Les allergies aux produits contenant du latex naturel ont fortement augmenté ces dernières années, surtout dans le secteur de la santé. Les allergies au latex peuvent être de type immédiat ou retardé. Les réactions immédiates, causées par des anticorps spécifiques de type IgE, sont de loin les plus fréquentes. La fréquence des sensibilisations au latex parmi le personnel de santé varie entre 3 et 15 %. On les observe aussi à l'extérieur de ce secteur, dans l'industrie du caoutchouc, les laboratoires, les salons de coiffure ou les entreprises de nettoyage. L'allergie au latex constitue un problème important pour la prévention des maladies professionnelles car elle est fréquente, les troubles qu'elle occasionne sont souvent importants et même potentiellement mortels et parce que le latex est omniprésent dans le cadre professionnel aussi bien que privé. C'est pourquoi il faut prendre des mesures préventives dans tous les établissements sanitaires et dans les entreprises où existent des contacts avec le latex.
Qu'est-ce que le latex?
Le latex, matériau de base des produits en caoutchouc naturel est extrait de la sève de l'arbre à caoutchouc tropical hevea brasiliensis. Cette sève contient essentiellement du polyisoprène et de l'eau. A l'état naturel, elle n'occasionne aucune atteinte à la santé. On a cependant pu identifier diverses protéines allergéniques dans la sève de cet arbre. La structure et la séquence des acides aminés de certaines d'entre elles ont été identifiées. Le grand nombre des divers allergènes contenus dans le latex rend difficile la fabrication d'un extrait utilisable pour le diagnostic et l'immunothérapie (désensibilisation).
Allergie de type immédiat
L'allergie au latex de type 1, provoquée par les IgE, peut être déclenchée par contact avec la peau ou les muqueuses, pénétration orale ou respiratoire dans la circulation sanguine. Il faut distinguer ici les allergies touchant les porteurs de gants, celles aux particules de latex aéroportées, ainsi que celles des patients qui, lors du contact avec les gants du médecin, peuvent présenter des réactions non seulement locales mais systémiques.
Le contact du latex avec la peau peut provoquer un syndrome urticarien (tableau). Le contact direct de gants en latex peut provoquer une urticaire localisée associant démangeaison, érythème, papules ortiées ou enflure diffuse de la peau. Les allergènes du latex libérés par la sueur peuvent apparemment pénétrer dans la peau par les follicules et être ensuite disséminés dans tout l'organisme par le flux sanguin. C'est pourquoi on peut observer également des urticaires généralisées, avec congestion des lèvres et des muqueuses (rhinoconjonctivite, asthme, oedèmes bucco-pharyngo-laryngés, troubles gastro-intestinaux). Des chocs anaphylactiques potentiellement mortels associent effondrement tensionnel et arrêt cardio-respiratoire.

In the last few years allergies to products containing natural rubber latex have increased considerably, especially in the healthcare sector. Latex allergies can either be immediate type or delayed type reactions. Immediate reactions are by far the most common, and are caused specifically by the Immunoglobulin E (IgE) antibody. The number of healthcare personnel with latex sensitivity varies between 3 and 15%. People with latex sensitivity are also found outside this sector, in the rubber industry, in laboratories, in hairdressing and in cleaning companies. Latex allergy is a major issue concerning occupational illness prevention as it is common, often bringing about significant and even potentially fatal problems, and also because it is found both in the work-place and at home. This is why it is essential that precautions are taken in all healthcare establishments and in companies where latex contact occurs.
What is latex?
Latex, the base material of natural rubber products, is extracted from the sap of the tropical rubber tree hevea brasiliensis. Principally this sap contains polyisoprene and water and in its natural state does not pose any health problems at all. However, it has been possible to identify a number of allergenic proteins from the sap of this tree, the structure and sequence of the amino acids from some of which have been determined. The wide range of allergens present in natural rubber latex has made the manufacture of an extract suitable for diagnostic means and immunotherapy (desensitisation) very difficult.
Immediate Type Allergy
Type 1 latex allergy, caused by IgE antibodies in the blood stream, can be triggered by contact with the skin or mucous membranes, through oral contact or inhalation. It is necessary to differentiate here between the allergies found in glove wearers (from airborne latex particles) and those of the patient, who through contact with the doctor’s gloves, can present with not only localised but systemic reactions.
In contact with the skin, latex can trigger contact urticaria syndrome (see table). Direct contact with latex gloves can cause localised urticaria associated with itching, reddening, and nettle rash or a diffused blistering of the skin. Latex allergens, released by sweat, can penetrate the skin through the hair follicles and are subsequently spread throughout the body by the bloodstream. This is why it is equally possible to observe generalised urticaria, with swelling of the lips and mucous membranes (rhinoconjunctivitis, asthma, oropharyngo-laryngeal oedema and gastro-intestinal problems). Anaphylactic shock, leading to an extreme drop in blood pressure and cardio-respiratory arrest can also occur. In severe cases this can be fatal.
