Amaç: Bu çalışmada, deneyimli ve deneyimsiz endotrakeal entübasyon uygulayıcıları için, kompresyonlu ve kompresyonsuz senaryolarda direkt laringoskopi ve video laringoskopi ile yapılan entübasyonların entübasyon süresini, kompresyonlu senaryolarda ise kompresyon ve ventilasyon performans kriterlerini manken üzerinde karşılaştırılmasıdır.
Metot: Bu simülasyon çalışması toplam ikisi elektif entübasyon, diğer ikisi ise kompresyon eşliğinde entübasyon olmak üzere dört senaryo üzerinden gerçekleştirildi. Toplam 90 kişi, 2’li gruplar halinde 45 tim olacak şekilde 4 senaryoyu Laerdal Skillmeter Resusci-Anne® with SimPad mankeni üzerinde gerçekleştirdi. Senaryoların ventilasyon ve kompresyon performans verileri kaydedildi.
Bulgular: Senaryolar arasında hem kompresyonlu hem kompresyonsuz senaryolarda entübasyon süresi açısından Video Laringoskopi açısından anlamlı fark tespit edildi (kompresyonlu senaryolarda DL 23,21 ± 11,33, VL 17,06 ± 4,71, p < 0,001, kompresyonsuz senaryolarda DL 19,40 ± 8,03, VL 15,04 ± 3,31, p < 0,001). Hem kompresyonlu hem de kompresyonsuz entübasyon girişimlerinde, direkt laringoskopinin ikinci denemede gerçekleşme oranı video laringoskopiye kıyasla istatistiki anlamlı olacak şekilde daha fazlaydı (p = 0,008 ve p = 0,011). Translation - English COMPARISON OF ENDOTRACHEAL INTUBATIONS PERFORMED WITH DIRECT LARYNGOSCOPY and VIDEO LARYNGOSCOPY SCENARIOS WITH AND WITHOUT COMPRESSION: A MANIKIN-SIMULATED STUDY
Aim: The aim of this study was to compare the intubation period of intubations operated with direct laryngoscopy and video laryngoscope for the scenarios with and without compression and to assess the performance criteria of compression and ventilation in a manikin-simu lated scenario with compression, for the experienced and new endotracheal intubation operators.
Method: This manikin trial was carried out through a total of four scenarios, two of which were elective intubation and the other two were intubations with compression. A total of 90 people in 45 groups (each group consists of 2 persons) performed 4 scenarios in the Laerdal Skillmeter Resusci-Anne® with SimPad manikin. Ventilation and compression performance data of the scenarios were recorded.
Results: A significant difference was found between the scenarios in terms of intubation period and video laryngoscope in both scenarios with and without compression (in scenarios with compression, DL 23,21 ± 11,33, VL 17,06 ± 4,71, p < 0,001; in scenarios without compression, DL 19,40 ± 8,03, VL 15,04 ± 3,31, p < 0,001). In intubation interventions with and without compression, the success rate of direct laryngoscopy in the second intervention was more statistically significant compared to video laryngoscopy (p = 0,008 and p = 0,011). In the intubation scenarios with compression, the intubation success rates of the new operators were lower than the experienced participants, and it was statistically significant (p = 0,009).

Rezin bazlı kompozitler (RBK), kolay uygulama, konservatif çalışmaya imkan verme ve estetik olmaları nedeniyle sıklıkla tercih edilmektedirler. RBK’ler organik rezin matriks, inorganik doldurucular ve çiftli silan ajanlarından oluşan karmaşık bir yapıya sahiptir [1].
Rezin matriks içerisinde sıklıkla kullanılan monomerler; bisphenol A glycidyl methacrylate (Bis-GMA), triethylene glycol dimethacrylate (TEGDMA), urethane dimethacrylate (UDMA) ve bisphenol A (BPA) dır [2]. Literatürde; RBK’in içeriğinin ağız ortamında ayrıştığı gösterilmiştir. Bu durum materyalin biyouyumluluğu konusunda soru işareti oluşturmaktadır. Araştırmacılar; monomer ebatlarının; polimerizasyonu tamamlanmayan monomerlerin miktarının ve ağız ortamının çözücülük kapasitesinin ayrışmada etkili olduğunu bildirmişlerdir [3].
RBK’den salınan monomerlerin toksik etkileri araştırılmış ve Bis-GMA, TEGDMA ve UDMA monomerlerinin yüksek toksisite potansiyeli olduğu bildirilmiştir [4][5]. Araştımacılar; reaksiyona girmemiş TEGDMA, Bis-GMA, ve UDMA’yı; sitotoksik, genotoksik, mutajenik ve alerjik etkiler gösteren toksik maddeler olarak tanımlamışlardır [4][6]. Ayrıca, marjinal boşluğa kolonize olan karyojenik mikroorganizmaların çoğalmasını da artırdıkları bildirilmiştir [7]. İlk olarak sentetik bir östrojen olarak geliştirilen BPA, bir endokrin yıkıcıdır. Doğal hormonlar gibi etkileşime girerek hücre hormon reseptörlerini bloke eder [4]. BPA ayrışmasının, polimerizasyonun tamamlanamamasından ya da zamanla rezin bütünlüğünün bozulmasından kaynaklandığı bildirilmiştir [8].
Monomerlerin polimerlere dönüşüm miktarı; monomerlerin kimyasal yapısı ve reaksiyon başlatıcıların konsantrasyonu gibi içsel faktörler ile polimerizasyon koşulları gibi dışsal faktörlere bağlı olarak değişebilir [9]. RBK’lerde kütleye yeterli ışın ulaşması polimerizasyonun istenen şekilde tamamlanabilmesi için çok önemlidir ki konvansiyonel RBK’lerde bu derinlik 2 mm olarak önerilmektedir [10]. Daha derin kavitelerde RBK’in uygulanabilmesi için inkremental teknik uygulanması zorunludur. Bu teknikte tabakalar arasında boşluk kalması, kontaminasyon riski, dar kavitelere uygulama zorluğu ve zaman kaybı gibi sebepler [11] araştırmacıları yeni arayışlara itmiş ve Bulk-Fill kompozitler (BF) geliştirilmiştir.
BF’ler daha büyük kütlelerde (4mm) kaviteye yerleştirmeye izin veren yeni geliştirilmiş RBK türevidir. Üretici firmalar bu durumu daha translusent doldurucular ve daha fazla fotoiniatorle açıklamaktadır. Bu materyalin kullanılması ile polimerize olmamış monomerlerin sayısının azaltılabileceği düşünülse de dönüşüm derecesi konvansiyonel RBK’den daha düşük bulunmuştur [12].
Yine son zamanlarda geliştirilmiş bir RBK türevi olan nanohibrit kompozitler (NH), yapısında makro ve mikro boyutlu partiküller ile nano-boyutlu dolgu materyallerini içerir. Nano ölçekli doldurucular kütle içerisindeki boşlukları doldurarak kütlenin daha az boşluklu olmasını sağlar. Bu nano doldurucular ışığın dalga boyutundan daha küçük bir partikül büyüklüğüne sahiptir ve NH’ler makro dolduruculu kompozitlerin mekanik özellikleri ile mikro dolduruculu kompozitlerin optik özelliklerinin kombinasyonunun sağlanması amacıyla üretilmiştir [13].
Dental ağartma işlemleri hastaların yüksek estetik beklentileri sonucunda günümüzde oldukça popüler hale gelmiştir. Ev tipi ve ofis tipi olmak üzere iki şekilde uygulanan ağartma ajanları yapısında bulundurdukları Hidrojen Peroksit (HP) ya da Karbamid Peroksit (CP)’in farklı formları ve konsantrasyonları ile hastalara uygulanmaktadır [14]. CP ağartma esnasında tükürük ile karşılarsa reaksiyona girerek HP ve üre açığa çıkartır. Bu kimyasal reaksiyon sonrası açığa çıkan serbest radikaller, rezin kompozitlerin hidrolitik bozunmasını hızlandırarak ya da doldurucu-matriks bağlantısını etkileyerek monomer salınımını artırabilir [15].
Salınan monomerlerin türünü ve miktarını belirlemek ve polimer esaslı malzemelerin bozunmasını ve klinik kullanımdaki önemini anlamak için reaksiyon mekanizmasını bilmek gereklidir. Bu sebeple bu çalışmada ağartma ajanlarının farklı kompozitler üzerindeki etkisinin monomer salınımı açısından incelenebilmesi için 3 farklı tip kompozite 2 farklı tip ağartma ajanı uygulanarak monomer salınımı incelenmiştir. Daha önceki çalışmalarda NH’ler kendi aralarında [16] ya da konvansiyonel rezin kompozitlerle [17][18][19][20], BF’ler kendi aralarında[21][22][23] ya da konvansiyonel rezin kompozitlerle [24] ağartma ajanı uygulanarak ya da uygulanmadan karşılaştırılmış olsa da hiç bir çalışmada BF’ler NH’ler ile ya da üçü birbiriyle karşılaştırılmamıştır.
Bu çalışmanın amacı; sıklıkla kullanılan BF’ler, NH’ler ve konvansiyonel rezin kompozitlere ağartma ajanları uygulanarak monomer salınım miktarlarını ve türünü karşılaştırmaktır. Çalışmanın başlangıç hipotezi farklı rezin kompozitlerinin farklı ağartma ajanları uygulanması sonucunda monomer salınımının benzer olacağı ve salınımın zamanla değişmeyeceği yönündedir.
Translation - English 1. INTRODUCTION
Resin-based composites (RBC) are commonly used as they are easily applicable, enabling conservative work, and providing an aesthetic appearance. RBCs are essentially composed of an organic resin matrix, inorganic filler and silane agent that bonds the two parts together [1].
The commonly used monomers in resin matrix are bisphenol A-glycidyl methacrylate (Bis-GMA), triethylene glycol dimethacrylate (TEGDMA), urethane dimethacrylate (UDMA), and bisphenol A (BPA) [2]. The studies in the literature have shown that RBC content degrades in oral environment. This puts a question mark in the minds on the biocompatibility of the material. The researchers have reported that the dimensions of monomer, the number of monomers with incomplete polymerization, and the oral environment’s ability to degrade are effective factors in degradation [3].
The toxic effects of monomers released from RBC have been investigated, and Bis-GMA, TEGDMA and UDMA monomers were reported to have a high toxicity potential [4][5]. The researchers have defined unreacted TEGDMA, Bis-GMA, and UDMA as toxic substances with cytotoxic, genotoxic, mutagenic, and allergic effects [4][6]. Besides, these were also reported to increase the growth of cariogenic microorganisms that colonize the marginal space [7]. Originally developed as a synthetic estrogen, BPA is an endocrine-disrupting compound. It interacts like natural hormones and blocks the normal action of hormone receptors [4]. It has been reported that incomplete polymerization, or disintegration of resin over time leads to the degradation of BPA [8].
The amount of conversion of monomers to the polymers may vary depending on endogenic factors such as the chemical structure of monomers and concentration of catalysts and exogenous factors such as polymerization conditions [9]. Sufficient intensity of light in RBCs is important to achieve adequate polymerization, and this depth is suggested as 2 mm in conventional RBCs [10]. Applying an incremental technique is necessary for applying RBC in deeper cavities. In this technique, the reasons such as space between the layers, the risk of contamination, diffıculty in manipulation due to limited access in small cavities, and prolonged treatment time [11] have pushed researchers to new searches and Bulk-Fill composites (BF) have been developed.
BFs are newly designed RBC derivative that allows placing in the cavity in larger masses (4 mm). Manufacturers explain this situation with more translucent fillers and more photoinitiators. Although it is thought that the number of unpolymerized monomers can be reduced by using this material, its conversion amount was found to be lower than conventional RBC [12].
Nanohybrid composites (NH), a recently developed RBC derivative, contain macro and micro-sized particles and nano-sized filling materials. The nano-scale fillers provide less spaces in the mass by filling the spaces in the mass. These nano-fillers have a smaller particle size than the wavelength of light and NHs are produced to combine the mechanical properties of macrofilled composites with the optical properties of microfilled composites [13].
Dental bleaching treatments have become very popular today as a result of patients’ high aesthetic expectations. Being applied in two ways as at-home and in-office, bleaching agents are applied to patients together with different forms and concentrations of Hydrogen Peroxide (HP) or Carbamide Peroxide (CP) in its structure [14]. When CP is exposed to saliva during bleaching procedure, it reacts and gives HP and urea. Free radicals released after this chemical reaction may increase monomer release by accelerating the hydrolytic degradation of resin composites or by affecting the filler-matrix link [15].
It is necessary to know the reaction mechanism to determine the type and amount of monomers released and to understand the degradation of polymer-based materials and their importance in clinical use. In this study, therefore, monomer release was investigated by applying two different types of bleaching agents to three different types of composites in order to examine the effect of bleaching agents on different composites in terms of monomer release. Although previous studies have compared NHs among themselves [16] or with conventional resin composites [17][18][19][20], and BFs among themselves [21][22][23] or with conventional resin composites [24], with or without applying bleaching agents; no study has compared BFs with NHs, or three of them with each other.
This study aims to compare the amount and type of monomer releases by applying bleaching agents to commonly used BFs, NHs, and conventional resin composites. The initial hypothesis of the study suggests that as a result of applying different bleaching agents of different resin composites, the monomer release would be similar and would not change over time.
