Alfaro-Quito-Pichincha

I am translating a Birth Certificate from Ecuador. The alignment is way off on the document, and it states:

nacido en Alfaro Quito
provincia de: Pichincha....el xxxxx de abril de

I though the the province was: Pichincha and the Canton was Quito; but with the "Alfaro" thrown in, now I am not sure what exactly the Alfaro goes with here? If it is part of the name of the Canton or the Province or exactly how to break down these three terms to state this on the certificate.....
I find many references to Alfaro in Ecuador, but none really clearly place it as a Canton by itself, or with Quito, or with Pichincha so that I am able to distinguish clearly between the three of them...

Anyone from Ecuador or who might be familiar with this subject.....I would really appreciate your opinion on this matter.... In any case, I must list all three, and state the geography breakdown of where this person was born in Ecuador here, so I have to have all three on whatever I decide to go with in this case.......

According to this Ecuador government site I found, and their system for coding locations, it would seem that 'Alfaro' is the 'parish' (parroquia?), Quito is the 'canton', and Pinchincha is the 'province'

I quote:

Ubicación Geográfica: Este campo contiene el código correspondiente a la provincia, cantón y parroquia en donde se ubica la dirección domiciliaria del empleado en relación de dependencia, de acuerdo a la TABLA 1. El código debe ser digitado igual como se presenta en la Tabla.

And here is the entry from Tabla 1 corresponding to your location:
