На самом деле, в бывшем советском медицинском образовании нет эквивалента для fellow. Это точно не интерн, т.к. его английский эквивалент - intern or resident. И я не согласна, что это аспирант, т.к. аспирант - это сугубо наука, он вообще может не заниматься клинической деятельностью, никакой дополнительной специализации он не получает. Ординатор - тоже не очень точный эквивалент, но это по-крайней мере, человек, уже имеющий право на самостоятельную работу (в отличие от интерна), и повышающий свою квалификацию по специальности.

In the North American system of medical education, a period of medical training following on from residency is generally called a fellowship. This refers to a period of several years during which the physician takes training in a subspecialty, such as cardiology or hematology/oncology. During this time, the physician is known as a fellow. Although their training is more advanced than that of residents, fellows usually continue to treat patients under the supervision of an attending physician - that is, one who has already completed a fellowship in the relevant subspecialty and is permitted to practice without direct supervision by other physicians. (Because they have completed their residencies, fellows may "moonlight" as attending physicians in the generalist field where they were trained, such as internal medicine or pediatrics, but they must be supervised by a fully trained attending when working in their subspecialty.) Most medical subspecialties have formalized fellowship programs that are accredited by the Accreditation Council for Graduate Medical Education (ACGME).
In England, these doctors are known as Senior House Officers (see second ref).

В США, medical intern младший чем fellow. В Англии, intern называется junior house officer. В России я не знаю!

Мне кажется, что в данном случае слово "practitioner" в равной степени относится и к "fellow", и к "nurse". Т.е. речь идет не просто о "fellow", а о "fellow practitioner". В таком случае, получается "группе, включающей лечащего врача, а также практикующего врача-коллегу или медсестру".

To be clear, after a general surgery residency a physician is fully qualified to perform general surgery independently. The fellowship is necessary for more specific training, such as pediatric neurosurgery.
