간장은 어떠한 질병이 일어나도 장애가 상당히 진행되기 전에는 자각증상이 나타나지 않기 때문에 질병의 발견이 늦어질 수 있어, 일명 ‘침묵의 장기’라고 부른다.
간장의 이상을 발견하는 검사로는 혈액에 의한 간기능검사가 기본이다. 건강진단에서는 보통 지표가 되는 효소 및 단백질 등이 혈액 중에 얼마만큼 함유되어 있는가를 측정하는 것에 의해 간접적으로 간장의 상태를 조사할 수 있다.
간기능의 상태를 조사하는 혈액검사의 항목은 여러 가지가 있지만, 일반적으로 건강진단에서 조사하고 있는 것은 ‘GOT (AST)', 'GPT (ALT)', 'γ-GTP', 'ALT', '총빌리루빈’ 및 ‘혈청총단백’ 등이다.
한편, 간기능 검사는 간장을 직접 조사하는 것이 아니기 때문에 하나의 검사 결과만으로는 알 수 없고, 여러 가지 검사 결과를 종합적으로 파악함으로써 이상을 발견할 수 있다.

▶ GOT [AST], GPT [ALT] 이들 검사치가 높은 경우는 우선 간장병을 의심한다. 특히 GPT가 높으면 간장의 이상에 의한 것으로 생각할 수 있다. GOT만이 높은 경우에는 심근경색 및 근염 등의 근육질병을 의심한다. 알콜성 간장애인 경우도 있다. GOT, GPT치의 상승이 발견되면 간염이나 간경변, 간암 등의 가능성을 의심하여 자세한 검사를 하는 것이 필요하다. 급성 간염의 초기일 수도 있기 때문에 수치가 100IU/L를 넘으면 즉시 진찰을 받도록 한다. 100IU/L를 넘지 않을 정도의 상승에서도 1개월 이내에는 진찰을 받을 것이 권장된다. C형 간염에서는 급성 간염으로 GOT나 GPT가 수백의 높은 수치를 나타내도 자각증상이 없는 경우가 드물게 있다. 때문에 알지도 못하는 사이에 만성간염이 되는 예가 많은데, 급성 간염일 때 치료하면 대부분 낫는다. 최근에는 건강한 간장의 기준이 되는 검사치를 기존의 GOT 및 GPT의 기준치보다 더욱 낮게 보는 경향이 있어 기준치의 하향을 검토 중에 있다.
▶ γ-GTP 이 검사치가 높은 경우에는 간장병 이외에 담석 및 담관암, 췌두부암(십이지장에 가까운 부위에 생기는 췌장암) 등 간장의 흐름이 저해되는 질병일 가능성이 있다. 기타 간기능 검사의 수치가 기본치 이내이고, γ-GTP만이 높을 경우에는 알코올이 원인일 가능성이 높다. 술을 너무 많이 마셔서 생긴 경도의 상승이라면 술을 끊으면 일주일 이내에 수치가 내려가기 시작한다. 그러나 술을 마시지 않았는데도 γ-GTP가 높을 경우에는 NASH (비알콜성 지방성 간염) 등을 의심할 수 있다. 또 여성에서 GOT, GPT의 수치가 그다지 높지 않은데, γ-GTP와 ALP가 높은 경우에는 ‘원발성 담즙성 간경변’을 생각할 수 있다.
▶ ALP ALP치의 상승은 간기능 저하 및 담즙울체를 나타낸다. 원인으로는 간염 등의 간장병, 담석 및 담도암 등을 생각하라 수 있다. 알코올성 간장애가 만성화하여 간경변이 된 경우에는 γ-GTP가 그다지 높지 않고 ALP가 현저히 높아지는 일이 있다. 그밖에 간기능 검사에서는 이상이 없고 ALP가 높은 경우에는 골질환 등을 의심할 수 있다.
▶총빌리루빈 총빌리루빈치가 높을 대에는 급성간염, 간경변, 알코올성 간염 등의 간장병 및 담석, 담낭염, 담관암 등의 담도의 질병이 의심된다. 간장병의 경우는 아울러 GOT 및 GPT의 상승이, 담도 이상의 경우에는 ALP 및 γ-GTP의 상승이 보인다. 보통 총빌리루빈이 2~3mg/dL 이상되면 황달이 나타난다. 황달이 있는 경우에는 즉시 진찰을 받아야 한다. 공복 시에 채혈하여 조사하면 간장 및 담도 이상이 없어도 2mg/dL 정도까지의 경도상승을 나타내는 사람이 있는데, 이는 ‘체질성 황달’로 걱정할 필요는 없다.
▶ 혈청총단백 혈청에 함유된 단백질의 총량을 조사한다. 주된 것은 알부민과 글로불린이다. 대부분이 가장에서 합성되기 때문에 간장의 기능에 어떠한 문제가 생기면 이상치가 나타난다. 혈청총단백의 수치가 너무 저하하면 간장병에 의한 간기능 저하, 영양 불량, 소화흡수장애 등을 생각할 수 있다. 간장병으로 요(尿)에 다량의 단백질이 나오는 경우도 수치가 낮아질 수 있다. 수치가 지나치게 높아질 때에는 교원병, 다발성 골수종 등을 의심한다. 대부분은 글로불린 중 면역글로불린이 높아지는 것에 의한다.
▶혈청알부민 알부민은 간세포에서만 만들어지는 단백질로, 건강한 사람에서는 혈청에 함유된 단백질의 절반 이상을 점유하고 있다. 혈청알부민의 수치가 낮아질 때에는 간경변 및 극증간염 등으로 간장의 단백질 합성 기능이 저하된 것으로 생각할 수 있다. 알부민은 혈액의 삼투압을 조절하는 중요한 물질로, 혈액중에 알부민이 적어지면 수분이 혈관 밖으로 빠져나와 버려 그것이 부종 및 복강내 물이 차는 ‘복수(腹水)’의 원인이 된다. 혈청에 포함된 글로불린과 대비 (A/G비)는 간염상태의 기준이 된다. 건강한 사람의 A/G비는 1이상이다.
▶콜린에스테라제 콜린에스테라제는 간장에서 만들어지는 효소의 하나이다. 혈액중의 수치가 낮을 때에는 간경변 및 극중간염 등의 중증 간장병에 의해 간장의 단백질 합성 능력이 저하된 것으로 의심된다. 영양상태가 나빠 수치가 낮아질 수도 있다. 한편 지방간의 경우는 혈액 중에 콜린에스테라제가 증가하여 수치가 높아진다.
▶ ZTT [혈철교질반응] ZTT는 혈액에 시약을 첨가하여 탁한 정도로 조사하는 검사인데 검사치가 높을수록 탁한 정도가 높다. 혈청단백의 일종인 면역글로블린이 많을수록 탁한 정도가 강해지는데, 이는 간장의 이상을 의미한다. 만성 간염 및 간경변을 발견하거나 이들의 진행정도를 조사하는데 유용하다. GOT 및 GPT가 대부분 정상인 만성간염이 ZTT 이상으로부터 발견되는 예도 있다. 간장병 이외에 교원병 및 관절류마티즘 다발성 골수종 등에서도 면역글로불린이 많아지기 때문에 이상치를 나태내는 경우도 있다.
Translation - English Blood Test for Liver Function
With any form of liver disease, diagnosis may happen too late because no symptoms arise until the disease is already progressing. This is also known as the 'period of reticence'. The standard test for determining liver abnormalities is the Liver Function [blood] Test (LFT). The LFTs indirectly assess the state of the liver by measuring the enzyme and protein content in the blood. There are many blood tests that check the state of the liver, but generally in routine checkups the: GOT (AST); GPT (ALT); γ-GTP; ALT; and bilirubin/serum protein levels and so on are tested.
Although this is the case, it is impossible to determine liver function based on one test - as LTFs are not directly examining the liver. Only through a comprehensive range of tests we can get clearer information about any existing abnormalities.

▶ When the GOT [AST] and the GPT [ALT] level results are both high, it can be deduced that liver disease/hepatitis exists. Especially when the GPT levels are high, we can deduce that there are liver abnormalities. If only the GOT results are high, muscular dystrophy related-diseases such as myocardial infarction and myositis could be a possibility. Alcohol-related disorders may also be present. If an elevated level of both GOT and GPT is found; liver disease, cirrhosis or cancer may be present, so a more thorough examination will be required. If the levels exceed 1001IU/ L, there is a possibility that it is the beginning of acute hepatitis and further examination should be conducted straight away. If the levels are not elevated beyond 1001IU/ L, then it is recommended to have the examination within one month’s time. As for Hepatitis C, even when the GOT and GPT are several hundred IU/ L high, there is rarely a case of acute hepatitis that is asymptomatic. As this is an uncertain period of diagnosis, there are many cases that develop into chronic hepatitis, so treat the patient when there is acute hepatitis, and most of them will recover. In recent years, we are currently observing a decline in GOT and GTP as standard reference tests as they are being seen as less valuable for the standard measure in testing liver health.
▶ Apart from hepatitis, diseases linked to a high level test result on a γ-GTP are those that impede the liver flow such as gall stones, cholangiocarcinoma (bile duct cancer), pancreatic head cancer (pancreatic cancer occurring in areas close to the duodenum). If there is only a high reading on the γ-GTP, and the other liver test results fall within normal range, then this is most likely caused by alcohol. If the increase is caused by excessive alcohol consumption, abstinence from alcohol will start to bring the high levels down within a week. However, where this is no history of excessive alcohol consumption and the γ-GTP is high, a NASH (Nonalcoholic steatohepatitis) can be suspected. In addition, with women the GOT and GPT levels are generally not that high, so if a high γ-GTP and ALP are found then "primary biliary cirrhosis" must be considered.
▶ When ALP levels are elevated, this shows that liver disfunction and cholestasis are present. The probable causes are thought to be liver diseases such as hepatitis, gallstones, and biliary tract cancer. When cirrhosis arises through chronic alcohol-related disease, the γ-GTP level is not very high, and the ALP becomes remarkably higher. Outside of this, when there are no abnormalities on all other liver function tests, an elevated ALP indicates a bone disease may be present.
▶ When total bilirubin is high, it may be caused by: liver diseases such as acute hepatitis, cirrhosis, and alcohol-related liver disease; or biliary diseases like gallstones, cholecystitis, and cholangiocarcinoma. With liver diseases there is also an elevation in GOT and GPT levels; with biliary abnormalities, a rise in γ-GTP and ALP levels can be seen. Usually jaundice appears if the total bilirubin is more than 2 ~ 3mg/dL. You should see a doctor immediately if jaundice is present. Taking blood after fasting will result in some people having as much as a 2mg/dL rise – even without any liver and biliary abnormalities existing; this is "constitutional (metameric) jaundice” and there is no need to worry.
▶ The serum total protein test examines the total amount of protein contained in the serum. The two main types of protein are albumin and globulin. Whatever type of liver-function problem there is, an abnormality will show on this test because the majority of proteins are synthesized in the liver. A decrease in total serum-protein levels indicates liver disease due to such things as malnutrition, poor liver function, and digestive malabsorption. Even when there are large amounts of protein present in the urine, the serum total protein level can be decreased. Collagenosis and multiple myeloma should be considered when levels are extremely high. In most cases, the type of globulin that has increased is immunoglobulin.
▶ Serum Albumin Test: Albumin is a protein made only by liver cells; albumin makes up more than half of the proteins found in the serum of healthy people. When low levels of serum albumin are present, it is likely that the liver’s function to synthesize protein is impaired such as in diseases like hepatitis, cirrhosis and ecchymosis. Albumin regulates the osmotic pressure of the blood and is an important substance. When albumin levels are low in the blood, water leaches out of the blood vessels causing hydroperitoneum (abdominal dropsy) - where water fills the abdominal cavity - and also edema. The contrast of the globulin contained in the serum, the (A / G [albumin / globulin]) ratio, is a measure of the extent of hepatitis. A healthy human A / G ratio is 1 or more.
▶ Cholinesterase Test: Cholinesterase is one of the enzymes made in the liver. Low levels of cholinesterase in the blood indicate the liver has reduced capacity to synthesize proteins and serious liver diseases such as hepatitis or cirrhosis may be present. Poor nutrition may also lead to lower levels. On the other hand, in the case of fatty liver, the cholinesterase in the blood goes up and the levels increase.
▶ ZTT [Serum Colloidal Response / Zinc Turbidity Test] ZTT is a test that measures the degree of chemical-substance turbidity present in the blood; the higher the test level, the higher the turbidity. The more immunoglobin-type serum protein there is, the stronger the turbidity – this means there is a liver abnormality. This is useful for discovering chronic hepatitis or cirrhosis and in investigating the degrees of their progress. There are cases of finding chronic hepatitis through the ZTT when GOT and GPT were within normal range. There are many diseases - besides liver diseases - that can be found because of high immunoglobin levels, for example, multiple myeloma, rheumatoid arthritis, and collagenosis.
