Reducir la Tasa de Mortalidad Infantil y Neonatal en la ciudad de Cartagena, mediante el apadrinamiento y suministro del tratamiento necesarios para a salvar sus vidas.
1.1. Objetivos Específicos:
Identificar población menor de 1 año con patologías definidas dentro de su edad en riesgo de muerte.
Suministrar los tratamientos médicos necesarios para salvar sus vidas.
Concientizar a los padres de la importancia de los seguimientos y controles médicos para evitar enfermedades y poder brindar una mejor calidad de vida a sus hijos.
Desarrollar actividades orientadas a la educación, crianza, cuidados, vínculo afectivo Madre/Hijo.
Crear espacios de integración a través de la Escuela de Padres que permitan interactuar e integrarse con los demás padres del PPC y a su vez aprovechar y desarrollar sus habilidades.
2. Proceso de Identificación y selección de la población al PPC:
Este proceso es realizado durante todo el año por la Gerencia del programa, de la siguiente manera:
Se realizan vista periódicas a los pacientes hospitalizados en el HINFP para ser incluidos en el programa, según criterios de apadrinamiento, vinculados o no al SGSSS.
Se realizan visitas periódicas a los CAP, Comunidades y otras instituciones de salud ser incluidos en el programa según criterios de apadrinamiento o en su momento para ser remitidos, con enfermedades prevalentes de la infancia.
Los niños deben tener una edad de 0 a 1 año.
Deben tener patologías prevalentes de la Infancia.
Los pacientes con bajo peso o muy bajo peso no hospitalizados, deben contar con valoración previa del Médico General en el Centro Médico xxxxx
Familia de escasos recursos y comprometidas con el proceso de el PPC.
3. Criterios para la vinculación al PPC
Los niños deben ser recién nacidos con un peso superior a 1.500 gramos o niños menores de 1 año, que cuenten con EPS’S, SISBEN o sin ningún tipo de seguro.
Estar hospitalizado en el Hospital Infantil xxxxx, donde la vida del paciente se encuentre en riesgo de muerte, con patologías definidas dentro la edad como: Bronconeumonía, EDA, Hidrocefalia, Neumonía, Sepsis Neonatal entre otros.
Niños que se encuentren en los Centros de Atención en Salud y los Hospitales públicos, que presenten enfermedades de alto riesgo que requieran de tratamiento médicos ambulatorios.
Niños que se encuentren -2 D/E (Desviación Estándar) los cuales son los que presentan bajo peso o muy bajo peso donde la vida del paciente se encuentre en alto riesgo; si estos no se encuentran hospitalizados deben recibir una valoración inicial por el Médico general del Centro Médico xxxxx donde se determina el diagnóstico, peso, talla y condición de riesgo del paciente para su ingreso al programa del PPC.
Familia de escasos recursos económicos y condiciones vulnerables.
Padres y familiares comprometidos con el proceso de apadrinamiento y responsabilidad hacia el menor.
4. Proceso de Inscripción de la Población al PPC:
Se deben cumplir los siguientes pasos:
4.1. PACIENTES HOSPITALIZADOS:
Realizar inicialmente en el Centro Hospitalario de Salud, una ronda e inspección en los diferentes servicios, para la detección de posibles casos para el apadrinamiento.
Solicitar Historia Clínica de los pacientes identificados e indagar con el personal asistencial de la institución la evolución del paciente.
Se aborda al familiar (madre, padre o responsable) del menor seleccionado y se realiza una inducción breve del programa y sus beneficios
Diligenciar el formato o ficha de apadrinamiento del recién nacido o menor de 1 año de edad, la cual debe ser firmada por sus padres.
Suministro de copia del documento de identificación del familiar responsable, copia del registro civil o nacido vivo si fuera el caso y copia del carnet de la EPS’S o SISBEN que tenga el paciente.
Diligenciamiento del Formato de testimonio y huellas del paciente y se le realiza su primera fotografía que evidencia el estado inicial como ingresa el menor de 1 año al programa.
Se le entrega al familiar, una orden que debe ser entregada en el xxxxxx, para la realización de una valoración Psicosocial por parte de la Psicóloga del área Psicosocial donde además esta orden contiene la dirección y teléfonos de la FUNDACION xxxxxxxx, para que al momento de su salida de la institución de salud, pueda continuar con su tratamiento ambulatorio, en el Centro Médico xxxxxx

2. Identification Process and Admission into PPC:
This process takes place throughout the year by Program Management as follows:
Through periodic visits to patients hospitalized in HINFP with the intention of including them in the program, whether or not they’re associated with SGSSS, as long as they meet sponsorship criteria
Through periodic visits to CAP, Communities and other healthcare institutions where children who could benefit from PPC, and meet the criteria, could be found.
Children should not be more than one year old.
They should have known childhood diseases.
Those patients with low weight or very low weight and not hospitalized, must first be evaluated by a General Doctor from the xxxxMedical Center.
Low-income family that is committed to the PPC process.
3. Requirements for Admission into PPC:
The children must be newborns with a weight superior to 1.500 grams, or less than a year old with EPS’S, SISBEN, or without any type of insurance.
Be hospitalized at the Infantil Napoleón Franco Pareja Hospital, where the patient’s life is at risk of death from a specific illness such as: Bronchopneumonia, EDA, hydrocephalus, pneumonia, neonatal sepsis, among others.
Children in Public Hospitals or Healthcare Centers with high risk illnesses that require out-patient medical treatment.
Children who are -2 S/D (Standard Deviation), that is, with low, or very low, weight such that the patients’ lives are at high risk. If they are not hospitalized, they must first be evaluated by a General Doctor from the Juan Felipe Gómez Escobar Medical Center where a diagnosis, weight, size, and level of the patient’s risk will be determined for admission into the PPC program.

Parents and close family members committed to the sponsorship process and who assume responsibility for the child.
4. Application Process for Admission into PPC :
The following steps must be completed:
4.1. HOSPITALIZED PATIENTS:
Do an initial round of the Health Center, checking the different areas to identify possible cases for sponsorship.
Request the Medical Records of those patients identified and discuss their progress with the nursing staff.
Make contact with the identified child’s family (mother, father or guardian) and offer a brief explanation about the program and its benefits.
Fill out the sponsorship application for the newborn, or child less than one year old, which must be signed by the parents.
Supply copy of the responsible family member’s identification, birth certificate, and copy of the patient’s EPS’S or SISBEN identification card.
Legal Consent Form along with the patient’s fingerprint and photograph showing the child’s condition upon admission into the program.
The family member will be given an order to take to the xxx Medical Center for a Psychosocial assessment by the psychologist from the Psychosocial area. In addition, the order will contain the address and telephone numbers of the xxxxxxFOUNDATION so that once the patient leaves the health institution, out-patient care can continue at the xxxMedical Center.
