Source text - English 3. On the 30th December 2008, an autopsy examination was performed by Forensic Pathologist, Dr Seuss, who concluded that the cause of death was due to sepsis, however, its exact cause was uncertain. At the time the examination was conducted, the pathologist was
aware of family concerns that morphine was given, even though Ms Doe was allergic to it. The pathologist comments in her report, that tests undertaken on anti mortem blood revealed no indication of anaphylaxis, hence there is no evidence of an allergic reaction.
4. The family wrote to the Coroners Court, questioning how Ms Doe became infected; how morphine was administered when she was allergic to the drug and whether investigations and treatment undertaken, was timely. The case was referred to the Clinical Liaison Service at the Coronial Services Centre for review. The Service discussed the circumstances of the death at
a multidisciplinary Case Review Meeting chaired by a coroner on the 26th June 2009, wherein
they had regard to the medical records of the Nowhere Hospital, family letters of concern, the pathology report and the police circumstances of death. Discussions indicated that it was difficult to isolate what caused her infection, she having been admitted to hospital with a community acquired chest infection on a background of chronic obstructive pulmonary disease and congestive cardiac failure. These and her other co-morbidities contributed to her reduced cardiac and respiratory reserve. Whilst the issue regarding the giving of morphine was a very valid concern, there was no evidence that Ms Doe reacted adversely to the medication. As to the timing of her presentation to the hospital, there were no indications to admit her directly to the Intensive Care Unit from the Emergency Department.

ils avaient considéré les dossiers médicaux de l'Hôpital Nowhere, les lettres faisant état de la préoccupation familiale, le rapport de pathologie et les circonstances du décès établit par la police. Les débats ont indiqué qu'il était difficile d'isoler ce qui a causé l'infection étant donné qu’ elle avait été admise à l'hôpital avec une infection à la poitrine acquise dans la communauté aux antécédents de maladie pulmonaire obstructive chronique et d'insuffisance cardiaque congestive. Ces infections et ses autres morbidités ont contribué à la réduction de sa réserve cardiaque et respiratoire. Même si la question concernant l'administration de la morphine a été une préoccupation très pertinente, il n'y avait aucune preuve que Mme Doe ait réagi négativement à ce médicament. Quant au moment de son arrivée à l'hôpital, il n'y avait aucun signe indiquant son admission directement à l'unité des soins intensifs du département d'urgence.

Interpreters will refrain from accepting assignments beyond their professional skills, language fluency, or level of training.
The interpreter strives to render the message accurately, conveying the content and spirit of the original message, taking into consideration its cultural context.
The interpreter strives to maintain impartiality and refrains from counseling, advising or projecting personal biases or beliefs.
The interpreter maintains the boundaries of the professional role, refraining from personal involvement.
The interpreter continuously strives to develop awareness of his/her own and other (including biomedical) cultures encountered in the performance of their professional duties. First and foremost, interpreters have to work at understanding the cultural basis of the way they themselves make sense of the world. Secondarily, this principle also acknowledges that there are many cultures that interpreters may encounter in the course of performing their duties. Does this mean that interpreters have to know everything about the patient’s culture?
The interpreter treats all parties with respect. Interpreters will not interject personal opinions or counsel patients.
When the patient’s health, well-being, or dignity is at risk, the interpreter may be justified in acting as an advocate. Advocacy is understood as an action taken on behalf of an individual that goes beyond facilitating communication, with the intention of supporting good health outcomes. Advocacy must only be undertaken after careful and thoughtful analysis of the situation and if other less intrusive actions have not resolved the problem.
Interpreters will keep abreast of their evolving languages and medical terminology.
The interpreter must at all times act in a professional and ethical manner.
