I am slightly confused about the above and its role in the following sentence:

Дана гр. [name of person in dative case], в том что на 06 ноября 2001 г. на ее имя в операционном отделе филиала 510 АСБ "Беларусбанк" имеется вклад "Победа" 1234567 в белорусских рублях на сумму 740 000 (семьсот сорок тысяча рублей).

Why is 'Дана' in the feminine form and what does it refer to?

Would it be better to change the order of the sentence and state it as follows:

I hereby certify that on 06 November 2001, a “Pobyeda” deposit, no 1234567 in the amount of 740 000 Belarusian roubles (seven hundred and forty thousand roubles) was made at the operational department of branch number 510 of Belarusbank to an account in the name of xxxxxxx.

Have I preserved the meaning of the original?
