There are numerous afferent and efferent tracts associated with the hypothalamus, which serve to interconnect the hypothalamus with the pituitary, limbic system structures, the spinal cord and brainstem, and cortical areas. Most tracts are reciprocal, providing loops of information that mediate the complex functions of the hypothalamus and limbic system. Two major pathways carry a majority of this two-way traffic to and from the hypothalamus, the medial forebrain bundle, and the dorsal longitudinal fasciculus. The medial forebrain bundle is a large fiber bundle that passes through the lateral hypothalamus, traveling rostrally and caudally. It brings information to the hypothalamus from forebrain structures (including the ventral striatum) and the reticular formation of the midbrain and rostral pons and, in turn, sends hypothalamic efferents out to these same areas. The dorsal longitudinal fasciculus carries information between the hypothalamus and the reticular formation of the midbrain and rostral pons. It is important to note that lesions to specific hypothalamic pathways are not common, and most hypothalamic dysfunction results from lesions in the hypothalamus itself or in structures or brain areas with which the hypothalamus is connected. However, a discussion of the major pathways is helpful in understanding the types of information that reach the hypothalamus and the targets to which the hypothalamus projects. An understanding of these inputs and outputs will facilitate an understanding of hypothalamic function.
