Die HHNA zeigt einen circadianen Rhythmus, aber spielt auch eine zentrale Rolle für die Stressantwort des Körpers, und ist deshalb für die Psychoneuroendokrinologie von zentraler Bedeutung. Bei Auftreten eines Stressors, wie dem plötzlichen Erklingen einer Sirene, wird im Hypothalamus CRH ins Blut ausgeschüttet, das an Rezeptoren des Hypophysenvorderlappens bindet. Dieser reagiert mit Freisetzung von ACTH, das wiederum über die Blutbahn zur Nebennierenrinde transportiert wird und dort an Rezeptoren bindet. Schließlich setzt die Nebennierenrinde als Folge Kortikoide wie Kortisol frei. Diese Hormone leiten die eigentliche Stressantwort, auch bekannt als Fight-or-Flight-Antwort, ein: Der Blutzuckerspiegel steigt, der Blutdruck wird erhöht, das Immunsystem wird weitestgehend unterdrückt und Verdauungsprozesse werden zurückgefahren. Dieser Feedforward-Kaskade stehen Feedbackprozesse gegenüber, um die Stressantwort zu regulieren: Über Bindung der Glukokortikoide an Kortikosteroidrezeptoren des Hypothalamus und der Hypophyse wird die weitere Hormonausschüttung in beiden Gehirnarealen schließlich unterbunden, was wiederum eine gesteigerte Freisetzung von Kortisol in den Nebennieren verhindert und die Homöostase wiederherstellt.

Quando si verifica un fattore di stress, come il suono improvviso di una sirena, il CRH viene rilasciato nel sangue all'interno dell'ipotalamo e si lega ai recettori nell'adenoipofisi. Questo reagisce rilasciando ACTH, che a sua volta viene trasportato attraverso il flusso sanguigno e raggiunge la corteccia surrenale, dove si lega ai recettori. Infine, la corteccia surrenale secerne ormoni corticoidi, come il cortisolo. Questi ormoni controllano la risposta allo stress vera e propria, anche nota come reazione "attacco o fuga" (fight-or-flight): i livelli di glicemia nel sangue si alzano, la pressione sanguigna aumenta, il sistema immunitario viene soppresso per quanto possibile e i processi digestivi si riducono. A questa cascata di feedforward si contrappongono i meccanismi di feedback nella regolazione della risposta allo stress: attraverso il legame dei glucocorticoidi ai recettori per i corticosteroidi dell'ipotalamo e dell'ipofisi, l'ulteriore secrezione ormonale in entrambe le aree del cervello viene inibita, il che a sua volta impedisce un aumento del rilascio di cortisolo nelle ghiandole surrenali e ripristina l'omeostasi.
