Indeed, this does not sound as native English, as far as I can tell.
Given the context you have provided it can very well be εφημερίες although I can hardly imagine a first year medical student **actively** participating in them. Perhaps, they simply attended.
It could also be clinical examinations, so κλινικές εξετάσεις (ασθενών). I think this is a likely mistranslation.

to guard is a clumsy translation obviously made by a non native speaker. IMO what the writer wants to say is that she was assigned specific care tasks with a certain amount of responsibility when looking after the patients, administering medicine, changing dressings and bandages and such like.

I have a feeling that he/she who wrote this recomendation letter is not a native english speaker, since the University is based in Romania. I believe though that "clinical probations" doesn't have a negative meaning and it refers more to "practice". What do you believe?
Additionally: guard services? εφημερίες?
