Right fourth (IV) nerve palsy

I think IV is the 4th cranial nerve affecting vertical eye movement and D is short for right.

agree Lirka : Agree, but my choice of wording would be "right trochlear nerve palsy" (I would not use 'right-sided'; perhaps "on the right" would be ok, too, but simply saying 'right' is better, IMO)

Yes, I had doubts on whether just saying "right" would be understable enough here but I guess it is

neutral Sue Davis : I think right refers to the right nerve, rather than right-sided. It is usually referred to as the fourth cranial nerve (or trochlear nerve, or cranial nerve IV)
