Source text - English Insulin resistance in CKD is a unique metabolic abnormality induced by factors such as uremic toxins, chronic inflammation, excess visceral fat, oxidative stress, metabolic acidosis, and deficiency of vitamin D and erythropoietin. Improvement with dialysis treatment is due at least in part to removal of uremic toxins.
To what extent insulin resistance also contributes to protein-energy wasting through the ubiquitin—proteosome pathway, to atherosclerosis, and to cardiovascular disease remains unclear in ESRD.
The net clinical effect of these defects in diabetic ESRD are variable: insulin resistance in advancing CKD may be offset by insulin retention and poor caloric intake; improvement in insulin resistance by dialysis may be offset by more robust caloric intake as uremia improves.

Улучшение при диализной терапии наблюдается вследствие, как минимум, частичного выведения
уремических токсинов. При ТХПН остается неясным, в какой степени инсулинорезистентность также способствует развитию белково-энергетической недостаточности путем убихитин-протеосомного пути, атеросклероза и сердечно-сосудистой
патологии. Итоговый клинический эффект этих нарушений при диабетической ТХПН разнится: инсулинорезистентность при прогрессирующей ХПН может быть нивелирована удержанием инсулина и низким потреблением калорий; улучшение течения инсулинорезистентности путем диализа может быть сведено к нулю более активным потреблением калорий по мере устранения уремии.

ABOUT ME
I am a professional translator, graduated from National Technical University of Ukraine “Kyiv Polytechnical Institute” in 2013 with a diploma of a bachelor of linguistics. Some years before that, 2007, I had graduated from a medical college with a diploma of a medical assistant. So now in my work I can apply my knowledge on two different areas – medicine and linguistics.
