This refers to lowering the incidence of at-risk behavior, i.e. the frequency with which these actions occur.
Lowering or decreasing at-risk behavior is grammatically incorrect IMO, since you don't lower the behavior, you lower the number of incidents of that behavior.
You can use behaviors (pl.) too, but I'm not too keen on it.

I think your translation above was fine. I'm not sure "It is concluded" is the most natural-sounding way to translate that phrase though. "It is determined" might be better (see definition 4 below and in the linked reference). Just my $.02
