Nella domanda di ammissione, redatta in carta semplice, come da schema allegato al presente avviso (Allegato “A”), gli aspiranti devono dichiarare, sotto la propria responsabilità:
a) cognome e nome; data e luogo di nascita, residenza, codice fiscale, stato civile;
b) di possedere la cittadinanza italiana o di altro Stato membro dell’Unione Europea;
c) per i cittadini italiani: il Comune di iscrizione nelle liste elettorali, ovvero i motivi della loro non iscrizione o della cancellazione dalle liste medesime; per i cittadini degli altri Stati membri dell’Unione Europea: il godimento dei diritti civili e politici negli Stati di appartenenza o di provenienza;
d) le eventuali condanne penali riportate, ovvero di non aver riportato condanne penali. (La mancata dichiarazione al riguardo sarà equiparata, ad ogni effetto di legge, a dichiarazione negativa);
e) il possesso del diploma di laurea e degli altri titoli prescritti dal precedente art. 4 comma 1;
f) la posizione nei riguardi degli obblighi militari (per i candidati di sesso maschile);
g) l’indirizzo presso il quale deve essere inviata qualsiasi comunicazione inerente la selezione, nonché il recapito telefonico;
h) l’adeguata conoscenza della lingua italiana (per i soli cittadini degli altri Stati membri
dell’Unione Europea);
Ai sensi della L. 125/91 e dell’art. 57 del D.Lgs n. 165 del 30.03.2001 è garantita pari opportunità tra uomini e donne.

1.14.3 Atrial septal defect is one of the most common congenital anomalies of the heart accounting for one-
quarter of all. Three-quarters are ostium secundum defects, one-fifth are ostium primum defects and one in 20 are sinus venosus defects.
1.14.4 The life expectancy with all but small (pulmonary/systemic flow ratio < 1.5:1) uncorrected secundum defects is not normal with an increasing risk of atrial rhythm disturbances including flutter and fibrillation from the fourth decade, and the eventual onset of right-sided heart failure in the sixth and seventh decades.
