Source text - English 1. PURPOSE
The purpose of this Health, Safety and Environmental Execution Plan is to describe the principles, methods, tools and techniques applied for efficient implementation of HSE management system by the CONSORTIUM constituted among Hyundai Engineering Co., Ltd. and Lilama 69-2 Co. (hereinafter “Tenderer”) will perform the Project of No.2 Diammonium Phosphate (hereinafter DAP2) fertilizer plant including Design, Engineering, Procurement, Construction, Pre-commissioning Commissioning, Training, Start-up, Performance Test Run and Operation Assistance phases in Vietnam (hereinafter “Project”). TENDERER places a great deal of emphasis on maintaining the highest standards of Health, Safety and Environment (HSE) Management System. Compliance with legislation will be maintained as a minimum objective. The HSE execution plan has been developed to provide the necessary guidelines to enable the TENDERER including the Subcontractors to manage HSE requirements at various execution stages of the PROJECT.
2. SCOPE
TENDERER will confirm and indicate its understanding of OWNER’s policy on design and operational safety and on the safety requirements on the form as per Article 9 ‘Health, Safety and Environment Management in Chapter 11 ‘Scope of Works & Requirements of the ITB.
This HSE Management System (hereinafter referred to HSE-MS) is designed to describe various safety methods and techniques to be followed by the TENDERER and his SUBCONTRACTORS during execution of the PROJECT. In order to comply with the latest HSE requirements, the plan will be continuously reviewed and updated as appropriate throughout the PROJECT.
3. HSE ORGANIZATION
The HSE Management System ensures that the PROJECT Health, Safety and Environmental Objectives shall be met through implementation of technical activities and procedures, a PROJECT HSE Team shall be appointed to perform all the technical activities included in this HSE Plan. The preliminary Project HSE Organizaion is shown in Section 2, ‘Organization’ of this PROPOSAL.
The TENDERER Head Office, Office of Safety and Environmental Management based in Seoul–Korea, supports the Project HSE Team with specialist advice, guidance, inputs, consultation, inspection, auditing and directions for the execution of the tasks.
The Project HSE Team interfaces with TENDERER Project Management, also with SUBCONTRACTORS and VENDORS, to establish that all activities comply with the Project HSE Objectives.

2. PHẠM VI

Hệ thống quản lý HSE này (sau đây gọi là HSE-MS) được thiết kế để mô tả các phương pháp và kỹ thuật an toàn khác nhau để NHÀ THẦU và CÁC NHÀ THẦU PHỤ tuân thủ trong quá trình thực hiện DỰ ÁN này. Để thực hiện theo các yêu cầu mới nhất của HSE, kế hoạch này sẽ được xem xét và cập nhật liên tục cho phù hợp trong suốt dự án này.
3. TỔ CHỨC
Các hệ thống quản lý HSE đảm bảo rằng các Mục tiêu về Sức khỏe, An toàn và môi trường của Dự án phải được đáp ứng trong suốt quá trình triển khai các hoạt động và quy trình kỹ thuật, một ĐỘI HSE của DỰ ÁN sẽ được chỉ định để thực hiện tất cả các hoạt động kỹ thuật được bao gồm trong Kế hoạch HSE này. Tổ chức HSE sơ bộ của Dự án được trình bày trong mục 2, "Tổ chức’ của Dự án này.
Trụ sở chính, Phòng Quản lý An toàn và môi trường của NHÀ THẦU đặt tại Seoul-Hàn Quốc, hỗ trợ đội HSE của dự án thông qua tư vấn chuyên môn, hướng dẫn, các yếu tố đầu vào, tư vấn, kiểm tra, kiểm toán và chỉ dẫn để thực hiện nhiệm vụ.

Source text - English Quantitative whole-body autoradiography revealed that [14C]finerenone distributes equally into rat cardiac and renal tissues. Finerenone treatment prevented deoxycorticosterone acetate/salt challenged rats from functional as well as structural heart and kidney damage at dosages not reducing systemic blood pressure. Finerenone reduced cardiac hypertrophy, plasma prohormone of brain natriuretic peptide and proteinuria more efficiently than eplerenone when comparing equi-natriuretic doses.
In a Phase IIa study, finerenone has been previously studied in individuals with stable HFrEF and mild (eGFR 60 to < 90 mL/min/1.73 m2) -to-moderate (30–60 mL/min/ 1.73 m2) chronic kidney disease (CKD). In the minerAlocorticoid Receptor Antagonist Tolerability Study (ARTS; NCT01345656), finerenone 5.0–10.0 mg/day reduced plasma natriuretic peptides levels and albuminuria to the same magnitude as spironolactone 25–50 mg/day but was associated with a significantly smaller mean increase in serum potassium concentration and smaller decreases in eGFR [24].
The MinerAlocorticoid Receptor Antagonist Tolerability Study-Diabetic Nephropathy (ARTS-DN; IMPACT No. 16243; NCT01874431) was a multicentre, randomized, double- blind, placebo-controlled, parallel-group, phase IIb study investigating the safety and efficacy of several oral doses of finerenone in patients with type 2 diabetes mellitus and a clinical diagnosis of DN, based on either high albuminuria (urinary albumin-to-creatinine ratio [UACR] 30 to 299 mg/g) or very high albuminuria (UACR 300 to 3000 mg/g), and with an estimated glomerular filtration rate (eGFR) of 30 mL/min/1.73 m2 or more. Finerenone doses of 1.25 mg up to 20 mg once daily (od) were investigated over 90 days of treatment compared to placebo on top of standard of care, including a RAS-inhibitor. Finerenone reduced albuminuria by 21 to 38%, compared to placebo, at doses of 7.5 mg to 20 mg od when added to RAS blockade, and neither hyperkalemia nor reductions in eGFR limited its use.
Based on the aforementioned properties but also first results in clinical trials with patients with the clinical diagnosis of DKD, finerenone could have the potential to address the unmet medical needs in this population. When added to standard of care therapy with a RAS blocker finerenone given once daily might demonstrate a delay in progression of renal disease but also a reduction of CV mortality and morbidity.
As part of its Phase 3 program, finerenone will be investigated in two clinical trials in two distinct populations: in Study 16244, finerenone will be investigated in high-risk patients for renal but also cardiovascular events and in a second trial, Study 17530, in a high-risk population for cardiovascular events. Accordingly, in Study 16244 the primary endpoint will focus on renal events whereas in Study 17530 the primary endpoint will focus on cardiovascular events.
3. Study objectives
The primary objective of this study is
• To demonstrate whether finerenone is superior to placebo, in addition to standard of care (SoC), in delaying the progression of kidney disease, as measured by the composite endpoint of time to first occurrence of kidney failure or a sustained

decrease of eGFR ≥ 40% from baseline over at least 4 weeks or renal death, in patients
with type 2 diabetes mellitus (T2DM) and the clinical diagnosis of DKD.
The secondary objectives of the study are to determine whether finerenone is superior to placebo, in addition to SoC, in
• Delaying the time to first occurrence of the following composite endpoint: cardiovascular (CV) death or non-fatal CV events (i.e. non-fatal myocardial infarction, non-fatal stroke, unplanned hospitalization for heart failure [HF] or its equivalent)
• Delaying the time to all-cause mortality
• Delaying the time to all-cause hospitalizations.
4. Study design
Study 16244 is a randomized, double-blind, placebo-controlled, parallel-group, multicenter, event-driven Phase III study. Figure 4-1 displays the overall study design.
Patients from approximately 650 study centers worldwide will be randomized in a 1:1 ratio to either finerenone or placebo in addition to SoC. Assuming a screening failure rate of approximately 50%, 9,600 patients will have to be screened to randomize approximately
4,800 patients.
The anticipated duration of the study is approximately 3 years, including an anticipated recruitment period of 2 years and a follow-up period of 1 year after enrolment of the last patient into the study. However, as an event-driven study, the actual length of the study will depend on the observed event rates, the patient recruitment rate, and length of the recruitment period.
As all randomized patients will belong to the Full Analysis Set (FAS) and be part of the efficacy analyses, it is important to avoid randomization of non-eligible patients into the study. In line with the intention-to-treat principle, once a patient is randomized, treatment should be started or continued, even if some inclusion/exclusion criteria are found to be violated, unless there is a concern for the patient’s safety at the investigator’s judgment.

Là một phần trong chương trình nghiên cứu pha 3, finerenone sẽ được nghiên cứu trong hai thử nghiệm lâm sàng ở hai quần thể khác nhau: trong Nghiên cứu 16244, finerenone sẽ được nghiên cứu ở những bệnh nhân có nguy cơ cao đối với các biến cố thận cũng như biến cố tim mạch và trong nghiên cứu thứ hai, Nghiên cứu 17530, ở nhóm bệnh nhân có nguy cơ cao đối với các biến cố tim mạch. 3. Mục tiêu nghiên cứu
Mục tiêu chính của nghiên cứu này là
• Chứng minh liệu finerenone có vượt trội hơn so với giả dược hay không, ngoài điều trị chuẩn (SoC), trong việc làm chậm tiến triển của bệnh thận, như được đánh giá theo kết cục hỗn hợp về thời gian xuất hiện suy thận lần đầu hoặc giảm eGFR liên tục ≥ 40% so với mức ban đầu trong ít nhất 4 tuần hoặc tử vong do thận, ở bệnh nhân đái tháo đường tuýp 2, và chẩn đoán trên lâm sàng là DKD.
Mục tiêu phụ của nghiên cứu là xác định liệu finerenone có vượt trội hơn so với giả dược không, ngoài tiêu điều trị chuẩn, trong việc
• Trì hoãn thời gian xuất hiện lần đầu của kết cục hỗn hợp như sau: tử vong do tim mạch (CV) hoặc các biến cố tim mạch không gây tử vong (như: nhồi máu cơ tim không gây tử vong, đột quỵ không gây tử vong, nhập viện bất ngờ do suy tim [HF] hoặc tương đương)
• Trì hoãn thời gian xuất hiện tử vong do mọi nguyên nhân
• Trì hoãn thời gian nhập viện do mọi nguyên nhân.
4. Thiết kế nghiên cứu
Nghiên cứu 16244 là một nghiên cứu Pha III, ngẫu nhiên, mù đôi, đối chứng giả dược, nhóm song song, đa trung tâm, hướng biến cố. Hình 4-1 thể hiện thiết kế nghiên cứu nói chung.
Bệnh nhân từ khoảng 650 trung tâm nghiên cứu trên toàn thế giới sẽ được phân ngẫu nhiên theo tỷ lệ 1:1 vào nhóm finerenone hoặc giả dược ngoài SoC. Giả sử tỷ lệ sàng lọc thất bại khoảng 50% thì 9.600 bệnh nhân sẽ phải được sàng lọc để ngẫu nhiên hóa khoảng 4.800 bệnh nhân.
Thời gian nghiên cứu dự kiến là khoảng 3 năm, bao gồm giai đoạn tuyển bệnh 2 năm và giai đoạn theo dõi 1 năm sau khi tuyển chọn bệnh nhân cuối cùng vào nghiên cứu. Tuy nhiên, do đây là nghiên cứu hướng biến cố nên thời gian nghiên cứu thực tế sẽ phụ thuộc vào tỷ lệ biến cố ghi nhận được, tỷ lệ tuyển bệnh, và thời gian của giai đoạn tuyển bệnh.
Vì tất cả bệnh nhân chọn ngẫu nhiên sẽ tùy thuộc vào FAS (the Full Analysis Set) và là một phần trong các phân tích hiệu quả nên phải tránh phân ngẫu nhiên những bệnh nhân không đủ điều kiện vào nghiên cứu. Theo nguyên tắc ý định điều trị, khi một bệnh nhân được phân ngẫu nhiên, việc điều trị phải được bắt đầu hoặc tiếp tục, ngay cả khi phát hiện vi phạm một số tiêu chí nhận vào/loại trừ, trừ khi có quan ngại về tính an toàn của bệnh nhân theo đánh giá của nghiên cứu viên.
