На момент поступление состояние пациента средней степени тяжести, нестабильное. Нестабильноегь состояния пациента обусловлена наличием стенокардии напряжения высокого функционального класса, стойко толерантной к проводимой кардиотропной терапии, осложненной тяжелым нарушением ритма сердца (по типу вентрикулярной экетрасистолии IV класса): поражением значимых сосудов сердца, высоким риском развития повторного фатального инфаркта миокарда. По данным ЭХО-КГ: гипокинезия верхушечного сегмента МЖП, акинезия задней стенки ЛЖ, что может быть причиной сердечной слабости как интраоперационно, так и в послеоперационном периоде: а также наличием тяжелой сопутствующей патологией: язвенная болезнь желудка, что может являться причиной развития желудочно-кишечного кровотечения на фоне применения антикоагулянтов после проведения операции; сахарный диабет 2 типа, II степени тяжести - у больного сахарный диабет, что может являться причиной замедления репарационных процессов в послеоперационном периоде: по данным лабораторного исследования у пациента тромбоцитопения, высокий уровень РФМК (5,5 мг%), что косвенно свидетельствует о скрытом ДВС - синдроме. Учитывая вышеописанное пациенту, по жизненным показаниям 25.06.12г. выполнено оперативное лечение в объеме прямой реваскуляризации миокарда: маммарно¬коронарное шунтирование с ПМЖВ и ДВ, АКШ с ЗМЖВ ПКА в условиях искусственного кровообращения и холодовой фармакокардиоплегии.
Translation - English Considering that the patient has ischaemic heart disease, exertional angina of a high functional class, post-infarction cardiosclerosis, and that he is in danger of developing a fatal myocardial infarction, the patient underwent coronary angiography: 60% stenosis of the trunk of the left coronary artery, 70-80% stenosis of the anterior interventricular branch along its length, occlusion of the circumflex branch in the middle third, 70% stenosis of the proximal third of the right coronary artery.
At the time of admission, the patient was in a moderately severe and unstable condition. The unstable nature of the patient’s condition was characterised by the presence of exertional angina of a high functional class, highly resistant to the conducted cardiotropic therapy, and complicated by severe disturbances in cardiac rhythm (characterized by ventricular extrasystole, grade IV), with significant involvement of the coronary arteries, and a high risk of developing a recurrent fatal myocardial infarction. Echocardiogram results: Hypokinesia of the apical segment of the interventricular septum, akinesia of the posterior wall of the left ventricle, which could be the cause of cardiac weakness in both the intra-operative and post-operative periods, as well as the presence of serious comorbidities: gastric ulcer, which could be the cause of gastro-intestinal bleeding with underlying use of anti-coagulants after surgery; type II diabetes mellitus, stage II severity, which could be the cause of slower healing processes in the post-operative period. According to laboratory findings, the patient has thrombocytopenia, a high level of soluble fibrin complexes (5.5 mg%), which indirectly indicates the presence of a hidden disseminated intravascular coagulation syndrome. Considering the aforementioned and the patient’s vital signs, the patient underwent direct myocardial revascularisation on 25/06/2012, mammary-coronary stenting of the anterior interventricular branch and the diagonal branch, and aortocoronary stenting of the posterior interventricular branch of the right coronary artery using cardiopulmonary bypass and hypothermic pharmacological cardioplegia.

Произрастали они в Вятском в таком количестве, что в 18 веке стоимость тысячи штук огурцов равнялась целому рублю. «Вятские – огурцы хватские» (хрустящие, ядреные) доставлялись солеными даже к царскому столу, и в качестве деликатеса экспортировались за границу. В 1906 году на продажу в Москву и Петербург, а также в Швецию, Швейцарию и Данию из Вятского вывезли 8 млн. килограммов соленых огурцов, правда, с учетом веса тары.
Рецепт соленых вятских огурчиков был достаточно прост: «дикие» огурцы (выращенные просто в полях, без поливки и ухода) складывали в огромные бочки, заливали соленым рассолом, закрывали и поздней осенью опускали в местную речку Ухтомку. По весне бочки вынимали - огурчики были готовы. Местные жители уверяют, что секрет особых вятских огурцов в уникальной родниковой воде, которая используется при засолке. 19 августа, на праздник Преображения, в Вятском до сих пор празднуют День огурца – в честь главного гастрономического сельского бренда.
Translation - English The Vyatskoe village has long been known for its delicious cucumbers. It was once called the cucumber capital of Russia. They grew in Vyatskoe is such numbers that in the 18th century the price of a thousand cucumbers was equal to a whole rouble. “The snappy Vyatskoe cucumbers” (crunchy, robust) were delivered pickled even to the Tsar’s table, and were exported overseas as a delicacy. In 1906, 8 million kilograms of pickled cucumbers (including the tare weight) were brought from Vyatskoe for sale to Moscow, St Petersburg and even Sweden, Switzerland and Denmark.
The recipe for pickling cucumbers was quite simple: “wild” cucumbers (grown in the fields without watering or other care) were collected in huge barrels, poured over with salty brine, sealed, and lowered into the local river Ukhtomka in late autumn. In spring, the barrels were lifted up as the cucumbers were ready. The locals swear that the secret of the special Vyatskoe cucumbers is in the unique spring water, which is used for pickling. On the 19th of August, during the Feast of Transfiguration, the people of Vyatskoe still celebrate the Day of the Cucumber, in honor of the main rural gastronomic brand.

In the darkness, Eldhouse had trouble distinguishing the faces of the two teenagers standing behind him. He choked out, “What do you want?”

- Конечно.
- По каким проявлениям?
- Характерная клиническая картина, которая не требовала дополнительного обследования, каких-то анализов. Чаще всего диагноз ставится по внешнему виду.
- Существуют какие-то определённые параметры?
- Да. Высыпания на лице, красные пятна, телеангиэкстазия и папулёзно-пустулёзная сыпь.
- Есть ли различия в простоте диагностики форм розацеа: папулёзно-пустулёзной (ППР) и эритематозно-телеангиэктатической (ЭТР)?
- Всё зависит от того, что видим на лице. Если только покраснения и расширение сосудов – это ЭТР. Если появились узелковые высыпания речь идёт о эритематозно-популёзной форме. Возникли гнойнички, говорим о ППР.
- Вспоминая случаи, когда вы ставили диагноз «розацеа», каковы причины первоначального обращения пациентов?
- Проблемы с внешним видом. Обеспокоенность покраснениями и высыпаниями на лице.
- По сути это уже симптомы розацеа. Они пришли, зная об этом заболевании, или у них был другой диагноз?
- Они узнают о диагнозе на приёме. Некоторые приходят уже с врачебным ведиктом. Они получали лечение, но оно оказалось малоэффективным, поэтому они выбрали другого специалиста.

- Issues with their appearance. Concerns regarding the redness and rash on their face.
- Basically, these are symptoms of rosacea. Did they come to you already knowing about this disease, or did they have a different diagnosis?
- They find out about their diagnosis at the appointment. Some of them come with a diagnosis already made by their doctor. They may have received some treatment, but it had been ineffective so they decided to see a different specialist.

Source text - Russian В последние годы эффективность использования многих аллопатических и гомеопатических препаратов при заболеваниях инфекционно-воспалительного характера связывают с их влиянием на факторы иммунитета (Г.Н. Дранник, 2006; О.Ф. Мельников и соавт., 2006). В условиях эксперимента нами было показано, что клиническая эффективность гомеопатического препарата тонзиллотрен (фирма DHU, Германия) при заболеваниях верхних дыхательных путей может быть связана с его действием преимущественно на реакции клеточного типа как врожденные, так и приобретенные. При этом препарат активировал деструктивную активность естественных цитотоксических клеток (ЕЦК), активность фагоцитирующих клеток, а также цитолитическую активность иммунных Т-лимфоцитов (Д. И. Заболотный, О.Ф. Мельников, 2006). Известно, что активация функции цитолитических клеток в значительной степени обусловлена влиянием системы интерферона и других цитокинов (А.С. Симбирцев и соавт., 2004), что определяет назначение препаратов с подобным свойством при вирусных инфекциях. Как активность естественных цитотоксических клеток (ЕЦК), так и специфических цитотоксических Т-лимфоцитов в значительной степени связывают с системой интерферонов и противовирусной резистентностью (Ф.И. Ершов и соавт., 2004; В.З. Кривицкая и соавт., 2005; Г.Н. Дранник, 2006). Translation - English In recent years, the efficacy of many allopathic and homeopathic medicines for infectious and inflammatory diseases has been linked to their effect on factors of immunity (G.N. Drannik, 2006; O.F. Melnikov et al., 2006). Under experimental conditions, we have shown that the clinical efficacy of the homeopathic drug Tonsilotren (DHU, Germany) for diseases of the upper respiratory tract may be associated with its action primarily on cell-mediated reactions, both innate and adaptive. At the same time, the drug activated the destructive activity of natural cytotoxic cells (NCC), the activity of phagocytes, as well as the cytolytic activity of T-lymphocytes (D.I. Zabolotny, O.F. Melnikov, 2006). It is known that the activation of cytolytic cells is largely due to the effects of the interferon system and other cytokines (A.S. Simbirtsev et al., 2004), therefore, drugs with similar properties should be prescribed during viral infections. The activity of both natural cytotoxic cells (NCC) and specific cytotoxic T-lymphocytes is largely associated with the interferon system and antiviral resistance (F.I. Ershov et al., 2004; V.Z. Krivitskaya et al., 2005; G.N. Drannik, 2006). Due to this fact, it seemed worthwhile to study the effect of Tonsilotren on the cytolytic activity of blood cells and tonsils, their production and possible production of various types of interferon, which can contribute to a deeper understanding of the drug's mechanism of action, expanding the indications for its use at different stages of a viral infection.
