I know these both mean care, but I am not sure what the difference is between them and I am not sure how to translate them when they come together in a sentence. I thought of "care and attention", but I don't think that seems right.
Example: Er der brug for meget pleje og omsorg kan der ydes hjælp fra hjemmehjælpere

Just use care, it covers both concepts of the Danish word.

: True to a point, but it is a very wide concept. 'Pleje' is (also) the professional kind provided by the 'hjemmehjælpere', while 'omsorg' is what the family and friends provide. They overlap, but there is a difference.
