l’opérateur expérimenté sont beaucoup plus probants que ceux de l’opérateur novice.
Cependant au cours des interventions, chaque opérateur a perforé une membrane.
Préservation de la membrane
Il semble que la technique Intralift offre plus de sécurité sur la préservation de la membrane de Schneider. Aucun opérateur (expérimenté ou novice) n’a entrainé au cours de l’intervention une perforation de la membrane avec l’utilisation des ultra-sons (Tableau 9).
Cependant l’échantillon d’étude étant trop petit, aucune affirmation ne peut être émise avec certitude quant à la limitation des lésions de la membrane sinusienne. Mais les résultats de notre expérimentation tendent à aller dans ce sens.
On a pu tout de même démontrer que le risque de perforation est minimisé avec l’utilisation d’ultra-sons qu’avec la technique de Summers. Une étude sur un plus grand nombre de cas serait nécessaire pour évaluer avec certitude le degré de sécurité qu’offre la piézochirurgie sur la membrane sinusienne.
Comblement :
La non homogénéité du comblement ou la dispersion du matériau n’est observée que dans la technique Intralift.
Lors de cette étape, nous avons utilisé des seringues à os de diamètre 3mm, dont l’efficacité reste à démontrer tant dans son remplissage que dans le comblement de l’alvéole créée.
Il serait donc plus approprié d’utiliser une spatule à insérer et de combler l’alvéole par apports successifs de matériau.
Après chaque apport il ne faudrait pas utiliser d’instrument conique à l’aveugle, type ostéotome pour faire pénétrer le biomatériau, au risque d’effracter la membrane.
En alternative, l’insert TKW5, pourrait être utilisé lors de la phase de compactage du matériau de comblement, technique du « Plug & Spray », préconisé par Satelec Actéon[2, 3], afin d’homogénéiser le comblement.
Avant de réaliser cette étape, il faut bien vérifier les réglages du générateur [2, 3], notamment diminuer le débit d’irrigation pour éviter un excès de pression hydraulique sur la membrane sinusienne. On utiliserait cette technique pendant une durée de 2 à 3 secondes si le matériau de comblement reste bloqué dans le canal et/ou pour le disperser uniformément sous la membrane sinusienne.
Les causes d’échecs :
D’après le tableau 11, on remarque que les causes d’échecs dans la technique Intralift sont :
Le défaut de comblement (paramètre maitrisable par modification de la technique d’insertion du biomatériau) ;
L’absence d’effraction de la corticale sous sinusienne.
D’après le tableau 12, on Remarque que les causes d’échecs dans la technique de Summers sont :
L’absence d’effraction de la corticale sous-sinusienne ;
L’effraction de la membrane sinusienne (présente chez les deux opérateurs).
5-Limites de l’étude :
Malgré les résultats encourageants par rapport à l’hypothèse du départ, cette étude reste modeste à cause de certaines limites :
Un faible effectif de pièces anatomiques (douze) ;
Un faible nombre d’opérateurs (deux).
Aucune affirmation ne peut être retenue avec certitude, seules des hypothèses peuvent être émises pour évoquer la préférence du système Intralift à celui de l’ostéotomie de Summers dans la mesure où les résultats sont assez proches à savoir :
Meilleure préservation de la membrane sinusienne avec l’utilisation de la technique Intralift ;
Temps de réalisation similaire avec la technique de Summers ;
Réalisable avec le même taux de réussite par un opérateur expérimenté que par un opérateur novice.
6-Conclusion :
La technique Intralift offre une alternative intéressante à la technique de Summers lors d’élévation de la membrane sinusienne par abord crestal.
Cette technique beaucoup moins traumatisante vis à vis du patient (absence d’impactage de l’ostéotome avec le maillet), elle a certainement un autre avantage sur la technique d’ostéotomie de Summers qui peut, en effet, être un facteur aggravant des benign paroxysmal positional vertigo (BPPV) [7, 8]. L’utilisation des ultra-sons, avec un signal modulé, permet la préservation des tissus mous comme la membrane sinusienne.
Il serait intéressant non seulement de développer une étude sur un plus grand nombre de cas pour pouvoir confirmer les résultats de cette étude, mais aussi d’évaluer la surface et la hauteur de décollement de la membrane par rapport à la pression hydraulique exercée. En effet, au vu des scanners post-opératoires on constate que l’élévation hydrodynamique due à l’insert TKW5 sous irrigation entraine un décollement membranaire sur une surface importante autour du canal d’accès, entrainant donc un volume de comblement plus important que celui retrouvé dans la technique de Summers qui semble plus réduite au niveau de sa base.

Translation - French Comparison of the Intralift technique and the Summers technique on human corpses:
Abstract:
Many surgical techniques have been developed for several decades in order to push up the sinus membrane in cases of low residual bone height. We wanted to compare two sinus lift elevation techniques by crestal approach: the Intralift technique using piezosurgery and the Summers technique using osteotome.
Through experiments on fresh anatomical parts, we searched:
. Whether the sinus mucous membrane of Schneider is better preserved using one of the two techniques over the other;
. Whether achievement time using one of the two techniques is quicker over the other;
. If the Intralift technique is, from the start, manageable for an inexperienced operator.
After analyzing the results achieved, we formulated hypotheses as to the safer nature of the Intralift technique on the mucous membrane over the Summers technique. Achievement time would be similar using either of the two techniques and ultrasound technique seems to be easier to perform for an inexperienced operator using the Intralift technique rather than the Summers one.
1- Introduction :
The development of implantology in recent years has led to consider more and more sophisticated surgical techniques.
Many surgical techniques have been developed over the last decades in order to push up the sinus membrane in cases of low residual bone height.
Since 2001, Professor Vercelotti [12, 13] described a sinus lift elevation technique by crestal approach using ultrasounds which could be an atraumatic sinus lift by crestal approach [4].
We’ve carried out a study, experimenting on fresh anatomical parts to study whether this approach ensures that progress is made in minimizing the risk of perforating the sinus membrane compared to the reference technique of Summers.
2- Technical equipment and method :
Our research consisted in simulating different protocols on human anatomical parts at the anatomy laboratory of the University of Bordeaux 2.
The study was made on 12 fresh anatomical parts. The average age of the subjects was 70. On each anatomical part, we performed an Intralift technique on one sinus and the Summers technique on the other one in order to compare the results.
Technical equipment
We used various equipment to carry out this study:
A camera with ring flash and macro lens (PENTAX K7, SIGMA MACRO 105mm lens F2.8 EX DG, SIGMA EM-140 DG ring flash).
For the Intralift technique: round burrs for contra angle; drill guides with a diameter of 2 mm; two Implantcenter 2® devices (Satelec Actéon); two classical contra angles and two piezo contra angles (Satelec Actéon); two sets of inserts TK1 to TK5 (Satelec Actéon); one Siemens Sensation 16 scanner (Herlanger Germany); one Dental® Software for the acquisition and the construction of cross sections;
For the Summers technique : two kits of osteotomes, two drilling kits, two mallets (Henry Schein ASA dental) ; two surgical kits ; two bone syringes ( with a diameter of 2.5 mm) ; collagen sponges ;
For the filling under the sinuses: fake Bio oss® (Geistlich pharma France);
Surgical suture (MERSILK 1-0);
Two electronic calipers.
Method
Chronologically, we went through the following steps:
Determination of the inclusion and exclusion criteria of the anatomical parts:
Inclusion criteria:
Bilateral gap of the posterior sector;
Maxillary with a bone resorption of the alveolar ridges;
Maxillary with a residual height of the ridge inferior to 9 mm
Exclusion criteria:
Pathological sinuses;
Maxillary with a residual height of ridge superior to 9 mm;
Sinus with septa in the area of intervention.
Pre-surgical scans;
The scanning of 12 anatomical parts was carried out at the radiology department of the Haut-Lévêque University Hospital in Pessac.
The analysis of the pre-surgical scans was conducted thoroughly in order to describe the features of each sinus. The height and width of the ridge under the sinus have been measured and recorded for each anatomical part. The selected parts showed bone heights ranging from 1 to 9 mm. This height represents the distance between the sinus floor and the top of the toothless crestal bone. We have also assessed bone density for each sector by Hounsfield measurement using the Dental®Registered Siemens software. Then we have correlated the values obtained with the Misch bone density classification in order to find out the type of bone for each of the anatomical parts [5, 8]. The areas for surgery were thus defined.
After the analysis of the pre-surgical scans, surgery was performed in two
steps:
Elevation of a mucoperiosteal flap:
The full flap made it possible to discover the entire maxillary ridge and a better visibility of the areas for surgery (palate dissection 1 to 3.JPG).
Marking of anatomical landmarks on the ridge:
Because we had no tooth left, we made markings on all the crestal bones to use them as landmarks during the surgery.
On the pre-surgical scans, we measured the length from the incisive foramen to the definite intervention area. These lengths were reported on a suture thread (MERSILK 1-0) with an electronic caliper and then on the top of the crestal bone of the anatomical part.
Elevation of a mucoperiosteal flap:
Simulation of the surgical procedure: The surgical procedure was carried out by two independent operators. The protocols carried out complied with the recommendations of the two techniques [1, 9-11].
Post-surgical scans: The post-surgical scans were carried out with the same scanner (Sensation 16® Scanner SIEMENS, Herlanger Germany) during the pre-surgical and post-surgical stages and by the same operator. The acquisition and the construction of the different cross sections were made with the same Dental®Registered Siemens software. (see DENTAL-ACQUIS.JPG and DENTAL-RECON.JPG)
Recording of the post-surgical data: The data were recorded in charts which gathered all the parameters which were to be evaluated (Charts number 13 and 14).
3 – Results:
The essential parameters which were to be compared were: The achievement time; The integrity (or not) of the sinus membrane.
These parameters were to be compared with the same operator and between the two operators.
The integrity of the membrane and of the lifting of the floor could be evaluated by comparing the pre-surgical scanning and the post-surgical one. (see cross sections coronales 9 post op D intralift, 9 preop D intralift and 12 post op D summers, 12 preop D summers).
Statistical analysis:
Hypotheses: The Intralift technique is easier to perform for a novice operator than the crestal osteotomy (Summers) technique. The achievement time is shorter with the Intralift technique than with the Summers one. The Intralift technique preserves the sinus membrane better (it is safer) than the Summers technique.
Method: All the tests were carried out with the Matlab®software (R2010b) at the CNRS-UMR 5287 Institute of Bordeaux 2 University.
Comparison of the achievement time of the two operators depending on the method used : First a Bartlett test was carried out to check the homoskedasticity of the data. These Bartlett tests enable us to validate the different analysis (see charts 1 and 2). An analysis of the multifactorial variance was carried out, with as the first factor the operator (BE vs. JL), and as the second factor the technique used (Intralift vs. Summers) (see chart 3). We can see an operator effect with a shorter achievement time when the operator is more experienced than with the novice operator. (p=0.0349). A learning curve is thus necessary to use the two techniques. Therefore there would be a correlation between the operator’s dexterity and the achievement time during the surgery in both techniques. But one can’t say one technique is quicker than the other, even when we differentiate the level of expertise of the operator (see chart 4).
Correlation between the average bone density and the achievement time (see charts 5 and 6) We can see a statistically significant correlation only in the case when the Intralift technique is used by the novice operator. It means that in the case when the bone density is higher, the achievement time is longer if the operator is a novice. If another technique is performed or if the surgical procedure is carried out by an expert, the bone density doesn’t influence the achievement time.
Preservation of the membrane depending on the operator and the technique used: We can see only one membrane was perforated while using the Summers technique with a skilled operator whereas all the membranes were preserved with the ultrasounds technique (see chart 7). A sinus membrane was perforated by the novice operator using the Summers technique whereas all the membranes were preserved with the Intralift technique (see chart 8). Therefore, the Intralift technique seems safer to preserve the integrity of sinus membranes because there is no perforated Schneider membrane when this technique is used whereas we had two perforated membranes with the Summers technique (see chart 9). Furthermore to consider a case as being positive depending on the operator involved, we defined three specific parameters: The break-in of the sinus floor; Sinus membrane preserved (not perforated); Visible filling (homogeneous dome-shaped appearance, without dispersal of the materials). The main purpose was to avoid perforation of the sinus membrane. The failed attempts were rather the consequence of operating errors which are likely to be better mastered after some training or by making changes in the protocol. Nevertheless we can notice that the novice operator seems to feel immediately more comfortable with the Intralift technique than with the Summers technique (see chart 10).
4-Discussions:
Intralift technique
As we can see on chart 9 (see chart 9), out of 12 surgical interventions, there was no perforation of the sinus membrane. Yet, ownership on behalf of the practitioner would be necessary because the vibrations caused by the ultrasounds may reduce the operator’s tactile sensation.
Summers technique
According to the results of this experimentation (see charts 10-11-12), this technique is more operator dependent.
The results of the experienced operator are far more probing than those of the novice operator.
Yet, during these interventions, each operator has perforated one membrane.
Preserving the membrane
It seems clear that the Intralift technique is safer as concerns the preservation of the membrane. During the surgical interventions, no operator (experienced or novice) caused a perforation of the membrane while using ultrasounds (see chart 9).
Yet, because the study sample is too small, we can’t assert for sure that the lesions on the sinus membrane are limited. But the results of our experimentation tend to prove they are.
Still, we were able to demonstrate that, compared to the Summers technique , the risk of perforation is minimized with the use of ultrasounds. A study on a larger sample would be necessary to evaluate more accurately the level of safety piezosurgery on the sinus membrane provides.
Filling
A non-homogeneity of the filling or a dispersion of the material was observed with the Intralift technique only.
In this step, we used bone syringes with a diameter of 3 mm, the effectiveness of which is still to be proven as regards both their filling as well as the filling of the alveolus created. It would therefore be more appropriate to use filling spatulas and to fill the alveolus through successive inputs of material.
After each input, no conical instrument like osteotomes should be used blindly to brush the biomaterial in order to avoid a break- in of the membrane.
As an alternative, the TKW5 insert could be used in the filling material compacting stage, using the “Plug and Spray” technique as recommended by Satelec Actéon [2, 3] in order to homogenize the filling.
Before carrying out this step, the settings of the generator must be checked thoroughly [2, 3] and in particular, the irrigation flow must be reduced to avoid an excessive hydraulic pressure on the sinus membrane. This technique should be used for two or three seconds if the filling material is stuck in the canal and/or in order to deliver it evenly under the sinus membrane.
Causes of failure
According to chart 11, we can see the causes of failure in the Intralift technique are:
The filling failure (a parameter which can be controlled through a change in the technique used to insert the biomaterial);
The absence of break-in of the sinus floor.
According to chart 12, the causes of failure in the Summers technique are: The absence of break-in of the sinus floor; The break-in of the sinus membrane (which happened with both operators).
Limitations of the study
In spite of promising results as compared to our original assumption, this study remains modest owing to some limitations:
A small number of anatomical parts (twelve); A small number of operators (two). No assertion can be retained with certainty, we can only make assumptions to evoke a preference of the Intralift system over Summers’s osteotomy in so far as the results obtained are rather close, namely: A better preservation of the sinus membrane when using the Intralift technique; An achievement time which is similar with both techniques; Achievable by an experienced or a novice operator with the same success rate.
5-Conclusion
The Intralift technique offers an interesting alternative to the Summers technique when elevating the sinus membrane by crestal approach. This technique which is far less traumatic as regards the patient (no impacting of the osteotome with a mallet) probably has another advantage over the Summers technique which can indeed be an aggravating factor of the benign paroxysmal positional vertigo (BPPV) [6, 7]. Using ultrasounds with a modulated signal makes it possible to preserve soft tissues like the sinus membrane.
It would be interesting, not only to develop this study on a larger number of cases in order to be able to confirm the results already obtained, but also to evaluate the surface and how high the membrane is removed according to the hydraulic pressure applied. Indeed, based upon the post-surgical scans, we can see the hydrodynamic elevation resulting from the TKW5 insert under irrigation causes the membrane to detach over a large area around the access canal which leads to a filling volume more important than the volume observed when using the Summers technique which seems to be smaller at its basis.

1. Quels sont les objectifs concrets du projet et son impact escompté sur les participants et les autres publics intéressés (élèves, enseignants, établissements, communauté locale, communauté éducative au sens large) ?
Notre projet Comenius 1 est centré sur l’Eau qui entoure nos élèves. En effet, l'approche d’un travail sur l’environnement et son exploitation spécifique par l’Homme leur permet d’aborder les réalités locales et les disponibilités en eau mais aussi la symbolique culturelle de l'eau.
Le projet sur l’eau va conduire les élèves à :
- Approfondir les connaissances sur l’eau, l’environnement et le développement durable à l’échelle locale et européenne.
- Améliorer les méthodes d’apprentissage.
- échanger et perfectionner les langues vivantes, approcher des langues moins utilisées en Europe.
- développer la conscience d’une citoyenneté européenne.
- utiliser les TIC.
Notre projet aura un impact multiple :
Au travers des recherches puis des échanges, les élèves seront conduits à réfléchir sur leur propre environnement, à rédiger et à travailler en groupe et en pluridisciplinarité au sein de leur établissement et avec les équipes partenaires. Ils utiliseront régulièrement les TIC pour élaborer une production et pour la diffuser auprès des établissements. Les élèves qui connaissent des difficultés dans un cadre traditionnel pourront grâce à ces activités développer leurs compétences transversales et différentes. Leur implication sera plus importante et ils seront acteurs de leur apprentissage.
Les travaux seront communs aux partenaires et nécessiteront une implication individuelle et collective, la solidarité étant indispensable pour aboutir à une production autonome. Des représentants des élèves participeront aux réunions de projet afin d’être les porte paroles des camarades et ensemble ils prépareront un compte rendu présenté à tous les participants.
De leur côté, les enseignants élaboreront un cahier des charges et se réuniront régulièrement pour effectuer un bilan et réajuster le déroulement du projet. De plus, les compétences transversales permettront la création de produits finis qui seront communiqués à tous. Enfin, les réunions de projet seront l’occasion de confronter et d’enrichir les différentes pratiques pédagogiques de chacun.
De surcroît, les établissements s’ouvriront vers l’extérieur par des sorties sur le terrain ainsi que par la rencontre d’organismes publics et privés. Ils insèreront le projet dans le tissu socio économique local retrouvant les objectifs du projet d’établissement. Le projet Comenius, par ce biais, apportera une image dynamique et attractive au collège car il prend une dimension européenne.
2. Quel type d'activités comptez-vous mener dans le cadre du projet ?
Pour construire le projet, nos élèves européens s’impliqueront dans diverses activités :
- élaboration de fiche d’identité de présentation des élèves en anglais et en français.
- utilisation des TIC : courriers électroniques, vidéoconférence, traitement de texte diaporama, Cd-rom.
- Travail sur des séquences littéraires ( récits, romans, poèmes)
- Recherches au CDI.
- Interview des partenaires publics et privés.

3. Quel(s) type(s) de produit final souhaitez-vous élaborer ?
Chaque année, les élèves feront paraître un journal biannuel en février et mai. Un Cd-rom ou un DVD compilera les sorties sur le terrain et les vidéoconférences. A la fin des trois années, forts des connaissances et des réflexions, les élèves élaboreront conjointement une CHARTE EUROPEENNE JEUNE DE L’ECO CITOYENNETE.
4. Comment comptez-vous évaluer l'évolution du projet et son impact sur les élèves et les enseignants participants, sur les établissements participants et, le cas échéant, sur la communauté locale ?
Quant à l'évaluation de l'avancée du projet, elle sera trimestrielle et associera enseignants et élèves dans l’élaboration d’outils d’évaluation. Plusieurs outils comme des boîtes à idées, des questionnaires pour les élèves et les adultes de l’établissement seront créés. Une page du journal sera réservée au courrier des lecteurs ainsi qu’un forum sur Internet. Dans chaque établissement les enseignants se réuniront régulièrement et pourront travailler avec des représentants d’élèves. De même, les réunions de projet biannuelles seront l’occasion de créer des outils communs et d’évaluer l’impact du projet sur tous les intervenants.
Des questionnaires seront élaborés à partir de leurs interrogations et des questions de leurs partenaires. Un outil commun sera créé afin de mieux comparer les résultats. Les élèves découvriront des modes de vie et de pensée différents et ainsi s’enrichiront. Ils modifieront leur représentation et développeront la tolérance en comprenant que les différences peuvent rapprocher.
5. Comment comptez-vous diffuser les résultats, l'expérience et les produits finaux parmi les établissements participants, les autres établissements et la communauté locale ?
Les productions seront rendues publiques puisque les réalisations des élèves seront mises en commun et disponibles sur le site Internet de tous les établissements. Le journal sera présent dans tous les centres de documentation afin de toucher un large public scolaire. Il sera aussi communiqué à nos partenaires, aux mairies et aux départements/régions. Il en sera de même pour le Cd-rom de bilan de fin d’année.
Une sélection d’articles participera au « Printemps de l’Europe », manifestation de l’Union Européenne qui a lieu chaque année. Au final, les élèves pourront présenter la Charte par l’intermédiaire de députés européens au Parlement de Strasbourg.
6. Quel type de mesures spécifiques envisagez-vous de prendre, le cas échéant, dans le cadre du projet ?
Avec ce projet sur l’Eau, nous voulons aborder les différences entre les environnements et ces analyses conduiront les élèves à constater des mises en valeurs humaines, des problèmes de l’Eau et des solutions similaires. Ils confronteront leurs expériences, découvriront par ce biais de nouvelles cultures, des modes de pensée différents et développeront leur ouverture d’esprit.
7. Veuillez décrire le rôle de tous les établissements participants et expliquer comment la coopération réelle entre les établissements participants sera assurée.
Chaque établissement contribuera à la rédaction du journal, à l’élaboration des Cd-rom via le réseau Internet et les vidéoconférences. De plus, chacun fera le choix d’une spécificité environnementale. Par exemple, le partenaire italien travaillera sur la baie et son exploitation touristique et industrielle, le partenaire grec sur l’eau et l’insularité, etc….Chacun étudiera aussi un paysage commun à au moins un autre partenaire. C'est ainsi que la première réunion de projet permettra de mettre en place la coopération entre les établissements et de définir les orientations sur lesquelles chacun aura déjà entamé une réflexion. Nos partenaires Grec et Bulgare de Bourgas qui ont déjà participé à Comenius 1 apporteront leur expérience pour l'évolution du projet.
8. Le projet fera-t-il appel aux technologies de l'information et de la communication ? Si oui, quel(s) type(s) de technologies ? Comment et dans quelle mesure seront-elles utilisées ?
D'un point de vue technologique, cette action est un véritable pas dans la modernité car les élèves utiliseront les TIC. Ils manieront le traitement de texte, les tableurs et la messagerie Internet pour favoriser la connaissance entre eux, pour échanger sur l’avancée du projet, pour faire des recherches de données chiffrées, de cartographie et de textes. L’exploitation donnera lieu à la création d’outils comme des graphiques ou des tableaux qui seront mis en commun. L’élaboration du Cd-rom favorisera la maîtrise d’un logiciel de présentation qui permet la création d’un diaporama. Pour que les élèves puissent se voir et échanger, les web cams seront nécessaires pour les vidéoconférences. Tout cela rendra possible la création de sites Internet qui n’existent pas chez tous les partenaires.
9. Comment la participation active des élèves sera-t-elle assurée tout au long du projet (planification, mise en œuvre et évaluation) ?
Les élèves deviendront rédacteurs en chef, journalistes, artistes. Chaque partenaire fournira des photos, des articles bilingues. Des comités de rédaction choisiront les thèmes et chaque établissement sera à tour de rôle chargé de la mise en page. Les articles reprendront tous les travaux semestriels. Les rédacteurs en chef finaliseront la production par une mise en commun lors d’une vidéoconférence. La Une sera décidée en direct. Enfin, une réunion de projet réunira des représentants des élèves et des enseignants. Nous y ferons un bilan des activités, l’état d’avancement du projet et les perspectives pour l’année suivante. Les élèves travailleront en équipe et feront un résumé en vidéoconférence vers les différents établissements.
10. Comment le projet sera-t-il intégré dans le programme des élèves participant au projet dans les établissements participants ?
Le projet sur l’Eau est transdisciplinaire mais chaque matière aura sa spécificité liée à son programme : - Lettres : étude de différents types de textes (informatif, argumentatif, descriptif, narratif), de différents genres (récits, poésie) et du symbolisme de l'Eau. - Histoire Géographie : lecture de paysages, enjeux environnementaux, climat, étude socioéconomique à différentes échelles spatiales, unité et diversité de l’Europe. Etude de l’histoire européenne du XVIIème siècle à nos jours. - Sciences : évolution des paysages, l’occupation des milieux de vie, cycle de l’eau, météorologie, l’environnement et le développement durable. - Technologie : utilisation des TIC, création d’un objet (maquettes virtuelles, réelles, Produit numérique). - Langues : apprentissage d’une langue étrangère, connaissance d’une nouvelle culture. - Arts Plastiques : travail sur l’image, le mouvement, les rythmes et mise en volume.
Translation - French Our Comenius project is centered on the Water which surrounds our pupils. The approach of a work on environment and its specific use by Man will first enable them to deal with the local realities and the amounts of available water as well as with the cultural symbolism of Water.
In a second step, they’ll be led to understand the use and the challenge involved by this resource through the studying of the differences and similarities between the partners in the historical, economic, environmental and cultural fields. The point is to integrate this reflection at several levels in a European dimension.
Water is the basis of long-lasting development. Being aware of this challenge at a European level will therefore enable the pupils to get involved in an Ecocitizenship project. While exchanging their experiences they are going to elaborate solutions and show that each of us can take a part in the process of European citizenship.
Working together with representatives from Western and Eastern Europe will enrich the project since the cultures and ways of thinking are varied.
The pupils will communicate and use their foreign language skills. They’ll use French, English, Spanish. They’ll also become aware of less widely spread languages when reading bilingual articles.
Through their research and the exchanges, the pupils will be led to think about their own environment, to write and work in a group in a multi-subject approach within their school and with their partners abroad. They’ll often have to use ICT to elaborate a product and broadcast it to the other schools involved. The pupils who usually find it difficult to cope with school in a traditional frame should be able, thanks to these activities, to develop some of their different and transversal skills. Their involvement will be more consistent and they’ll become the actors of their training.The task will be common but it will require both personal and collective involvement. Solidarity will be essential to lead to an autonomous output. Pupils’representatives will take part in the project meetings to be the spokespersons of their schoolfriends and together they will make a report of these meetings to all the participants.
As for the teachers, they will elaborate specifications and will meet regularly to take stock of the situation and reset the course of the project.. Moreover, transversal skills will lead to the making of “products” which will be communicated to all. Last, project meetings will give us the opportunity to compare and enrich our teaching practises.
In addition, the schools will open on the outer world through field trips and meetings with public and private organizations. They will insert the project in the local socio-economical frame catching up with the objectives of our school project. From this angle of approach, the Comenius project will bring out a dynamic and attractive image of our schools because the pupils will discover different ways of living and thinking. Thanks to it, they will become all the richer. They will change their internal representation and will develop tolerance when understanding that differences can draw people closer.
As far as trainings are concerned, the pupils will get to know each other through the exchange of E-mails. One pupil will have two contacts. The number of pupils will allow to create a link between all the project partners. A form will be elaborated by the native and foreign language departments.There will be photos of the pupil, of his (her) class, of the classrooms and of the local environment related to the topic of the project.
Each class will have a partner class, with the incentive to share and exchange work when different landscapes will appear to have something in common. Then they will put these synthesises on line on the websites they will build little by little and will feed the newspaper which will be one of the objectives of their outputs twice a year.
The pupils will take part in trips to discover their environment as far as Water is concerned. They will learn how to read landscapes of estuaries, rivers, bays etc...
As for the “products”, they will draw sketches, collect plants, observe wildlife and the imprint of Man on the field. Their sketchess will be studied in class, dated and compared to former maps so as to make them understand the changings imposed by Man. Then they will be completed thanks to the documents supplied by the regional Water Boards which will identify the origins, the quantity, the quality and the way Water is being used. Models will be made of a dune, of a river and of a harbour (touristic or industrial). The pupils are going to build a weather station. The readings will be regular and reported to all. Twice a year, the amount of precipitation will be recorded. Accounts on these landscapes and on these resources will also be written in different languages and broadcast. They will work on the interviews with public or private partners or with the people they will meet on the field too. Questionnaires will be elaborated from their questioning and their partners’ questioning.
A common tool will be created to make it easier to compare their results.
In the French class, the pupils will work on these informative and argumentative “products”. In more literary lessons they will also approach the imaginary side of WATER in the European culture and its symbolics ; they will also study its analysis as a literary and artistic particular pattern.
Moreover, they will collect different material (driftwood, pebbles, polished glass etc..), they will take photos showing the imprints of Water on the landscape. The colours of the Water will be created through a work on palettes of paint and the pupils will draw up a catalogue for each region and each country that we will be able to compare.
The study of the different palaces (Caserte, Versailles…) will show them that water can be put “on stage”. They will find this out from the sketches, the plans and the texts telling of the kings’ walks. There again, by comparing their productions, they will eventually discover the common cultural influences. This will lead to the handling of Water by Man : water games, water wheels, dams and mills…
Thus, the pupils will be editors, journalists, artists. Each partner will provide photos, articles in two languages. Editing committees will select the themes and each school will, in turn, be in charge of the lay-out. The articles will take up all the works of the past six months. The editors will finish the production by deciding upon a contents during a teleconference. The decision concerning the front page will be reached “live”.
Finally, there will be a reunion of both the pupils’ and the teachers’ representatives. The result of the activities will be shown, as well as the current state of the project and the prospects for the following year. The pupils will be working in teams and will make a summary through a teleconference to the different schools.
From a technological point of view, this action is a real step into modernity, for the pupils will be using ITC tools. They will be handling the techniques of word processing, of a spreadsheet and Internet messaging to develop the knowledge of each other, to have exchanges on the course of the project, to do research on data, maps and texts. All this will give birth to tools like graphs or tables, which will be shared by the partners.
The making of the CD-rom will enable the mastering of a presentation software, hence allowing the creation of a diaporama.
So that the pupils may see each other and exchange ideas, webcams are necessary for the teleconferences. This will make the creation of websites possible. For the moment, only our Italian partner has such a site.
Thus, each school will participate in the writing of the newspaper, in the making of the CD-roms via the web. Moreover, each one will choose an environmental specificity and also study a landscape common to at least one other partner. The first project reunion will enable to set up a cooperation between the schools and define the orientations each one will have thought over beforehand. Our Bulgarian partner, who has already taken part in Comenius 1, will bring his own experience to the project.
Result : Twice a year, in February and May, the pupils will publish a newspaper. A CD-rom or a DVD will compile the field trips and the teleconferences. By the end of three years, what with their knowledge and reflections, the pupils will elaborate in common a Young European Eco-citizenship Chart.
As for the marking of the course of the project, it will be regular and it will associate both teachers and pupils in the elaboration of the marking tools. Several tools, such as suggestion boxes, questionnaires for the pupils and the adults of the school, will be created. A page of the newspaper will be devoted to the reader’s corner, as well as a forum on the Internet. In each school, the teachers will regularly meet and work with the pupils’ representatives. Just as the twice-yearly project reunion will offer the opportunity to create common tools and evaluate the impact of the project over the participants.
The production will be made public since the pupils’ achievements will be gathered and available on the website of all the schools. The newspaper will be present in all the School Resource Centres in order to touch a large school audience. It will also be communicated to our partners, to the town councils and the local authorities. The same will be done with the end-of-the-year CD-rom.
A selection of articles will participate to the “European Spring”, a yearly EU event. Finally, the pupils will present the Chart to the Strasburg Parliament via the European MPs.
Thus, with this project on water, we want to point out the differences between the environments and these analyses will lead us to note the improvements made by Man on Water and the solutions to the problems encountered.
The project on Water is a multi-subject one but each subject will have its specificity linked to its syllabus :
Literature : study of different types of texts (informative, argumentative, narrative, descriptive), of different genres (tales, poems), and study of the water symbolism.
History & Geography : landscape reading, environmental challenges, climate, socio-economic study on different scales, unity and diversity of Europe. Study of the History of Europe from the17th century to today.
Sciences : Evolution of the landscapes, water cycle, weather forecasting, environment, long-lasting development.
IT : use of the computer-assisted communication tools, making of an object (virtual or real models).
Languages : learning a foreign language, knowing a new culture.
Arts : work on the picture, the movement, the rhythm, the volumes.

Source text - English Excerpt : What follows is at this point still but a sketch, but perhaps that may be altogether appropriate for discussing one of the most famous tales from The Sketch-Book of Geoffrey Crayon, Gent. (1819). In my view, Irving humorous and memorable “Legend of Sleepy Hollow” functions as a playful critique of the emergent nationalism in the United States of that era. The tales and sketches of the “early” Irving reveal the conflicts and uncertainties surrounding the transformation of social spaces in the United States during the first half of the nineteenth century. With respect to both the form and the content of his early writings, Irving established a cultural space that was distinctively apart from the main currents of national development and, at the same time, intimately related to the emergent national space. By maintaining the ekphrastic style of the master artist in his sketches, Irving paints a scene that is both picturesque and exotic, but by introducing the more temporal narrative element, the tale discloses a tension between the onrushing flow of history and the static, well nigh utopian spirit of the place itself. Such early writings appear before Irving becomes the respectable author of larger works on a national and international scale, such as his fanciful biographies of Christopher Columbus or George Washington. This younger Irving is still bows down before the spirit of place, the genius loci of a zone that lies within, but is as yet inassimilable to, the U.S. national space. In the process of narrating and describing the legend of Sleepy Hollow, Irving helps to disclose and map those shifting spaces that were redefining lived experience in that era.

En conservant le style ekphrastique de l'artiste professionnel dans ses esquisses, Irving dépeint une scène qui est pittoresque et exotique, mais en introduisant l'élément de récit plus temporel, le conte dévoile une tension entre le flux torrentueux de l'histoire et l'expression statique et quasiment utopique du lieu lui-même. Des premiers écrits de ce genre apparaissent avant qu'Irving ne devienne l'auteur respectable d'oeuvres plus importantes à l'échelle nationale et internationale, comme ses biographies imaginaires de Christophe Colomb ou de George Washington. Cet Irving plus jeune s'incline toujours devant l'esprit du lieu, le génie des lieux d'une zone qui reste incluse dans l'espace national des Etats-Unis, mais qui y est cependant inassimilable. Dans le processus d'écriture de la Légende de Sleepy Hollow, Irving aide à dévoiler et à dresser la carte de ces espaces changeants qui redéfinissaient l'expérience vécue à cette époque. ....

I am French. I graduated from high school in California, I have a Master's Degree in English studies (Literature and Civilization), a Master's Degree in Educational Sciences and a Teaching Degree (CAPES). I’ve been a high-school English teacher in France for 22 years (working for the French Ministry of Education) and I’ve also taught technical English in an IT school. Because I love translating, I have often volunteered for translations along the years. I retired as a teacher two years ago and I launched my own small company as a freelance translator. Currently, I’m looking for new assignments.
