Warm the affected area through a piece of cloth

I think it means that something warm should be applied to the affected area, but not directly to the skin, but through some thin barrier, which is usually cloth. Just like when they recommend to apply ice, it's recommended not to apply it directly, but through something - like a towel, for example.
It's not clothing - it's 布, cloth. Clothing would be 服, for instance.
