Italian to English Law/Patents Certificates, Diplomas, Licenses, CVs certificato dell'anagrafe

the name of the office/department issuing a "certificato di residenza, cittadinanza e stato libero". No other context - it is just at the top of the certificate in question.

Any suggestions that don't sound like the translationese I am so tempted to use? And what on earth is coordinamento sociale when it's at home?
