today, the only treatment that *may* produce... is allogeneic SCT

currently = today, at this time

So the only treatment that may work now but is not guaranteed to work is allogeneic SCT.

the only one that might, in the future, prove to be

The treatment mentioned has not yet been demonstrated to be a "definitive treatment for advanced disease that can produce reliable durable remissions or curative results", but there is some evidence that it may prove to be so in the future. I.e. either more evidence is needed or it needs development. For the moment, there are no other candidates.

except perhaps

It simply means that SCT is the only therapy that may perhaps bring a cure. They're not sure, there's no conclusive proof of its efficacy, but at least it is the most promising therapeutic modality currently available.
