Source text - English Current recommendations by the National Institutes of Health in a 2000 consensus statement suggest that "adjuvant hormonal therapy should be recommended to women whose breast tumors contain hormone receptor protein, regardless of age, menopausal status, involvement of axillary lymph nodes, or tumor size" (7). Tamoxifen is the most commonly used hormonal therapy, with data supporting a five-year course of therapy. In general, all older women with ER+ or PgR+ tumors should be considered candidates for hormonal adjuvant therapy. Adjuvant hormonal therapy is not currently recommended for ER-negative breast tumors (8).
Cytotoxic chemotherapy represents the second systemic therapy available to women in the adjuvant setting. The most common adjuvant chemotherapy choices include cyclophosphamide, methotrexate, and fluorouracil (CMF), and doxorubicin and cyclophosphamide (AC). Individual tumor characteristics (such as HER2/neu status) are currently under study in determining their influence on adjuvant chemotherapy choice. In addition, the use of agents such as taxanes and trastuzumab (a monoclonal antibody that targets HER2) in the adjuvant setting is currently under intensive investigation given the success of these agents in the advanced breast cancer treatment setting.

UU. recogidas en una declaración de consenso en el año 2000 sugieren que “debe recomendarse el tratamiento hormonal adyuvante a las mujeres cuyos tumores mamarios contengan la proteína del receptor hormonal, independientemente de la edad, estado menopáusico, afectación de los ganglios linfáticos axilares o tamaño del tumor” (7). El tamoxifeno es el fármaco usado más habitualmente en el tratamiento hormonal y se dispone de datos que respaldan su uso durante un ciclo de 5 años. En general, todas las mujeres de edad avanzada cuyos tumores tengan RE o RPg deben considerarse idóneas para recibir tratamiento adyuvante hormonal. En la actualidad no se recomienda el tratamiento hormonal para los tumores que carecen de RE (8).
La quimioterapia citotóxica representa el segundo tratamiento sistémico disponible para las mujeres dentro del marco del tratamiento adyuvante. Las opciones de quimioterapia adyuvante más habituales son la combinación de ciclofosfamida, metotrexato y fluorouracilo (CMF) y la combinación de doxorrubicina y ciclofosfamida (AC). Actualmente se están estudiado características tumorales individuales (tales como el estado HER2/neu) para determinar su influencia sobre la elección de la quimioterapia adyuvante. Además, se está investigando activamente el uso de otros fármacos, como los taxanos y el trastuzumab (un anticuerpo monoclonal que actúa selectivamente sobre HER2), en el tratamiento adyuvante dado el éxito de éstos fármacos en el tratamiento del cáncer de mama avanzado.
