piston huméral

French to English Medical Medical (general)
This is from physical therapy instructions after patient has had surgery to repair a torn rotator cuff:

- Apprentissage du placement actif de l'épaule (sonnette interne et piston huméral).

I'm not sure what kind of motion "piston huméral" is, thanks in advance for any insights.

Not standard, although I remember learning this as a student many moons ago as a frozen shoulder exercise.

Hi Hilary. Sorry for putting you on the spot ;-)

Sounds good to me. I haven't been able to find any English equivalent in documents on shoulder/rotator cuff rehab - certainly not a standard exercise in any case. From what I understand the goal is to recenter the humeral head in the glenoid fossa by strengthening pec maj, lat dorsi and teres maj.

SJLD's 1st reference is correct. It is a way of reeducating scapular dyskinesis. The patient is asked to retract the shoulder blades (sonnette interieure is medial or downward rotation), and then to push his/her elbow into the therapist's hand before working on shoulder abduction. This opens up the acromio-clavicular space. It works on increasing scapular stability and corrects the dyskinesis. I have double-checked with colleagues, we don't seem to have a particular term for "piston humeral", so maybe shoulder girdle retraction with resisted depression (pushing elbow down against resistance). Does this make sense?

I just texted my physical therapist, and she suggested "scapular pumping". I just sent her a few more details and am waiting for her response.

I realise that "sonnette interne" is not the same as "piston huméral", I was just sending in findings for both:)

Thanks Liz, but I think Piston Huméral is something separate. Sonnette interne is scapular rotation and pendulum exercises (common therapy practice) is in the next line (movements pendulaires). But thanks for your references!

First, the subjects learned humeral head lowering through passive abduction of the shoulder. This phase also focused on muscular control of the scapula, perception of passive humeral head lowering, active contraction of the pec major and latissimus dorsi, as well as, the perception of the humeral head lowering affect during active co-contraction of the pec major and latissimus dorsi.
