Bon de consignation/déconsignation

I don't know exactly how to translate these two terms in this context.

Avant d’effectuer ou de faire effectuer un travail, il y a lieu d’analyser s’il est necessaire de faire procéder à la consignation fluides et/ou électriques de l’installation considérée. C’est le rÔle du donneur d’ordre et/ou du chargé de consignations.

Dans ce qui suit le bon de consignation/déconsignation est dénommé « Bon vert »

lockout/removal of lockout certificate/card etc.

Lockout (also tagout if all that is done is affix a tag to a valve, switch, etc. informing people not to operate it) is an operation performed prior to maintenance on systems where there would be danger if someone were to turn apparatus on during the operation. There is bound to be some administrative papertrail attached to it. Whether this involves "certificates", "cards", etc. I don't know.

The best I know for "déconsignation" is "removal of lockout" (the AFNOR nuclear dict. gives "removal of administrative lockout"), but it would be nice if s.o. could find sth. snappier.

"Card" - and hence "green card" - will make it sound like a soccer match or US immigration.

Lockout card: A card indicating the procedure to be followed to lock out equipment. Each time it is used and equipment is locked out, the person responsible for the lockout signs the card and indicates the date of the lockout.

True, but I think "bon" is as much the question as "consignation". For me anyway!

Isolation PTW; disconnection (isolation) / reconnection ticket

In most cases, the order to isolate the facility to be worked on forms part of a permit to work (PTW). Quite often, PTWs requiring isolation differ in color from other PTWs (hence the "bon vert"); I've seen them called "isolation PTWs". If there is a separate document to be signed off upon isolation and reconnection, "ticket" would probably be the ticket ;-)

Commissioning / decommissioning note

I don't much like "removal of lockout certificate" - what about "withdrawal", or, as suggested above, "commissioning / decommissioning note"?
